Flexible Benefits (Medical, Dental and Life) Insurance. The Hospital shall provide medical, dental and life insurance plans to all nurses working a minimum of sixteen (16) hours per week in accordance with the terms of those plans. Other costs such as deductibles, co-pays, co-insurance, out of pocket maximums, etc. shall remain unchanged through the duration of this Agreement.

Flexible Benefits (Medical, Dental and Life) Insurance. For new hires and transfers into the bargaining unit as of January 1, 2019, benefits eligibility shall be effective beginning the first of the month following thirty (30) days of continuous employment as a benefit’s eligible employee. All full‐time and all part‐time employees regularly scheduled to work thirty (30) or more hours per week (0.75 FTE) shall be eligible for the Employer's flexible benefits insurance plan providing medical, dental and life insurance benefits. Employees will have the option of participating in a MultiCare sponsored Wellness Plan. Those who choose not to participate will be subject to health insurance premium surcharge.

  • HEALTH AND LIFE INSURANCE The Sheriff will continue to offer to employees of this bargaining unit the same health and life insurance programs being offered to other Sheriff’s Office employees. Due to the changing nature of the health insurance market, the Sheriff retains the right during the term of this Agreement to develop plan changes or to change carriers in order to reduce costs or for other reasons. Changes to the level and types of benefits shall be subject to bargaining as provided by law. Rates paid by the employee will be established by the Sheriff.

  • Group Life Insurance Plan Section 1 - Eligibility Regular full-time and regular part-time employees who are on staff January 1, 1979 or who join the staff following this date shall, upon completion of the three-month probationary period, become members of the Group Life Insurance Plan as a condition of employment.

  • Life Insurance No portion of your IRA may be invested in life insurance contracts.

  • Term Life Insurance The Employer will maintain and make available to full-time and part-time employees, the current term life insurance plan as set forth in the document "Summary of Health Benefits, Maryland State Employees."

  • Group Term Life Insurance The Welfare Plan will include Group Term Life Insurance in accordance with the following Table of Hourly Job Rate Brackets and corresponding coverages. Benefits will be payable as a result of death from any cause on a twenty-four (24) hour coverage basis.

  • Leave for Medical and Dental Care Where it is not possible to schedule medical and/or dental appointments outside regularly scheduled working hours, reasonable time off for medical and dental appointments for employees or for dependent children shall be permitted, but where any such absence exceeds two (2) hours, the full-time absence shall be charged to the entitlement described in Clause 20.13. "Medical and/or dental appointments" include only those services covered by the B.C. Medical Services Plan, the Employer's Dental Plan, the Extended Health Benefit Plan and appointments with the Employee and Family Assistance Program.

  • Group Life Insurance The Hospital shall contribute one hundred percent (100%) toward the monthly premium of HOOGLIP or other equivalent group life insurance plan in effect for eligible full-time employees in the active employ of the Hospital on the eligibility conditions set out in the existing Agreements.
