Taking of Personal/Xxxxx’s Leave. An employee may take paid personal/xxxxx's leave: (a) where the employee is not fit for work because of a personal illness, or personal injury, affecting the employee; or (b) to provide care or support to a member of the employee’s immediate family, or a member of the employee’s household, who requires care or support because of: (i) a personal illness, or personal injury, affecting the member; or (ii) an unexpected emergency affecting the member.

Taking of Personal/Xxxxx’s Leave. (a) An Employee may take paid personal/xxxxx's leave: (i) where an Employee is not fit for work because of a personal illness, or personal injury, affecting the Employee; or (ii) to provide care or support to a member of the Employee’s immediate family, or a member of the Employee’s household, who requires care or support because of: (A) a personal illness, or personal injury, affecting the member; or (B) an unexpected emergency affecting the member. (b) If an Employee has a need to take paid personal/carer’s leave they must: (i) notify their supervisor as soon as practicable on the first day they are absent from work (and wherever practicable, 2 hours before the commencement of their shift) or, if that is not practicable, as soon as practicable; and (ii) provide a medical certificate or other reasonable proof (including a Victorian or Commonwealth statutory declaration) of their need to take leave: (A) for all absences of 2 consecutive days or more; (B) for any absences, directly prior to a public holiday or non-rostered day, or directly after a public holiday or non-rostered day; and (C) for any other absences if they are asked to do so. (c) If an Employee takes paid personal/carer’s leave they will be paid at their base rate of pay for the ordinary hours they would have worked if they were not on leave.

Taking of Personal/Xxxxx’s Leave. An Employee may take Personal/Carer’s Leave: Where the Employee is not fit for work because of a personal illness, or personal injury, affecting the Employee; or To provide care or support to a member of the Employee’s immediate family, or a member of the Employee’s household, who requires care or support because of a personal illness, or personal injury, affecting the member, or an unexpected emergency affecting the member. The Employer recognises that an Employee who is personally affected by the illness or death of a pet may access Personal/Xxxxx’s Leave. Personal/Xxxxx’s Leave is paid at the Employee’s base rate of pay for the Employee’s ordinary hours of work in the period. Employees who are absent from work for more than 2 days for Personal/ Carers Leave are required to provide: A Medical Certificate from a Registered Health Practitioner or a Statutory Declaration in accordance with the NES; or, Where the Personal/Xxxxx’s Leave relating to a pet as outlined in clause 33.3, a certificate or letter from a Veterinarian, Specialist or Hospital registered with the Veterinary Practitioners Board of New South Wales. To minimise the impact of Personal/Xxxxx’s Leave on the Employer and its clients, Employees are required to take all reasonable steps to inform their Supervisor of their inability to attend work including, for Xxxxx’s Leave, the details of the name and relationship of the person for whom they are required to provide care. If possible, this notification should include an estimate of the duration of the absence.
Taking of Personal/Xxxxx’s Leave. (a) If an Employee needs to take (or needed to take) personal/carer’s leave (paid or unpaid) in accordance with this clause, the Employee must notify the Employer as soon as practicable. (b) The Employer may require that an Employee submit a medical certificate or statutory declaration for any personal/carer’s leave that the Employee takes (paid or unpaid), in accordance with the Act. (c) An Employee will be required to submit a medical certificate for any personal/carer’s leave (paid or unpaid) that the Employee takes where: (i) the Employee has taken two or more consecutive days of personal/carer’s leave; or (ii) the personal/xxxxx’s leave is taken on the day before or after a Public Holiday.

  • Variation of period of parental leave Unless agreed otherwise between the employer and employee, an employee may apply to their employer to change the period of parental leave on one occasion. Any such change to be notified at least four weeks prior to the commencement of the changed arrangements.

  • Personal/Carer’s Leave 18.1 Entitlement to paid personal/carer’s leave a) Paid personal leave will be available to an Employee (other than casual Employees) when they are absent due to: (i) personal illness or injury (sick leave); or (ii) for the purposes of caring for an immediate family or household member who is sick and requires the Employee's care and support (carer's leave). b) The amount of personal leave to which an Employee is entitled is as follows: (i) Upon commencement of employment Employees will automatically be credited with 5 days Personal and/or Xxxxx’s leave. After 6 months of employment, the leave will begin to accrue progressively up until it reaches 10 days at the conclusion of 12 months employment. (ii) Once the Employee has completed one year of continuous employment, the Employee shall be credited with a further ten days personal leave entitlement at the beginning of the Employee's second and subsequent year, which subject to clause 18.1(f) hereof, shall commence on the anniversary of engagement. c) In any year unused personal leave accrues. d) An Employee will inform the Company of the Employee's inability to attend for duty, and need to take personal leave, as soon as practicable. e) An Employee shall prove to the Company's satisfaction that the Employee’s Personal/Xxxxx’s leave is/was justified. Such evidence may be a medical practitioner’s certificate, or a statutory declaration. An Employee will not be required to provide such evidence for single days of absence but only where two or more consecutive days of absence are taken. f) If an Employee’s employment is terminated by the Company and is re-engaged within a period of six months, then the Employee's unclaimed balance of sick leave shall continue from the date of re-engagement. In such case the Employee's next year of service will commence after a total of twelve months has been served with that Company excluding the period of interruption in service from the date of commencement of the previous period of employment or the anniversary of the commencement of the previous period of employment, as the case may be. g) Unpaid carer’s leave will be in accordance with the NES. 18.2 Immediate family or household a) The entitlement to use personal leave for the purpose of carer's or compassionate leave is subject to the person being either: (i) a member of the Employee's immediate family; or (ii) a member of the Employees' household. b) The term immediate family includes: (i) a spouse, de facto partner, child, parent, grandparent, grandchild or sibling of the Employee; or (ii) a child, parent, grandparent, grandchild or sibling of a spouse or de facto partner of the Employee.

  • Personal Leave Written request for a personal leave of absence without pay will be considered on an individual basis by the Hospital. Such requests are to be submitted to the employee's immediate supervisor at least four (4) weeks in advance, unless not reasonably possible to give such notice, and a written reply will be given within fourteen (14) days except in cases of emergency in which case a reply will be given as soon as possible. Employees needing personal leave days for appointments with medical practitioners may utilize the personal leave language. Such leave shall not be unreasonably withheld.
