With Full Pay. (i) Critical Illness - Critical illness shall mean a life-threatening illness. For the critical illness or illness requiring admission to hospital intensive care unit of father, mother, husband, wife, son, daughter, brother, sister, parents of spouse and siblings of spouse - not more than five days or until death occurs. A statement will be required if death does not occur, signed by the teacher, signifying to the critical nature of illness, or the illness requiring admission to a hospital intensive care unit and the qualified medical practitioner's name, address and telephone number who may be contacted to verify the statement. The statement shall contain an authorization allowing the doctor to release information relating to the illness giving rise to the leave to the secretary-treasurer of the Board.

  • Bill Pay We will process bill payment transfer requests only to those creditors the Credit Union has designated in the User Instructions and such creditors as you authorize and for whom the Credit Union has the proper vendor code number. We will not process any bill payment transfer if the required transaction information is incomplete. We will withdraw the designated funds from your checking account for bill payment transfer by the designated cutoff time on the date you schedule for payment. We will process your bill payment transfer within a designated number of days before the date you schedule for payment.   • Cashing out annual leave The employee may, with the agreement of the employer, request in writing, to cash out up to two weeks of their annual leave during each 12 month period. Annual leave cannot be cashed out in advance of it being credited to the employee. Cashed out annual leave will be paid at the rate of pay that the employee receives at the time when the request is made.
