Health Care Reimbursement. All eligible bargaining unit employees shall be entitled to participate in the CSU Health Care Reimbursement Account (HCRA)
Health Care Reimbursement. It is agreed that Employers covered by this Agreement shall contribute, as per the allocation letter provided by the Union. Contributions shall be each compensable work hour of operating engineers, including supervisory Employees when covered by this Agreement, employed by such Employers in work contained in the terms of this Agreement. Said contributions shall be made, on or before the fifteenth day following the month in which the hours were worked, to the Locals 302 and 612, of the International Union of Operating Engineers Construction Industry Health Care Reimbursement Plan in the manner as set forth in the Fund Agreement of said Fund. The details of the Health Care Reimbursement Plan established by this Fund shall continue to be controlled and administered by a Joint Board of Trustees composed of equal representation from the Unions and the Chapters of the Associated General Contractors of America, Inc., who are the signators to the Fund Agreement of the aforesaid Fund. Each Trustee appointed by the Union shall be a member of the Union, and each Trustee appointed by the Employers shall be a member of an affiliated firm of the Chapters of the Associated General Contractors of America, Inc. It is understood that the Union and Employer Associations are principal parties to the Fund Agreement and, therefore, shall be furnished full information on the actions of the Trustees and the operations of the Trusts.
Health Care Reimbursement. As Employee and his dependents are covered under the Company’s group health plans (including without limitation, for medical, dental and vision), if Employee and/or his eligible dependants either timely elect continued group health care coverage under the applicable provisions of the Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA) and under the terms of the Company’s group health plans, and/or they opt for separate health care insurance coverage, the Company will reimburse Employee for the financial equivalent of what the applicable COBRA premiums for Employee and his eligible dependents could be/ could have been until the earlier of (i) the period ending twenty four (24) months after the Resignation Date; and (ii) the commencement of Employee’s employment with another employer with a comparable or better coverage under its healthcare plans for which the Employee and his dependents qualify; provided, however, that if the taxable portion of the final six (6) months of such COBRA premium equivalent payments would otherwise exceed $15,500, then the Company’s obligation to reimburse Employee for such shall be capped at $15,500.
Health Care Reimbursement.
a. The purpose of this section is to provide qualifying employees with assistance toward the purchase of health insurance, or the payment of other health care related expenses, for the employee and/or the employees’ dependents.
b. An employee must work a cumulative total of at least 1000 hours since date of appointment as a City hourly employee, without a break in active service of more than 90 days, to qualify for a health insurance allowance. Beginning the following quarter (Quarters are July-Sept, Oct-Dec, Jan-March, April- June), if the employee works at least 200 hours during any quarter the employee will receive a payment of $80 per quarter. Effective for the quarter beginning July 2017, the payment amount will increase to $100 per quarter. An employee who has qualified for the health reimbursement allowance by working at least 200 hours in each of any 3 consecutive quarters, and does not work at least 200 hours in the quarter immediately following such 3 consecutive quarters, will still be eligible for the allowance. In order to be eligible for an allowance the following quarter, however, the employee must meet the 200 hour minimum. The payment will be made as regular lump-sum earnings. Employees will be responsible for all tax consequences related to the allowance.
c. Once qualified, an employee will remain eligible under this section unless or until the employee has a break in active service of more than 90 calendar days.
d. In the unlikely event that an hourly employee works the minimum number of hours during the City’s designated measurement period, as established under the Affordable Care Act, the City may elect to offer employee-only enrollment in one or more of the City’s qualified medical plans. If offered, such enrollment will be at the employee’s expense, though the employee may receive partial reimbursement under the health care reimbursement allowance provisions, above. The City may, upon notice to the Union, offer additional payment toward the premiums to qualifying employees.

In the unlikely event that an hourly employee works the minimum number of hours during the City’s designated measurement period, as established under the Affordable Care Act, the City may elect to offer employee-only enrollment in one or more of the City’s qualified medical plans. If offered, such enrollment will be at the employee’s expense, though the employee may receive partial reimbursement under the health care reimbursement allowance provisions, above. The City may, upon notice to the Union, offer additional payment toward the premiums to qualifying employees.
Health Care Reimbursement. Participants may set aside salary up to the IRS statutory maximum per calendar year on a pre-tax basis to be used to reimburse their out-of-pocket eligible health care expenses.
Health Care Reimbursement. The Company shall reimburse Executive for Executive's monthly premium payment with respect to Aetna policy number W155977115. Executive shall provide the Company with a copy of each invoice with respect thereto.
Health Care Reimbursement. If you timely elect continued coverage under COBRA, then the Company will pay the premiums necessary to continue your current health care coverage for a period of twelve (12) months following the Separation Date (the "COBRA Payments"). The COBRA Payments cease if you become eligible for benefits under another employer's medical benefit plan. You agree to notify the Company within three (3) days of such eligibility.
Health Care Reimbursement. Until the Company institutes its own employee health care plan, at which point Employee shall pay for his own health care premiums if Employee decides to participate in the Company’s employee health care plan, Employee shall be responsible for the payment of his own health care benefits..
Health Care Reimbursement. Any funds applied to options 1-3 would be tax-free.
