The Personnel Action Forms clause establishes the requirement for documenting employment-related changes or actions through standardized forms. In practice, this means that any adjustments to an employee’s status—such as hiring, promotions, salary changes, or terminations—must be formally recorded using designated forms, which are then processed and maintained by the human resources department. This clause ensures that all personnel actions are consistently documented, providing a clear record for compliance, payroll, and organizational tracking purposes.
Personnel Action Forms. The Employer shall also forward to the Union a copy of each Personnel Action Form (on employees within the bargaining unit) on the day such personnel action is approved by LSNYC.
Personnel Action Forms. Each academic employee shall receive a copy of any personnel action forms issued. Such personnel action forms shall state:
a. The appropriate step placement and salary or part-time rate.
b. The teaching load or other assignment as appropriate.
c. The beginning and ending dates of the assignment period.
d. The total number of days, hours or percent of workload as appropriate, of the assignment period.
Personnel Action Forms. The provisions of Division III, Section 12.6 of the District’s Administrative Code Personnel Policies and Procedures are incorporated herein, and made a part hereof by this reference. Proposed changes to this policy that are within the scope of bargaining will not be implemented without first obtaining the concurrence of the Association.
Personnel Action Forms. Personnel Action Forms (PAFs) shall be used to specify conditions of hiring, termination, pay, shift, or leave of absence. Reasons for the termination, change in status, pay, shift and leave of absence shall be noted in the forms by both the nurse and Employer whenever possible, and upon request, the nurse shall be given one copy of the form. All PAFs shall be maintained in the nurse’s personnel file. Nurses shall be able to print or be provided a paper copy of the completed form.

Full-time academic employees on leave must be notified by certified mail. Each individual employee who intends to return or commence employment in her/his respective tenured or probationary appointment shall so notify the College by signing the individual contract and returning it no later than fifteen (15) calendar days after its receipt.

This will not constitute an individual job contract and will conform with all other definitions and provisions of this Agreement. A copy of the Personnel Action Form will be given to the nurse and one will be retained in the personnel file. Any variation from the standard shift or pay rate will be considered a flexible shift under Section 8.6.

Upon request, a copy of the Personnel Action Form showing change in status, pay, shift or leave of absence will be made available to the employee at the time of the change. A new employee will be given access to the form specifying date of hire, and step for salary purposes.
