OUTREACH SERVICES. The Transfer Agent shall provide the Outreach Services described below to assist the Fund in locating lost shareholders and re-establishing contact with inactive shareholders thereby reducing the number of escheated accounts. The Transfer Agent and/or its third-party subcontractor (the “Outreach Subcontractor”) shall provide the following Outreach Services: Identify all accounts where 'date of last contact' exceeds two years and include these in the Transfer Agent’s UPA database Mail contact letters to inactive accounts requesting the dealer and/or shareholder to contact Transfer Agent to keep their account in an active status. In order to capture contact, maintain a dedicated secure web site, a dedicated toll free number and letter barcoding for auto-indexing for the Federated Funds Identify all RPO accounts unresponsive to the two required SEC searches, accounts reflecting outstanding checks, and accounts that have been unresponsive to a contact mailing. Send a file of these accounts to the Outreach Subcontractor for discretionary search and research purposes to identify the shareholder as deceased. If the beneficiary is located, accounts will follow the Outreach Subcontractor's legal claimant process. Mail a confirmation letter for RPO accounts and outstanding checks to newly located address instructing owners to contact Transfer Agent to update their account Provide Standard Summary Reports to the Fund upon completion of the outreach services detailing the results of the effort.
OUTREACH SERVICES. The process of meeting basic needs, building trust, and establishing rapport with PATH-eligible Consumers who do not access traditional services, and help connect these Consumers with housing, community mental health, and other services. Outreach results in increased access to and utilization of community services by PATH-eligible Consumers who are homeless and have mental illnesses/co-occurring disorders.
a. Active Outreach is defined as face-to-face interaction with literally homeless people in streets, shelters, under bridges, and in other non-traditional settings. In active Outreach, workers seek out homeless individuals.
b. Outreach may include methods such as distribution of flyers and other written information, public service announcements, and other indirect methods. This type of Outreach is allowable but must be offered in conjunction with active Outreach activities.
c. Outreach may also include passive Outreach, also called “in-reach”, defined as when Outreach staff is placed in a service site frequented by homeless people, such as a shelter or community resource center, and direct, face to face interactions occur at that site. In passive Outreach, homeless individuals seek out Outreach workers. Passive Outreach is allowable but may not occur unless the PATH provider is also conducting active Outreach activities.
OUTREACH SERVICES. The CONTRACTOR will prioritize eligible individuals in the South Service Planning Area. The outreach and case management services required by this subdivision shall include, but not be limited to, working with homeless shelter providers and community organizations providing homeless services, as well as providing direct outreach in areas where persons experiencing homelessness congregate. CONTRACTOR shall actively conduct outreach to populations who are likely eligible for HDAP services (see Section 1. C. of this Scope of Services). CES assessments and enrollment in CES will be performed by the CONTRACTOR for:
a. All participants who enter CONTRACTOR’S shelter and navigation sites.
b. Individuals attending CONTRACTOR’S Alternative Sleeping Location’s new day program, if the Participant wishes to work on a housing plan.
c. Individuals encountered by CONTRACTOR’S Street Outreach team, if the Participant wishes to work on a housing plan.
d. HDAP services will be delivered at CONTRACTOR’S shelters, navigation centers, and interim and permanent housing sites by CONTRACTOR’S staff.
OUTREACH SERVICES. CONTRACTOR shall provide outreach services to homeless persons in COUNTY who have a mental illness, and may also have a co-occurring substance abuse disorder and/or are veterans, who could benefit from linkage to appropriate services.
OUTREACH SERVICES. Defined by HRSA/Xxxx Xxxxx Program Guidance; Outreach Services “are aimed at identifying persons with HIV who may know or be unaware of their status and are not in care.” Outreach Services cannot be delivered anonymously.
OUTREACH SERVICES. 18 Outreach activities shall raise awareness and educate the community regarding mental health 19 issues and available services and resources. CONTRACTOR shall focus on building rapport and trust 20 with individuals who may not be aware of available behavioral health services and would otherwise be 21 unserved or under served. CONTRACTOR shall utilize traditional and nontraditional outreach 22 methods/strategies to raise awareness, educate and reach individuals including but not limited to:
23 a. Public/Community Events Outreach: Events that are organized by other entities where 24 the public can attend and receive information about available services (i.e., health fairs, community 25 festivals).
OUTREACH SERVICES. See Section 3.2.9.3.1 of the ITN
OUTREACH SERVICES. The Provider shall ensure that a variety of appropriate outreach activities are designed to familiarize potentially eligible clients with available contract services, to explain the purpose of these services, and to facilitate access to these services. These activities may include, but are not limited to, the development of brochures, posters, media advertisements, and public announcements regarding meetings, workshops, or training. The Provider shall submit all materials to the contract manager for review and approval thirty (30) calendar days prior to publication and dissemination.
OUTREACH SERVICES. 1. Contractors outside of metropolitan Bakersfield shall deliver the Ongoing Ability for Self-Improvement Skills (OASIS) program to CalWORKs clients to assist clients in identifying and accessing services.
2. Contractor shall request OASIS schedule changes in advance. Requests for schedule changes shall be reviewed for approval by the BHRS designee.
3. Contractor shall provide Kern County Department of Human Services (DHS) with weekly copies of the sign-in sheets for CalWORKs Welfare-to-Work clients scheduled to attend the OASIS Workshop. Contractor shall contact DHS via telephone within one (1) business day if a scheduled client does not attend the first (1st) day of the OASIS workshop.
OUTREACH SERVICES. CONTRACTOR shall provide outreach services to the kinship community to inform FAMILIES of their options related to permanency for the children in their care, including information and assistance regarding guardianship and adoption.
