Insurance Rebate. The short-term sick leave plan shall be registered with the Unemployment Insurance Commission The employee’s share of the Employer’s unemployment insurance premium reduction will be retained by the Hospital towards offsetting the cost of the benefit improvements contained in this Agreement. Any dispute which may arise concerning an employee’s entitlement to long-term disability benefits, and which is not covered by the appeal mechanism provided for under the policy of insurance, may be the subject of grievance and arbitration under the provisions of this agreement.
Insurance Rebate. 12.8.1 Eligible teachers who choose to forego their right to coverage shall receive an "opt out” benefit in the amount of twelve (12%) percent of the annual premium cost for the most expensive Consortium PPO alternative for which the eligible teacher was eligible to be paid during the month of September if the coverage is not used during the entire preceding year. Eligible teachers who forego their right to coverage but are receiving health insurance coverage from the Consortium as part of family coverage are not eligible for any "opt out" benefit.
12.8.2 Eligible teachers who choose to forego dental or vision coverage for a full year of a contract year shall receive a rebate for the benefit in the amount of twelve (12%) percent of the Board's annual premium cost of that insurance benefit waived.
12.8.3 Eligible teachers must inform the Board Treasurer by August 10th of each contract year as to the insurance option they are selecting for rebate, if any.
12.8.4 If an eligible teacher is hired at any time after August 10th, the eligible teacher shall be afforded insurance rebates on a pro-rated basis.
12.8.5 Eligible teachers shall have the right to enter any insurance options for which they are eligible during the year, but shall forfeit the right to rebate payment for that year.
Insurance Rebate. If any insurance premium costs decrease, or if any premium costs remain static and the district receives a distribution resulting from a credit, dividend, refund, or similar mechanism for the period beginning October 1, 1997, through September 30, 2011, the amount of the distribution which is the classified bargaining unit’s share at the time of the cost decreases or distribution shall be placed in an account to be used solely for the purpose of offsetting future premium increases until the account is exhausted.
11.4.1 The amount of the distribution as defined above, shall be used to offset increases for health/medical insurance, dental insurance, life insurance, and income protection insurance through September 30, 2012, unless the account is exhausted an earlier date.
11.4.2 Insurance premiums in effect on October 1, 2012, will be used as the base from which to determine increases in premium cost from which to draw against the account.

The Hospital shall pay the full cost of any medical certificates required of an employee. Pay for sick leave is for the sole and only purpose of protecting the employee against loss of income when he is legit- imately ill.

In order to qualify for sick leave, an employee must notify his supervisor, or, in his absence, the switchboard operator at least one hour prior to the beginning of the employee's working day if on the A.M. shift, and two hours if on the shift, and the Employer reserves the right to require proof of illness by medical certificate or such other form of proof as the Employer may If the employee has found it necessary to leave a message with the switchboard operator, she will call the person responsible for staffing prior to her next regularly scheduled shift. In cases of prolonged illness, the above stated medical certificates must verify the ability of the employee to return to work and perform their normal duties. An employee who is absent for whatever reason and whose date of return to work is not established, shall give her supervisor, or in their absence, the switchboard, hours notice of their intention to return to work. Failure to do so will result in the employee being sent home without pay. Upon the death of an employee at any age, the family or designated beneficiary of the employee may claim of the sick leave credits accrued to an employee at the date of death. Employees who retire under the of Hospitals of Ontario Pension Plan regardless of length of service will be permitted to cash out of their sick leave bank. The Hospital shall pay the full cost of any medical certificates required of an employee.
