Verification of Illness. Written verification by an approved licensed medical practitioner or other satisfactory proof of illness or family illness may be required at the discretion of the department head.
Verification of Illness. Medical verification may be requested from the employee requesting sick leave. Where such is required, the employee shall be notified during the illness that such verification is required upon the employee's return to work.
Verification of Illness. The Board may require a physician's certificate as a basis for pay during leave after an absence of three (3) days of personal illness, or as it may deem necessary in other cases. (School Code: Section 24-6)
Verification of Illness. Employees shall verify any absence due to illness or injury that extends for a period of six (6) or more consecutive workdays by providing a written statement from the employee’s attending eligible medical or health care provider certifying illness or injury. If the period of absence is anticipated to extend beyond two (2) weeks, the written statement by the eligible medical or health care provider shall include an expected return to work date. Upon the member’s return to work, the District shall be provided a written statement containing a release from the eligible medical or health care provider stating that the employee is able to return to work with or without work restrictions. If work restrictions are specified as a condition of the employee’s return to work, written verification from the attending eligible medical or health care provider will be provided stating the extent and duration of the restrictions. An eligible medical or health care provider shall be considered to be any third party that is entitled to receive medical reimbursement from our insurance carrier.
Verification of Illness. When a bargaining unit member has been absent from work due to illness for a period of seven (7) consecutive workdays, the unit member shall be required to submit an attending physician’s verification of illness.
Verification of Illness. 12.2.1 The District may require written verification of the unit member’s or family member’s illness by the treating physician or practitioner whenever a unit member’s absence record shows chronic absenteeism or a pattern of absences immediately before or after weekends and/or holidays or whenever clear evidence indicates that absence is not related to illness or injury.
Verification of Illness. Xxxx leave absence in excess of 3 consecutive work days must be verified in writing by the employee's doctor. In lieu of such medical verification, the Department Head may accept the employee's signed statement concerning the nature of the illness. Supervisors may require a signed statement from the employee explaining the nature of the employee's illness in cases of sick leave absence of 3 days or less.
Verification of Illness. The District may request verification of illness any time the District has a reasonable basis to believe that the sick leave has been abused. Such verification, which is at the District Human Resources Department's discretion, may consist of written affidavit or a physician's statement when appropriate.

The Superintendent or the Superintendent's designee, may, at any time during the leave, require an employee to submit from the employee's physician, pertinent medical records and, at the District's expense, to be examined by a District-appointed physician, psychologist, or other professional to determine if the employee is disabled, mentally or physically, or ill to the extent of being unable to perform the normal daily duties and functions.
Verification of Illness. Normally a statement from the bargaining unit member shall be accepted as proof of illness, injury, or quarantine. The District may require a statement from the attending physician for absence due to illness, injury, or quarantine.
