Notification to Supervisor. Any employee needing to be absent because of sickness or other physical disability shall notify the appropriate Department Director or immediate supervisor at least one day prior to such absence if circumstances permit, or as soon thereafter as possible. Any employee falsifying a reason for sick leave shall be subject to discipline, up to and including termination.

Notification to Supervisor. Each regular, full-time employee shall notify on-duty shift commander at least two (2) hours prior to the start of such employee's work shift that he/she is not able to work that day. Any deviation from the two (2) hour notification shall require justification by the employee to his/her supervisor. Should an employee become ill on the job, such employee shall only be assessed sick/health leave for the actual time missed, recorded to the nearest one-half (½) hour. Failure of any regular, full-time employee to provide proper notification to his/her supervisor shall be cause for denial of sick/health leave benefits, and such employee shall be subject to disciplinary action.

Employees scheduled for leave under this Clause shall be granted leave for the actual time required. Medical Absence Confirmation For sick leave of three (3) or less consecutive working days, a doctor’s sick slip may be required at the discretion of the management supervisor or designate; such confirmation must indicate day(s) the employee was unfit for work. For sick leave of more than three (3) consecutive working days, a doctor’s sick slip must be obtained and returned to the management supervisor or designate. If required, the doctor’s sick slip must be provided to the management supervisor in order for the employee to receive payment under the sick leave benefit plan. The Company will reimburse the employee for the cost of required doctor’s sick slip.
