Requesting Sick Leave. A. Employees must request and receive approval for use of sick leave from the designated or appropriate administrator in the Department as soon as possible after the onset of illness.
B. While supervisors must be informed of the general nature of an illness, such information shall be treated by the supervisor with appropriate confidentiality.
C. In the case of extended or chronic illness, the designated or appropriate administrator in the Department may require statements from a physician or dentist which includes the anticipated date of return. Upon request of the Employer, when the Employer has reasonable cause to believe that an employee has abused or is abusing sick leave, employees utilizing leave under this Article may be required to furnish a statement from a medical practitioner as defined in the Family Medical Leave Act stating that the practitioner finds the employee unable to work due to illness. Requests to furnish a statement from a medical practitioner may be oral or written. Oral requests shall be reduced to writing as soon as practicable. The written requests shall state the reason(s) for the request as well as the period of time that the employee will be required to furnish the statement. If an employee does not bring a medical practitioner's statement of illness when requested, the supervisor may deny the use of sick leave.
D. If an employee becomes ill while on vacation leave and presents satisfactory proof of illness or injury, the designated or appropriate administrator in the Department may approve the use of sick leave in lieu of vacation leave.

X. Xxxxx of sick leave shall be one form of just cause for disciplinary action. Abuse shall be defined as use of paid sick leave for reasons other than those listed in this Article. Use of paid sick leave in a pattern such as Mondays, Fridays, or the day after payday is an example of a use of sick leave that may constitute abuse.
E. A designated or appropriate administrator may require an employee to leave the workplace:
1) If the employee is unable to perform job responsibilities in an up-to- standard manner because of what appears to be a health condition; or
2) If the employee may expose others in the workplace to illness or disease. Such time shall be charged against sick leave, if available.
F. If an employee becomes ill while on vacation leave and presents satisfactory proof of illness or injury, the designated or appropriate administrator in the Department may approve the use of sick leave in lieu of vacation leave.
Requesting Sick Leave. Requests for use of sick leave shall be signed by the employee and subject to the approval of the department head. Requests to use sick leave credits for a death in the employee's immediate family shall be in accordance with the Funeral Leave section of the Leaves of Absence Article.

No sick leave shall be granted for the above reasons unless the employee reports to his/her supervisor the necessity for the absence no later than one (1) hour before his/her regular scheduled starting time. For an employee whose shift begins after 11:00 A.M., the employee must call his/her supervisor to report the illness by 10:00
Requesting Sick Leave. Whenever practicable, employees shall submit written requests for sick leave, on forms furnished by the Employer, in advance of the period of absence. Supervisors shall respond promptly and shall answer all written requests in writing. When advance notice is not possible, employees shall notify their supervisor by telephone or other means at the earliest opportunity. Because of the personal nature of an illness, it is agreed that while supervisors must be informed of the general nature of an illness, such information shall be used only for administrative purposes. Departments shall post the names and phone numbers of supervisors designated to grant sick leave.
Requesting Sick Leave. Except in emergency circumstances, employees should request sick leave in advance. The employee is responsible for notifying his/her supervisor or designee when he/she is prevented from reporting to work because of illness or injury. Such requests for sick leave shall be made as soon as possible and normally by 10 am on the day of the absence. When requests for sick leave are in excess of three (3) workdays, the supervisor may also require a medical certificate or other evidence as to the reason for an absence. Leave approving officials have the responsibility to determine that the circumstance of the absence justifies approval of the sick leave request.
Requesting Sick Leave. A. Sick leave requests (for medical, dental, or optical examinations) should be approved verbally, in writing, or in WebTA prior to leave being taken. Supervisors, or designees, will respond to leave requests in a timely manner, normally on the same day of the request.
B. In the event of an emergency situation, requests for leave should be submitted in WebTA as soon as possible. In these circumstances, interim verbal requests should be made using the following guidelines:
1. Unless previously instructed otherwise by the supervisor, the requesting employee must call and speak with his/her supervisor or designee or leave a voice mail message for the supervisor or designee;
2. Requests must be made within one (1) hour of the employee’s regular reporting time, or within one hour of the supervisor’s regular reporting time, whichever the supervisor designates;
3. The employee must specify that he/she is requesting the use of sick leave or family medical leave; and
4. The employee must specify the duration of the absence or call in every day of the absence.
Requesting Sick Leave. Earned sick leave will be granted to employees when they are incapacitated and unable to perform their duties provided that employees are not reporting for work due to circumstances described in applicable laws and regulations. In requesting sick leave, employees must furnish notice to the supervisor or the supervisor's designee as soon as possible prior to the start of the employee's shift, but no later than 1 hour before the start of the employee’s shift, unless emergency conditions preclude such notification.
Requesting Sick Leave. To the extent the use of such leave is foreseeable, requests for sick leave shall be submitted in advance using WebTA. When an employee is unable to request sick leave in advance because of unforeseen sickness, injury or other circumstances, the employee, if possible, shall make a reasonable effort to inform his or her immediate supervisor of the need for sick leave and the expected return-to-duty date by telephone no later than the beginning of the employee’s fixed schedule or the core period. If the supervisor is unavailable, the employee must use the call transfer procedure to talk to or leave a message with the person to whom the call is forwarded. The employee may be transferred to another supervisor by that person. Employees may be required for good and sufficient reasons to call in to report continued sickness or incapacitation on each day they are absent unless sick leave for a continued sickness or incapacitation has been previously approved.
