Notice of Nondiscrimination. GLSA shall take appropriate and continuing steps to notify Patients, GLSA personnel, and the public of the rights and protections afforded by the ADA, Section 504, and Section 1557, and the following:
(a) That it does not discriminate on the basis of race, color, national origin, sex, age or disability in its health programs and activities (Nondiscrimination Statement);
(b) The identity of, and contact information for the Civil Rights Coordinator, the responsible employee designated pursuant to Paragraph 24;
(c) The availability of the grievance procedure and how to file a grievance;
(d) How to file a discrimination of disability-based complaint with HHS and DOJ; and
(e) The terms of GLSA’s approved non-discrimination policy referenced in Paragraph 25 Within 45 calendar days of the Effective Date of this Agreement, GLSA shall provide photographic evidence that it has prominently displayed this Notice of Nondiscrimination in prominent, visible locations within all patient waiting areas at all GLSA locations, and to all interested persons by posting the Notice prominently on the GLSA website.
Notice of Nondiscrimination. By November 15, 2017, the District will submit to OCR for its review and approval its notice of nondiscrimination pursuant to 34 C.F.R. §106.9, which will notify students, parents/guardians, employees, and other relevant persons that it does not discriminate on the basis of sex in the education programs or activities that it operates and that it is required by Title IX not to discriminate in such a manner. The notice will state that inquiries concerning the application of Title IX should be referred to the Title IX Coordinator, and it shall include the name, title, office address, and telephone number for the District’s Title IX Coordinator. The District will publish this notice consistent with the requirements of Title IX at 34 C.F.R. §106.9. This notice will supplement Policy 103, Nondiscrimination in School and Classroom Practices, and Policy 248, Unlawful Harassment.
Notice of Nondiscrimination a. Consistent with the requirements of 34 C.F.R. §§ 106.8 and 106.9, the College will amend its notice of nondiscrimination to include the contact information for the Title IX Coordinator (name, title and contact information, including phone number, office and email address) and a statement that inquiries concerning the application of Title IX may be referred to the Title IX Coordinator or OCR and will publish the revised notice in all required locations, including but not limited to the College and Safe Place and Wellness Program (Safe Place) websites, in its promotional materials, the student and employee handbook, application forms and registration materials, student pamphlets, and other electronic and printed publications that provide information to employees, applicants, and students about the College.

The University notified OCR on January 11, 2018 that it had reviewed and revised its notice of nondiscrimination to meet the requirements of 34 C.F.R. §§ 106.8(a) and 106.9, including, but not limited to: ensuring that the notice of nondiscrimination includes the prohibition on the basis of sex for students and applicants for admission; and including a statement that inquiries concerning Title IX may be referred to the Title IX coordinator or to OCR and providing the name or title, address, and telephone number for the University’s Title IX coordinator.
Notice of Nondiscrimination. Within ninety (90) calendar days after the Effective Date of this Agreement, TBHC shall revise its proposed Notice of Nondiscrimination. The revised notice shall state that TBHC does not discriminate on any ground prohibited by Federal law, including gender-based discrimination; provide the process for filing and resolving grievances about discrimination; and provide the title, telephone number, email address, functions and office location of the responsible employee designated by TBHC pursuant to section V.E.2 of this Agreement. Within sixty (60) calendar days of approval by OCR, as provided in Section IV.J., TBHC shall take steps to notify Patients, Staff Members and Contractors of the information contained in the Notice of Nondiscrimination. This information shall be communicated by:
1. Posting signs in visible locations in TBHC facilities and on TBHC’s website and intranet system;
2. Including it in materials routinely given to patients at admitting/registration or at other times;
3. Including it in TBHC materials routinely available for take-away in patient waiting areas;
4. Including it in materials routinely given to employees at orientation.

The Mid-Del Public School District No. I-52 does not discriminate on the basis of race, color, national origin, sex, pregnancy, gender, gender expression or identity, religion, veteran status, sexual orientation, disability, age, or genetic information in its programs and activities and provides equal access to the Boy Scouts and other designated youth groups. The University will maintain its notice of nondiscrimination, including a statement that CWRU neither discriminates nor tolerates discrimination on the basis of sex in its education programs and activities. At a minimum, CWRU will publish this notice in all student and employee handbooks, athletics handbooks, and housing handbooks and manuals, as well as in online versions of the same. The full notice will be included on the CWRU Equity webpage, CWRU Greek Life webpage, and CWRU Athletics Title IX webpage. Online versions of the notice will include the Title IX Coordinator’s name.
B. Within 30 days of the date of this Agreement, the University will submit to the Department for review and approval its proposed notice of nondiscrimination and a list of where and when the notice or a link to this notice will appear (i.e., in which University documents, informational materials, and webpages). If the Department chooses to provide comments to these materials, the Department will do so within 30 days of receipt.

Applicants for admission and employment, students, parents of elementary and secondary school students, employees, sources of referral of applicants for admission and employment, and all unions or professional organizations holding collective bargaining or professional agreements with Independence Unified School District 446 are hereby notified that this institution does not discriminate on the basis of race, color, national origin, sex, age, or handicap in admission or access to, or treatment or employment in, its programs and activities.
