License Application. Iowa West shall, each year, or as otherwise required, promptly and timely apply to the Commission pursuant to Chapter 99F of the Iowa Code as a sponsor for a license to conduct gambling games. Ameristar shall, each year, or as otherwise required, promptly and timely apply to the Commission for a license to operate an excursion gambling boat under Chapter 99F of the Iowa Code.
License Application. 2.1 WGP will promptly make an application to the appropriate authorities in Illinois for a license to construct and operate a marijuana cultivation and growing facility. WGP will use its best efforts to have the license granted. WGP will promptly apprise AmeriCann of any communications it receives from applicable Illinois agencies or authorities, and will forward to AmeriCann, within two days of receipt, copies of any communication it receives from any person relative to the license application.
2.2 WGP will notify AmeriCann (via email), within two days of receiving notification that its license application has been accepted or rejected.
2.3 Within seven days of receiving notification that its license application has been accepted, WGP must notify AmeriCann of its election to proceed to close the Loan. If WGP does not notify AmeriCann of its election to proceed to close the Loan, WGP must pay AmeriCann a breakup fee of $150,000, which will be payable to AmeriCann via wire transfer upon the expiration of such seven day period, plus $47,600 per month for twenty-four months.
2.4 If WGP does not notify AmeriCann of its election to close the Loan, the deposit will be returned to AmeriCann.
2.5 If a license to construct and operate a marijuana cultivation and growing facility has not been granted to WGP by January 31, 2015, then AmeriCann, at any time thereafter, may at its sole option, terminate this Agreement by written notification to WGP. Upon such termination, the deposit will be returned to AmeriCann.
2.6 If WGP’s license is rejected, but WGP decides to reapply for a license, WGP will give AmeriCann notice of such fact, and the provisions of Sections 2.1 through 2.5 will apply to any new license application.
License Application. Subject to Landlord's responsibility for construction-related matters as stated in Paragraph 2(b) above, Tenant shall be responsible, at its sole cost and expense, for obtaining the License Amendment from the appropriate state assisted living licensing agencies as required for Tenant to operate the assisted living and Alzheimer care portions of the Addition and, in connection therewith, for preparation and submission of any and all applications and other materials required by such licensing agencies, together with payment of any and all application fees, charges and expenses related thereto. Landlord agrees to cooperate fully with Tenant in connection therewith.
License Application. The pending license application (File No. XXXX-000000XX) shall have been granted.
License Application. 7.1 Party B is responsible to complete relevant formalities such as business license and tax registration certificate etc necessary for operation voluntarily and assume the expenses caused thereof.
7.2 If Party B encounters obstacles (not fault of Party A) in the process of completing license and permits, resulting in being unable to or unnecessary to use the leased premises, and it proposes to rescind this Contract by this reason, its liability for breach of contract shall not be exempted.
License Application. A person shall file an application for licensing in the name of the licensee with the department on forms specified by the department. The application shall contain the fol- lowing:
(a) For a U.S. applicant, the federal employer identification number, or in the case of a sole proprietorship, the social security number. For a Canadian−based applicant, the department shall assign account numbers according to a uniform numbering sys- tem.
(b) Owner’s, partner’s or corporate name.
(c) Legal business name if different than par. (b).
(d) Physical location of the business.
(e) Mailing address of the business.
(f) Signature of licensee or authorized representative or attor- ney in fact.
(g) Number of decals required by licensee.
(h) Application fee of $15.00.
(i) License fee of $3.00.
(j) Decal fee of $2.00.
(k) Statement of existence of bulk storage in Wisconsin and other international fuel tax agreement member jurisdictions.
License Application. For each small wireless facility, LICENSEE shall submit an application to LICENSOR for a supplemental license and required permits that includes:
a) Site specific structural integrity and, for a municipal utility pole, make-ready analysis prepared by a structural engineer, as that term is defined in Section 4 of the Structural Engineering Practice Act of 1989;
b) The location where each proposed small wireless facility or utility pole would be installed, and photographs of the location and its immediate surroundings depicting the utility poles or structures on which each proposed small wireless facility would be mounted or location where utility poles or structures would be installed. This shall include a photo simulated depiction of the completed facility;
c) Specifications and drawings prepared by a structural engineer, as that term is defined in Section 4 of the Structural Engineering Practice Act of 1989, for each proposed small wireless facility covered by the application as it is proposed to be installed, to include dimensional details of small wireless facilities and mounting hardware used to attach equipment to the utility pole or small wireless support structure;
d) The equipment type and model numbers for the antennas and all other wireless equipment associated with the small wireless facility;
e) A proposed schedule for the installation and completion of each small wireless facility covered by the application, if approved;
f) Certification that the collocation complies with the Collocation Requirements and Conditions stated in Section 7-6-4 of the South Barrington Village Code, to the best of the applicant’s knowledge;
g) Specific designation of any proposed small wireless facility as an “eligible facility request,” as that term is defined in the Federal Telecommunications Act, 42 USC §1455;
h) A Concealment Element Plan, if required by Section 7-6-13 of the South Barrington Village Code;
i) All other documentation and materials identified in Section 7-6-4 of the South Barrington Village Code; and
j) The application fee due.
