NOTIFICATION OF ILLNESS AND ABSENCE. I-1 Nurses shall notify the relevant Manager or designate, or her delegate, before their scheduled time of duty on the first day of illness when not available for duty, and shall do so at least two (2) hours in advance, and shall use her best efforts to provide an estimated date of return to work. If possible, nurses will provide such notification four (4) hours in advance when scheduled for evening or night shifts.

Any casual part-time nurse covered by this collective agreement must notify the relevant Head Nurse, or her delegate when not available for periods in excess of one week. Casual part-time nurses shall notify the relevant Head Nurse or her delegate of their intention to be available for work following absences due to illness.

Where this is not possible, paid will be allowed at such time as requested by the nurse and approved by the Employer, and, in any event, will taken within sixty calendar days before or after the day on which they are due unless otherwise mutually agreed. Where a paid holiday falls on a day scheduled as a day off, time off in lieu of such paid holiday will be allowed at such time as the Employer may approve. Where this is not possible, paid holidays will be allowed at such time as requested by the nurse and approved by the Employer, provided however, that such time off must be granted within sixty calendar days of return to duty. Where one or more paid holidays falls during an employee's scheduled annual vacation, another day or days appropriate will be added prior to or following the annual vacation period. Requests regarding the scheduling of the day off in lieu in Articles or may be submitted by a nurse. The Employer will consider such requests having regard to the staffing requirements. Such requests will not be unreasonably denied. The premium pay of time and one-half provided in accordance with Article will be paid for all hours worked between hours and midnight on the paid holidays listed in Article I-l.   • Notification of Illness Nurses should notify the Medical Center of absence from work because of illness as far in advance as possible, but at least three and one-half (3 ½) hours before the start of the nurse’s shift. Repeated failure to give such minimum notification will result in reduction of otherwise payable sick leave for that shift by two (2) hours. Repeated failure as used in this section means more than twice every two years.

  • Notification of Individuals To notify individuals of the breach or unauthorized use or disclosure when notification is required under state or federal law and to pay any costs of such notifications, as well as any costs associated with the breach. The County Compliance Manager shall approve the time, manner and content of any such notifications.

  • Verification of Illness Written verification by an approved licensed medical practitioner or other satisfactory proof of illness or family illness may be required at the discretion of the department head.

  • Notification of Incidents If Contractor becomes aware of or has reasonable suspicion of a privacy incident or security incident regarding any State data, Contractor must report such incident to the State and the State Chief Information Security Officer as soon as possible, but no later than twenty-four (24) hours after such incident. The decision to notify the affected data subjects and the form of such notice following report of a privacy incident or security incident are the responsibility of the State. Notwithstanding anything to the contrary in this Contract, Contractor will indemnify, hold harmless and defend the State and its officers, and employees for and against any claims, damages, costs and expenses related to any privacy incident or security incident involving any State data. For purposes of clarification, the foregoing sentence shall in no way limit or diminish Contractor’s obligation(s) to indemnify, save, hold harmless, or defend the State under any other term of this Contract.   • Sickness and Accident If the Contractor’s Employees fall sick in the period during which they are engaged on the RLA, the UNDP shall not be responsible for arranging or paying for medical treatment and attention. The UNDP shall not be required to pay for the services of the Contractor’s Employees for any period that the Contractor’s Employees are incapacitated by sickness. If in the opinion of the UNDP, any of the Contractor’s Employees either has been or will be incapacitated by sickness for an unreasonable period or period then, and in that case, it shall be at the discretion of the UNDP to decide if and when the employment of the Contractor’s Employee under the RLA shall be terminated and the Contractor be required to replace him. In this event, the Contractor shall on receipt of instructions from the UNDP comply forthwith and shall substitute for the Employee whose services are so terminated another and satisfactory person and the whole costs of such replacements shall be at the Contractor’s expense.

  • Separation of Insureds; No Special Limitations All insurance required by this Section shall contain standard separation of insureds provisions. In addition, such insurance shall not contain any special limitations on the scope of protection afforded to the City, its directors, officials, officers, employees, agents, and volunteers.

  • TERMINATION OF INSURANCE A. Your policy will lapse if you do not pay your premium when due.

  • Sickness and Accident Insurance During the term of this Agreement, the Employer shall pay the required premiums for each employee for sickness and accident insurance which will pay Three Hundred Fifty Dollars ($350.00) per week for a period of fifty-two (52) weeks. This benefit shall be payable from the thirty-first (31st) day of disability due to illness or injury.

  • Separation of Insureds All liability policies shall provide cross-liability coverage as would be afforded by the standard ISO (Insurance Services Office, Inc.) separation of insureds provision with no insured versus insured exclusions or limitations.

  • Definition of Immediate Family Immediate family includes husband, wife, child, stepchild, brother, stepbrother, sister, stepsister, parent, stepparent, or any person serving as a parent, or who has served as a parent, or any other person living in the same household as the employee.
