Notice of Illness. An Employee who will be absent from duty on account of sickness or injury, shall notify her/his immediate supervisor or designate (if immediate supervisor not on duty), at least one
(1) hour prior to the commencement of the scheduled shift, indicating the nature and expected duration of such illness. No Employee shall be entitled to benefits for time previous to such notification unless the delay shall be shown to be unavoidable.
Notice of Illness. Employees who may be absent from duty due to illness or injury, shall notify their immediate Supervisor or designate as soon as possible prior to the commencement of the scheduled shift indicating the expected duration of such illness. In accordance with Article 4.05, Return to Work and Duty to Accommodate, the employee shall inform the Supervisor of the anticipated date of return to work and any limitations or restrictions as specified by their physician and/or medical practitioner. No employee shall be entitled to benefits for time previous to such notification unless the delay shall be shown to have been unavoidable. Employees will report to their Supervisor or designate upon resuming duties.
Notice of Illness. An employee on sick leave must report daily, early in the day, to his/her supervisor or Administrator or designee, or to the Human Resources Manager if the supervisor and designee are unavailable. It is understood that when an employee is hospitalized or on an extended sick leave, daily reporting will not be required.
Notice of Illness. The employee must satisfy the employee's supervisor or manager that an absence is due to personal illness or injury or xxxxx's responsibilities. WaterNSW may request satisfactory evidence that would substantiate the reason for leave as detailed in WaterNSW policy. The employee may not be entitled to the leave if the employee fails to provide:
(i) notice (as soon as practicable) and where possible within an hour of the usual starting time. The employee must notify the employee’s supervisor or manager of the illness or injury and how long the employee expects to be off work; and
(ii) satisfactory evidence (where required and if previously requested prior to or at the time of notification) in the following instances:
(A) for all absences of three consecutive working days or more;
(B) for all absences where five uncertified days personal/carer’s leave have been taken for the year;
(C) if an employee is to undergo personal/carer’s leave case management.

1. A police officer who claims a right to benefits under §207-c of the General Municipal Law, either because of a new illness or injury or the recurrence of a prior illness or injury, shall make application for the same in the form of a "Town of West Seneca Police Department General Municipal Law §207-c Application" either personally or through his/her representative. Application shall be made within ten
Notice of Illness. In order to receive a day of paid EIT/PTO (for illness), the employee must provide their supervisor with sufficient notice of his/her impending absence. Day shift (7-3) must notify their Department Manager or designee two (2) hours prior to the start of the shift; evening shift (3-11) must notify their Department Manager or designee three (3) hours in advance of the beginning of the shift; and, night shift (11-7) must notify their Department Manager or designee three (3) hours prior to the beginning of the shift. Failure to provide such notice may result in loss of pay for that day.
Notice of Illness. 15.3.1 Any employee absent from duty due to sickness shall inform the supervisor and the Central office by eight thirty a.m. (8:30 a.m.) unless prevented by hospitalization or emergency. Any employee absent for three (3) days without notice shall be considered to have abandoned his/her duties and shall be subject to dismissal unless it can be demonstrated that he/she is unable to physically comply with the notice requirements.

Day shift (7-3) must notify Nursing Service Administration two (2) hours prior to the start of the shift; evening shift (3-11) must notify Nursing Service Administration three (3) hours in advance of the beginning of the shift; and, night shift (11-7) must notify Nursing Service Administration three (3) hours prior to the beginning of the shift. Failure to provide such notice will result in loss of pay for that day.
Notice of Illness. ▪ EVERY traveling member whether they are part of the GSD PACKAGE GROUP or associated with a GSD travel member must immediately notify GSD STAFF of any POSSIBLE COVID RELATED SYMPTOMS.
