Notification of Sick LeaveAn Employee who may be absent from duty by virtue of being sick or disabled or because of an accident shall notify their immediate supervisor, nursing office or designated area of the Facility/Agency of such and the anticipated duration of such illness as soon as possible, preferably no less than one (1) hour prior to the commencement of her scheduled shift unless the lack of giving notification can be shown to have been unavoidable. Employees upon resuming duties will report to the Employee's immediate supervisor and/or designated area within the Facility/Agency.
Notification of Sick LeaveEmployees shall notify the Employer one-half (½) hour before commencement of shift if they are not able to report to work, and must continue to do so on a daily basis unless a doctor's certificate determines the period to be off for illness.
Notification of Sick Leave. In order to receive compensation while absent because of illness, injury, disability or special leave, the employee shall notify his/her immediate supervisor or designee prior to the start of the work shift, or as soon as practicable. The employee shall provide the reason for the absence and the estimated length of absence. If the length of the absence exceeds the original estimate, the employee shall promptly notify his/her supervisor or designee of the need for and length of additional leave.

Failure to follow these procedures will render the employee ineligible to use sick leave credits for the absence.

Notification of Sick Leave. 1) When absent due to sickness, an employee must notify his or her Department Head or other such person as the Department Head may designate with such notice to clearly indicate the employee is absent due to sickness. 2) An employee who is assigned to work normal working hours should notify management prior to the assigned starting time but, in any event, notice must be received by management within the first hour of the assigned work period. 3) An employee assigned to work on a shift should give management at least four (4) hours notice but, in any event, management must receive a minimum of one (1) hour's notice prior to the assigned starting time. 4) Failure to give notices required herein may entail a loss in sick pay.
