Completion of Forms. The Company agrees to reimburse employees for the costs of medical forms and specialist reports when required by the WI and LTD carriers. This is agreed on the basis that the Company will be reimbursed by the carrier.

b. After a course has been completed, the employee must complete a “Request for Tuition Reimbursement” form. The following items must be returned with this form: approved “Request to Participate in the Tuition Reimbursement Program” form, tuition xxxx, grade report, class syllabus which contains the names of required textbooks, and a receipt for the cost of required textbooks. Requests for reimbursement should be forwarded to the HR department no later than ninety (90) days following the completion of the course.