License Application. Numeral 9 modified by Article 14 of Agreement 3-2006. Every person intending to enter the business of Investment Manager in or from the Republic of Panama shall request the issuance of the corresponding license to the Commission, for this purpose must present an application document containing the accurate information and documentation in order to verify if said person complies with the requirements for the issuance of said license and for operating the business in accordance with Decree-law 1 of 1999 and the present Agreement. The application for the License of Investment Manager shall contain the following information:
1. Corporate name of the applicant
2. Commercial name in the event it differs from the inscribed name.
3. Data about the inscription of the company
4. Legal domicile of the applicant
5. Correct address of the premises where operations will be held and of any branches, if any.
6. Authorized social capital and total patrimony paid of the company.
7. Personal Identification of the person or persons appointed as Main Executives (a minimum of one person), who must possess the license of Main Investment Executive. In the event that the person designated as Main Executive of the Investment Manager is not the owner of the License to that position, his designation will only be acceptable if he has already approved the examination required for the same on the date of the application. In that case, the formality for both the Main Executive of the Investment Manager License and the License for Investment Manager can be done at the same time, but the issuance shall be conditioned to the issuance of the latter.
8. Personal identification of the person appointed as Compliance Officer, who shall possess a Main Executive License. In the event that the person appointed as Compliance Officer is not the owner of the license, his designation will only be acceptable if he has already approved the examination required for the same on the date of the application. In that case, the formality for both the Main Executive of the Investment Manager License and the License for Investment Manager can be done at the same time, but the issuance shall be conditioned to the issuance of the latter
9. Estimate date to begin operations in accordance to the dispositions contained in Article 71 of this Agreement.
10. Names of the owners who have control over the Investment Manager and submittal of the share composition of the requesting Company on Form DM1-3 which is enclosed as...
License Application. Every person willing to obtain the license of Main Executive of an Investment Manager shall submit an application providing the following information:
1. Full name, nationality, personal identification number of applicant. In the event the applicant is a foreigner, copy of the document certifying the residence status in the Republic of Panama. And work permit issued by competent authority.
2. Place and date of birth of applicant.
3. Date on which the exams to apply for the license where passed.
4. Physical address, telephone, fax, e-mail address and mail address of the applicant.
5. Personal identification of the Investment Manager for whom he will be working with a brief description of the relationship with the regulated entity and its terms.
License Application. 4.1.1 The Customer shall provide Company a written notification of all proposed actions that impact service including, but not limited to, installation, replacement, reorientation, relocation, Material Changes or removal associated with the proposed or existing Attachment(s) utilizing the forms in APPENDIX II, Forms A-1 (Application for Street and Area Lighting Attachment License) and A-2 (Application for Street and Area Lighting Attachment License Detail). The Company shall perform an assessment and provide a response to the application based upon the proposed action(s), description and engineering/construction detail provided. 4.1.2 Proposed new underground sourced Attachments or modifications of existing Attachments for the purpose of Material Change of the Facilities, within or upon Structures may be authorized with Company approval. Authorized Attachments will comply with designated Company standards to facilitate appropriate ingress/egress of Facilities to Structures and assure compatibility of Facilities for the purpose of connections to Electric Distribution System. 4.1.3 The Company will make commercially reasonable efforts to accommodate Customer’s request for a Street and Area Lighting Attachment License. However, Company may, in its sole discretion, refuse to grant a Street and Area Lighting Attachment License or refuse authorization for the relocation, reconfiguration, Material Change of existing Attachments when Company reasonably determines that conditions including, but not limited to, the following exist:

(ii) The existing Facilities on the Pole or within the Structure may not be rearranged to accommodate the proposed Attachment changes, or
(iii) The proposed Attachments will negatively impact other customer services provided by Company. The list of above-mentioned conditions is not an exhaustive list and other conditions may exist that would require Company to refuse to grant a license.
