What this means for you: When you open a Card Account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see a copy of your driver’s license or other identifying documents. You must activate the Card before it can be used. You may activate the Card by contacting XXXX.xxx through its mobile application, website at xxx.xxxx.xxx, or by calling 000-000-0000. You will need to provide personal information in order to verify your identity for Card registration. You will not receive a Personal Identification Number (“PIN”) with the Card. However, you will be prompted to select a PIN when you activate the Card. See the activation instructions in the “Activate The Card” section. You should not write or keep your PIN with the Card. Never share your PIN with anyone. When entering your PIN, be sure it cannot be observed by others and do not enter your PIN into any terminal that appears to be modified or suspicious. If you believe that anyone has gained unauthorized access to your PIN, you should advise us immediately following the procedures in the paragraph titled “Your Liability for Unauthorized Transfers.” You are responsible for all authorized transactions initiated and fees incurred by use of the Card. If you permit another person to have access to the Card or Card number(s), we will treat this as if you have authorized such use and you will be liable for all transactions and fees incurred by those persons. You are wholly responsible for the use of the Card according to the terms and conditions of this Agreement. You may not request an additional Card for another person. By activating the Card or by retaining, using or authorizing the use of the Card, you represent and warrant to us that: (i) you are at least 18 years of age (or older if you reside in a state where the majority age is older); (ii) you are a U.S. citizen or legal alien residing in the fifty (50) states of the United States (“U.S.”) or the District of Columbia; (iii) you have provided us with a verifiable U.S. street address (not a P.O. If we are unable to verify your identity, we may, at our sole discretion, (i) permit you to activate and use your Card to spend down any Available Balance on your Card, (ii) issue you a refund check for any Available Balance on your Card, or (iii) require further investigation. You will not be able to reload your Card and may not be able obtain cash or use your Card internationally until we are able to verify your identity.

This means, before opening a Card Account and the Deposit Account (which is governed by the Deposit Account Agreement further below), you are required to provide us your legal name, valid U.S. mailing address and residential address (if different), valid email address, your Phone number(s), your Social Security Number (SSN) or Individual Tax Identification Number (ITIN), your date of birth, your Passport, and other information that we request in our sole discretion that will help us to identify you. You must tell us when this information changes. We may ask you for additional documents to verify any changes. We may restrict or close your Card Account and Deposit Account if we cannot verify your information, or if you do not provide it as requested. To open a Card Account and Deposit Account with us, you must be a U,S, citizen, lawful permanent resident, or non-permanent resident alien on a valid, long-term visa in the United States, the District of Columbia, Puerto Rico, American Samoa, Guam, Xxxxxxxx Islands, Northern Mariana Islands, Palau and U.S. Virgin Islands. Additionally, you must be a consumer who isat least eighteen (18) years of age, and with a valid Social Security Number, Tax Identification Number or ITIN or passport. All deposits and withdrawals must be in U.S. dollars. The Card Account and Deposit Account are individual accounts and must be owned and titled in the name of one person who will be responsible for the accounts. The account cannot be owned or titled jointly. Authorized users other than you are not permitted, but you are responsible for use of the Card Account or Deposit Account by others as set forth in the Smartcard Agreement. In order to obtain a Card Account and Deposit Account, you must also agree to go paperless. This means that you must: (1) provide us with a valid email address, (2) verify you have the technical capability to transact with us electronically, (3) intend to transact with us electronically, and (4) agree to accept electronic delivery of all communications that we need or decide to send you in connection with your Card Account and Deposit Account. If you fail to provide accurate information that we request, we may cancel your Card. In addition, funds tied to suspected illicit or illegal activity may be subject to both internal and potentially federal investigation. If you falsify, misrepresent, or fail to provide requested information, we and/or PayPal may refuse to approve your request for the Debit Card, cancel any of your previously issued Card(s) and/or restrict and/or close your Card Account.
