Physical Examination Requirements. All physical examinations shall be performed by medical doctors knowledgeable about the physical demands of fire fighting in accordance with NFPA 1582. Physical examinations shall comply with the minimum standards of the Nevada State Industrial Insurance System and the State of Nevada Occupational Safety and Health standard regulations. Pulmonary function tests shall be performed by certified technicians on a fully automated Spirometer and shall be interpreted by a physician. The stress ECG shall be performed in the presence of a physician, nurse or technician certified in Advanced Cardiac Life Support (ACLS) and evaluated by a board-certified cardiologist. All examinations shall be performed in one (1) facility. Audiograms shall be performed by certified personnel and shall be pure tone at frequencies of 250-6600 Hz. Audio testing shall be done according to Nevada OSHA regulations. SCBA mask fit and NIOSH rated particulate respirator mask fit testing shall be performed by a certified technician from the fire department life support division. Testing required: 1. Previous personal/history. 2. Standard core physical examination including breast, pelvic and digital rectal examination with a test for fecal occult blood. 3. Visual acuity measurements including near and distant vision, and color perception.

Physical Examination Requirements. A physical examination is required for all students entering Preschool, entering Kindergarten or upon transferring into Westport Community Schools. We also requi a physical exam in Grades 3, 6 and 9. If a physical is required for your child this school year, please send in a copy of their physical exam. Notices will be sent home if a copy of your child’s physical exam has not been received. The school physician is available late winter / early spring and will perform a physical exam if you wish. Vision Screenings are done in Preschool, Kindergarten, Grades 1-5, Grade 7 and Grade 10. Hearing Screenings are done in Preschool, Kindergarten, Grades 1-3, Grade 7 and Grade 10. Postural Screenings are done in Grades 5-9. Parents will be notified of any concern with a screening that necessitates a medical follow-up with your own physician or optometrist. A response letter with testing results should be forwarded to the Health O ce. Height, Weights and BMI are done in Grades 1, 4, 7 and 10 to monitor growth and development. Please notify the school nurse in writing by the end of September if you do NOT wish for your child to participate in the BMI screening. A request to not participate must be received each year that you opt out of the screening. If you would like a copy of your child’s BMI results please let the nurse know and a copy will be forwarded to you.
Physical Examination Requirements. Newly employed school employees in the Sandusky City Schools shall present documented evidence of having a negative tuberculin test within ninety (90) days before the first work day, or if a known positive reactor have a chest x-ray or other appropriate examination revealing the absence of tuberculosis in the communicable state within ninety (90) days before the first day of work. Prior to employment, all applicants for custodial, food service, maintenance, transportation, and supply support staff positions shall be required to submit a statement of employability from a licensed physician relative to the performance of the function and activities involved in the position for which the applicant is being hired. The required physical examination shall include such tests as found desirable for all employees or classification of employees, by the school physician. If an employee is required by state law to take a random drug test, then the employer shall pay for the test and for the time of the employee to report and take the test. The Board also agrees to pay for the cost of a physical examination. Whenever an employee’s health appears to be a hazard to children or others in the school system, the Superintendent may require a physical examination by the school physician. All required examinations shall be made at no cost to the employee, provided the services of the school physician are used. The school health service shall provide the chest x-rays, flu vaccine, and Hepatitis B vaccine for employees as routinely scheduled as long as it is available from the Erie County Health Department.

Physical Examination Requirements. All candidates must pass a physical examination conducted by an approved healthcare provider as outlined in N.J.A.C. 6A:16-2.2 (h). - Students who are disqualified by the school physician from participating in a sport may appeal a disapproval to the Superintendent of Schools. - Students who have lost an organ, limb or appendage, but who are otherwise qualified, may not be excluded from participating in contact sports. However, such students must appeal to the Superintendent of Schools, providing a statement of approval from the doctor most familiar with their condition and parents/guardians will be required to sign a waiver.

If the Family Leave is foreseeable, the employee must provide the agency/department with thirty (30) calendar days notice of his or her intent to take Family Leave. 2.
