Certificate of Absence. All forms used for the certification of an absence shall be compatible with the terms and provisions of this Agreement. The member shall certify to the Superintendent the cause of the member’s absence. Such certification shall also constitute a request by the member for authorization of absence. The school office shall give to the member the appropriate form(s) to be used by the member for the certification of absence. The member shall complete and return the form(s) to the school office within one (1) work day after receipt of such forms. Failure to complete and return the form(s) in a timely manner may result in a delay in the issuance of pay. The building principal shall sign and forward to the appropriate offices the member’s certificate of absence within one (1) work day of receipt of such certification. Approval by the Superintendent of such request shall constitute an authorization of absence for duty. Members shall not be considered absent when on Board-sponsored/approved field trips. Members shall be responsible, however, for submitting an Attendance Variations Form for Modification of Worksite when on such field trips lasting one (1) school day or longer.
Certificate of Absence. On return from absence, the teacher shall return the completed form (Appendix D) to the designated office.
Certificate of Absence. Upon request, an employee will be provided a copy of the 7 Certificate of Absence on file with the payroll clerk. 8
Certificate of Absence. The employee shall certify to the employer the cause of the employee's absence. Such certification shall also constitute a request by the employee for authorization of absence. Approval by the employer of such request shall constitute an authorization of absence from duty. The employee shall enter the absence into the electronic reporting system utilized by the District within one (1) working day upon the employee's return.
Certificate of Absence. 1. An employee who is absent shall file the form provided for that purpose.
2. A teacher who is absent shall file, with the principal of the school, the form provided for that purpose.
3. Secretarial employees shall send the form to the Superintendent's office along with their weekly time sheets.
4. If an employee is absent, a doctor’s certificate may be requested by the Administration and shall be produced within five (5) days before any pay is deducted. If a doctor’s certificate is not presented, the employee consents to a payroll deduction for the absence. The Superintendent's discretion as to who is requested to provide a doctor's note shall not be subject to the grievance procedure.
D. 1. Any teacher who retires shall be entitled to pay for unused accumulated sick days. Such pay shall be at the rate of fifty dollars ($50.00) per day for a maximum of 150 days.
Certificate of Absence. In cases of suspected abuse, chronic sick leave use, modified duty assignments, or at the discretion of the City Manager, a physician's certificate may be required regardless of the length of absence.

Certificate of absence request shall be submitted in compliance with the printed regulations contained on the Attendance Variations Form (S-2j). The employee shall complete and return the Attendance Variations Form to the Department of Human Resources within one (1) working day upon the employee’s return. When an employee has been absent for more than five (5) consecutive work days because of personal illness, one (1) copy of the Certificate of Health (Form S-2e) shall be filed with the Department of Human Resources immediately upon the employee’s return to work. In the event that the absence is for ten (10) days or more, a “Certificate of Health” is to be filed at the end of each payroll period. Failure to file the “Certificate of Health” form will result in delay of compensation for sick leave days. This Certificate of Health shall be signed by the employee’s healthcare provider and shall list the name and address of the attending physician and the dates the physician was consulted. Nothing in this form shall be construed to waive the physician-patient privilege. The employer may inquire of the physician if such consultation did, in fact, occur. Falsification of a statement is grounds for suspension or termination of employment.
