Responsibilities of the Student. Conform to all rules and policies of Caruthersville High School and the work site. The student is expected to demonstrate honesty, punctuality, cooperation, courtesy and a willingness to learn.

Responsibilities of the Student. 1. The student shall compile a list of learning objectives in accordance with department guidelines that are mutually agreeable to CMU, the organization, and the student.
2. The student shall complete a CMU Waiver of Liability, and turn it into CMU, before beginning the internship.
3. The student shall report to the organization during times mutually agreed upon by the organization and the student.
4. The student shall complete a total of clock hours in performance of duties supporting the agreed upon learning objectives.
5. The student shall complete the Internship Experience Evaluation form provided by CMU at the end of the internship.
6. The student shall complete other program-specific requirements specified by CMU and attached to this contract.
7. The student shall register for Internship, credit hours and pay applicable tuition and fees to CMU. (Students must complete at least 45 clock hours for every 1 credit hour.)
Responsibilities of the Student. 1. The student agrees to abide by the FHWN’s Mission Statement in its current version and thus is obliged to adhere to proper academic manners and etiquette. Through their behaviour, the student will uphold the reputation of the FHWN, avoiding anything which would be detrimental in this regard.
2. The student agrees to assist the FHWN in the realisation of its mandatory quality management system and in particular, partake in course evaluation procedures.
3. It is the student’s personal responsibility to ensure that adequate financial means are available to finance their studies.

4. The student accepts the responsibility to take every possible precaution not to damage and thus preserve the condition of the infrastructure of the FHWN and to report any damage without delay to Study Services.
5. The student accepts the responsibility to disclose, in writing, personal data as required by law and to notify the FHWN immediately of any changes thereof during the term of the contract.
6. The student accepts the responsibility to notify the FHWN of any changes to their personal data as disclosed at the start of their studies, in particular changes of name, address, or employment status (in the case of part-time degree programmes). Should this not be complied with, any notification by the FHWN delivered to the last known address shall be deemed as delivered.
7. The student is required to report accidents immediately to the Head of Study Services using the necessary “AUVA Unfallmeldung für Studierende” form. This must be done at the latest within three days. In case of non-compliance, this may result in the loss of insurance claim benefits.
8. Should, during the course of a degree programme, internship(s) be required, the student is obliged to participate in the timely successful conclusion of the internship contract(s). If a contract cannot be closed through no fault of the FHWN, or if an internship cannot be successfully concluded in time, students are not entitled to any claims against the FHWN, e.g. if a student cannot provide the vaccination required for the internship.
9. At the start of their studies, the student is required to provide a police clearance certificate and agrees to a security check according to §§55 ff Sicherheitspolizeigesetz (applicable only to students of the Faculty of Security).

a) The Student will have passed relevant competencies/subjects that will allow the Student to operate at his or her assumed level of training;
b) The Student must satisfy all Occupational Health and Safety requirements of the Workplace Leaning Provider as advised by the WPLProvider prior to the Student commencing the WPL Program or prior to commencing a particular task.
c) Student must negotiate and agree to the start and finish dates for the WPL Program with the WPLSupervisor.

Responsibilities of the Student. School shall advise each participating student in writing that he or she is responsible for demonstrating professional behavior appropriate to the environment of the Facilities, including protecting the confidentiality of patient information and maintaining high standards of patient care and always providing patient services under the direct supervision of a licensed clinical professional of the Clinic. From time to time, students may be asked to make presentations to Clinic staff on best practices and advancements that the students are studying in their School physical therapy curriculum.

Stay enrolled at least on a half-time basis in the educational institution; Adhere to the same standards of conduct as regular employees; Understand their relationship with the Mission as being that of a guest to a host and that they are not considered Federal employees; Demonstrate the capacity to perform the assignments to be provided; and Ability to pass a thorough security and medical certification process.
Responsibilities of the Student. The student must adhere to all NASW Code of Ethics, BSW Practicum Manual requirements, BSW policies, and agency policies and procedures. Student must complete 450 hours in field placement (225 hours each semester). Students may not start hour’s early or complete hours early – students may begin on the first day of the semester and must complete on the last day of the semester (before finals week). Students must complete all required CMU paperwork in Handshake prior to starting practicum. Student may ONLY begin practicum after the Social Work Program and Social and Behavioral Sciences Department Head have APPROVED the agency, supervisor, and paperwork. The student must report to the practicum during times mutually agreed upon by the site supervisor and the student. The student shall register for Practicum I/II and Practicum Seminar I/II for credit hours and pay applicable tuition and fees to CMU.
