GROUP HEALTH AND LIFE INSURANCE. The group health insurance, including the services of health care organizations, and the group life insurance provided by the Town of Norton to its employees and to the employees of this Committee shall be available to employees of this bargaining unit.

GROUP HEALTH AND LIFE INSURANCE. 15.1.1. The Association and the District Negotiating Team will make recommendations to the Board regarding insurance provider(s) and changes in benefits, coverage and funding.
GROUP HEALTH AND LIFE INSURANCE. Section 1 A school department employee in the Town of Somerset , Massachusetts, or the Somerset-Berkley Regional School District will be eligible to participate in the Town’s or District’s “Employees’ Group Insurance Plan”. At this time he must either apply for the above coverage or sign a waiver stating he does not wish to participate in this program. The Town of Somerset or the Somerset-Berkley Regional School District will pay the maximum percentage permitted by Town meeting enactment of the cost of the following types of insurance coverage: a. A $4,000 term life insurance plan of the type presently available to association members. b. Individual or family coverage, whichever applies in the particular case, for Blue Cross and Blue Shield of the type presently available to Association members. c. Employees shall be responsible for paying one hundred percent (100%) of the premiums for whichever dental plan is in effect.
GROUP HEALTH AND LIFE INSURANCE. 13.1 The Town will continue for the duration of this Agreement to provide employees with group health and group life insurance substantially equivalent to that which is being provided employees on the effective date of this Agreement. The Town and the Union agree that all employees eligible to enroll in the Town’s health insurance group plan as stipulated in the Town’s Contributory Rules and Regulations shall, effective January 1, 2006, contribute 20% towards the premium of any HMO, POS or EPO which the Town in its discretion may offer. 13.2 Any claims or disputes concerning eligibility for or payment of benefits under said insurance plans shall be determined in accordance with the applicable insurance policies and contracts and shall not be subject to the grievance procedure provided for in this Agreement.
GROUP HEALTH AND LIFE INSURANCE. Employees working less than an average of 30 hours per week are not eligible for the District’s health insurance coverage. The Board shall offer health insurance coverage for employees regularly working an average of 30 or more hours per week in accordance with the Affordable Care Act. For the purposes of eligibility, bus drivers who are contracted for at least 20 hours per week and are routinely available for field trips are considered to have met the average of 30 or more hours per week requirement. Any Driver who fails to be routinely available for field trips as required by this provision will be removed from eligibility for Board provided insurance, subject to any continuation rights under COBRA. To be routinely available as set forth herein, a driver must agree to take at least half of the field trips that are offered to the driver during the course of any 9 week grading period during the school year. Reinstatement of benefits for those who have failed to qualify will occur only following a successful 9 week period of adequate field trips taken. The Board will pay (its proportionate cost of) the premium for one (1) family or two (2) single comprehensive major medical, prescription, vision and dental health plan for any married couple who are both eligible for insurance coverage when both are employees of this Board. Eligible dependents of bargaining unit members are covered as required by law (including the “birthday rule”). Eligible dependents covered to age 19 years or 26 years if dependent upon bargaining unit member for support and regular one-half time or more student. Dependents that are eligible under state or federal law that fall outside of these criteria may be covered under the plan with the bargaining unit member paying the full cost of such coverage as allowed by law.

GROUP HEALTH AND LIFE INSURANCE. Section 1 The City of Attleboro will provide employees during the term of this Agreement with a group health insurance plan, including hospital, surgical and medical benefits, and with group dental plan which are substantially equivalent to that which is being provided on the effective day of this Agreement. The City shall pay seventy-five percent (75%) and the employee shall pay twenty-five percent (25%) of the cost of the premium for said group health insurance plan, excluding the group dental plan. The City shall pay fifty percent (50%) of the cost of the premium for the group dental plan. Section 2 The City shall make available to employees, in accordance with the provisions of Section 16 of Chapter 32B of the Massachusetts General Laws, the services of certain health care organizations. The City shall, so long as it is required to do so by the provisions of said Section 16 of Chapter 32B, pay the same dollar amount to the cost of the premiums of said health care organizations as it pays to the cost of the premium for the group health insurance plan provided for in Section 1 above. The employee shall pay the balance of the cost of the premium. In the event Section 16 is amended so as not to require the City to pay said same dollar amount to the cost of the premiums for health care organizations, then the City shall pay the same percentage of the cost of the premiums of health care organizations as it does for the cost of the premium for the group health insurance plan provided for in Section 1 above. The employee shall pay the balance of the premium. Section 3 The City shall provide employees with group life insurance in the amount of $2,000 and group accidental and dismemberment insurance in the amount of $2,000. The City shall pay seventy- five percent (75%) and the employee shall pay twenty-five percent (25%) of the cost of the premium for such insurance. Section 4 Any claims or disputes concerning eligibility for or payment of benefits under any of said insurance plans shall be determined in accordance with the applicable insurance policies and contracts and shall not be subject to the grievance procedure provided for in this Agreement.

The City shall pay the same percentage of the cost of the premiums of health care organizations as it does for the cost of the premium for the group health insurance plan. The employee shall pay the balance of the premium. 2. The City shall provide employees with group life insurance in the amount of two thousand dollars ($2,000) and group accidental and dismemberment insurance in the amount of two thousand dollars ($2,000). The City shall pay 75% and the employee shall pay 25% of the cost of the premium for such insurance. 3. Any claims or disputes concerning eligibility for or payment of benefits under said insurance plans shall be determined in accordance with the applicable insurance policies and contracts and shall not be subject to the grievance procedure provided for in this Agreement. 4. Employees shall be able to be eligible to participate in a flexible benefit plan for unreimbursed medical expenses and/or dependent care expenses. There shall be no cost to the employer. 5. The parties agree to re-open this provision if required by legislation.
GROUP HEALTH AND LIFE INSURANCE. 21.1 All unit members who are employed at least twenty (20) hours per week are eligible to participate in any group insurance programs offered by the Town of Belmont in accordance with the conditions associated with any such programs. Effective September 1, 2008, the Town of Belmont will pay 80% of the cost of the premium for the HMO health insurance plan, and members of the bargaining unit will pay 20% of the cost. Upon agreement by all town and school unions, the Town of Belmont will pay 75% of the cost of the premium for the PPO health insurance plan, and members of the bargaining unit will pay 25% of the cost. Subject to agreement by school and town unions, The BEA agrees to the following plan design changes: Office Visit Co-Pay from $5.00 per office visit to $15.00 per office visit. Prescription Drug Co-Pay from $5/ $10/ $25 to $5/ $25/ $40. 21.2 All unit members who are eligible may participate in the provisions and benefits of Section 125 Flexible Spending Account Plan administered by the Town of Belmont in accordance with the conditions associated with any such program.
