Completion Report. As promptly as possible after the Project Completion Date but in any event no later than the Financing Closing Date, the Borrower/Recipient shall furnish to the Fund a report on the overall implementation of the Project, in such form and substance as may be specified in the Financing Agreement or as the Fund shall reasonably request. At a minimum, such report shall address (i) the costs and benefits of the Project, (ii) the achievement of its objectives, (iii) the performance by the Borrower/Recipient, the Project Parties, the Fund of their respective obligations under the Agreement and (iv) lessons learned from the foregoing.
Completion Report. The Completion Report shall generally use the following format provided below for each Component or Project after completion. The Executive Summary should include a brief summary of project information and include the following items: • Brief description of work proposed to be done in the original Grant application. • Description of actual work completed and any deviations from Exhibit A. List any official amendments to this Grant Agreement, with a short description of the amendment. The following items should be provided, unless already submitted as a deliverable: • A copy of the Groundwater Sustainability Plan (GSP) that meets all the requirements of the GSP Regulations (for GSP Development Projects), or verification (e.g., acceptance email, or other approved documentation from SGMA), that the GSP was submitted to DWR as required. • A copy of any final technical report or study, produced for or utilized in this Project as described in the Work Plan • Electronic copies of any data collected, not previously submitted • Discussion of problems that occurred during the work and how those problems were resolved • Final Component schedule showing actual progress versus planned progress Additional information that may be applicable for Implementation Projects and/or Components includes the following: • As-built drawings • Final geodetic survey information • Project or Component photos A list showing: • Summary of Project costs including the following items: o Accounting of the cost of project expenditure o Include all internal and external costs not previously disclosed (i.e., additional cost share); and o A discussion of factors that positively or negatively affected the project cost and any deviation from the original Project cost estimate. • Benefits derived from the Component, with quantification of such benefits provided, applicable for Implementation Components. • A final project schedule showing actual progress versus planned progress as shown in Exhibit C. • Certification from a California Registered Professional (Civil Engineer or Geologist, as appropriate) that the project was conducted in accordance with the approved work plan and any approved modifications thereto. • Submittal schedule for the Post Performance Report.
Completion Report. 1. When reporting on the results of testing, the Monitor must:
i. Specify the procedures performed and the details of the samples selected;
ii. State the factual results of performing the procedures and not express an opinion on the results;
iii. Link the factual findings to the Deficiency being tested;
iv. List any new deficiencies in compliance with MFDA requirements that are noted during the testing on the original Deficiencies;
v. Indicate any restrictions or limitations on the Monitor’s ability to perform the procedures; and
vi. Provide recommendations to remedy any new deficiencies or any continuing Deficiencies.
Completion Report. The Grantee shall prepare and submit to the State a separate Completion Report for each project or component included in Exhibit A. The Grantee shall submit a Completion Report within ninety (90) calendar days of project/component completion. Each Completion Report shall include, in part, a description of actual work done, any changes or amendments to each project, and a final schedule showing actual progress versus planned progress, copies of any final documents or reports generated or utilized during a project. The Completion Report shall also include, if applicable for Implementation Project(s), certification of final project by a registered civil engineer, consistent with Exhibit D. A “Certification of Project Completion” form will be provided by the State.
Completion Report. 6.1 On or before the Completion Date, the Contractor shall submit the Completion Report to the Company.
6.2 The Company shall review the Completion Report and the Deliverables contained in it to ensure that the Services provided by the Contractor meet the requirements of this Agreement including the SOS.
6.3 In addition to the Completion Report, upon request by the Company, the Contractor shall submit to the Company additional documents detailing the results of the Services.
6.4 After the Company has examined the Completion Report, if it considers that the Completion Report complies with the requirements of this Agreement, the Company shall notice to the Contractor of the amount of the Service Fee payable in accordance with the Agreement (“Fee Notice”) .
Completion Report. The Recipient agrees that promptly following the completion of each Component of the Project, the Recipient shall execute and deliver to FACTOR a Completion Report in form and content satisfactory to FACTOR. This Completion Report shall include, but not be limited to, information certifying the Total Eligible Costs incurred and expended, and the Recipient’s explanation in narrative form of each variance between the budgeted costs and the actual costs incurred and shall identify each separate source of public funding. Completion Report. Caltrans will provide in writing to DTSC the following information within 180 days of completion of the construction phase for any highway project for which Xxxxxxxx managed lead-contaminated soil under this transition plan:
1. Project number;
Completion Report. Within 180 days of completion of the construction phase of any project for which this Agreement applies or is used, provide to DTSC, in writing, the following: 4.12.1
1. Project number;
2. Project description;
3. Project limits (District - County - Route - Begin Postmile /End Postmile);
4. Actual start and end dates of construction; and
5. A list of all USEPA ID numbers (including temporary ID numbers) assigned by DTSC for use on the project.
Completion Report. A final accounting and payment for material removed and a completion report must be submitted no later than 30 days following contract completion, or following termination of the contract by the seller or by operation of law. The completion report shall include a series of ground level photographs taken before, during, and after the extraction along with a statement confirming:
i) compliance with stipulations requiring the removal of personal property, restoration of the extraction area to a clean condition, and reclamation.
ii) accuracy of the photographs accompanying the report as depicting the site before extraction, during operations, and after completion and reclamation at the end of each contract period. Failure to submit a satisfactory report and/or required photographs subjects the site to a field inspection requirement for which the buyer may be assessed, at the Director’s discretion, either the actual cost incurred by the Division of Mining, Land & Water, or a minimum of $500.00. (11 AAC 05.130). Reimbursement for costs for the field inspection under this section may be taken from the performance guaranty.
