Application for Leaves of Absence. (a) Employees desiring a leave of absence with or without pay shall submit an application for such leave to their immediate non-bargaining unit supervisor and General Manager/Director for absences of less than twenty (20) working days (short term leaves) for Regular Full-Time Employees and less than four (4) consecutive weeks for Regular Part-Time Employees. For longer leaves of absence the request shall first be submitted to their Divisional Manager then to their Divisional Director/General Manager and then to the Municipal Administrator. In all cases each request will be judged based on the circumstances and merits of each application. The final and binding decision to grant short term leaves shall rest with the non-bargaining unit supervisor and the final and binding decision for long term leaves shall rest with the Municipal Administrator.

Application for Leaves of Absence. (a) Employees desiring a leave of absence with or without pay shall submit an application, in writing where possible, for such leave to the Administrator who shall make the decision based on the circumstances and merits of each application. The decision of the Admini- strator shall be final and binding.

Application for Leaves of Absence. Leaves of absence without pay may be taken for appropriate reasons upon mutual agreement of the Professional Employee and the President.   • Application of Agreement 4.1 This Agreement applies to:

The Executive agrees that [he] [she] has carefully read this Release and is signing it voluntarily. The Executive acknowledges that [he] [she] has had twenty one (21) days from receipt of this Release to review it prior to signing or that, if the Executive is signing this Release prior to the expiration of such 21-day period, the Executive is waiving [his] [her] right to review the Release for such full 21-day period prior to signing it. The Executive has the right to revoke this release within seven (7) days following the date of its execution by [him] [her]. However, if the Executive revokes this Release within such seven (7) day period, no severance benefit will be payable to the Executive under the CIC Agreement and the Executive shall return to the Company any such payment received prior to that date. THE EXECUTIVE HAS CAREFULLY READ THIS RELEASE AND ACKNOWLEDGES THAT IT CONSTITUTES A GENERAL RELEASE OF ALL KNOWN AND UNKNOWN CLAIMS AGAINST THE COMPANY UNDER THE AGE DISCRIMINATION IN EMPLOYMENT ACT. THE EXECUTIVE ACKNOWLEDGES THAT [HE] [SHE] HAS HAD A FULL OPPORTUNITY TO CONSULT WITH AN ATTORNEY OR OTHER ADVISOR OF THE EXECUTIVE’S CHOOSING CONCERNING [HIS] [HER] EXECUTION OF THIS RELEASE AND THAT [HE] [SHE] IS SIGNING THIS RELEASE VOLUNTARILY AND WITH THE FULL INTENT OF RELEASING THE COMPANY FROM ALL SUCH CLAIMS.   • Beneficiary Designation/Release of Information/Right to Reimbursement To the extent permitted by applicable Law and except as otherwise provided for in this Agreement, all beneficiary designations, authorizations for the release of information and rights to reimbursement made by or relating to SpinCo Employees under Company Benefit Plan shall be transferred to and be in full force and effect under the corresponding SpinCo Benefit Plan until such beneficiary designations, authorizations or rights are replaced or revoked by, or no longer apply, to the relevant SpinCo Employee.

  • Notice of variation of order of application The Agent may give notices under Clause 17.2 from time to time; and such a notice may be stated to apply not only to sums which may be received or recovered in the future, but also to any sum which has been received or recovered on or after the third Business Day before the date on which the notice is served.
