Outbreak. (a) Upon recommendation of the Medical Officer of Health, all employees may take such treatments as the Officer may direct. If the cost of such treatment is not covered by some other source, the cost will be borne by the Employer.

Outbreak. CEPI will notify Awardee in writing in the event of an Outbreak or if there is an Increased Outbreak Preparation Need, in each case identifying the Affected Territory (“Outbreak Notice”). Once an Outbreak Notice has been provided by CEPI, CEPI shall have the right to direct how the Safety Stock referred to in Clause 15.2 a. or any Product manufactured pursuant to Clause 16.3Error! In consultation with relevant public health authorities in the Affected Territory, CEPI may request that Awardee discuss in good faith whether and how the Project Results could be utilized in response to the Outbreak Notice. Awardee is committed to use commercially reasonable best efforts to address Outbreaks and Increased Outbreak Preparation Need wherever they occur in the world. Following receipt of an Outbreak Notice, Awardee will use its commercially reasonable best efforts to increase the supply of Product available for use by CEPI or its nominees to an amount which equals at least [***] of the production forecast for the Products prepared by Awardee immediately prior to service of the Outbreak Notice and Awardee will use its commercially reasonable best efforts to ensure that such increased capacity is available for delivery to CEPI within [***] of the date of service of the Outbreak Notice. For clarity, Awardee will use commercially reasonable best efforts to keep such deadline of [***] (including discussing with Awardee’s contract manufacturers how they can meet the proposed deadlines), however, Awardee’s ability to meet deadlines will be subject to the lead times of Awardee`s contract manufacturers and the time required for the release testing of the Product. In the event that CEPI’s request for Product to meet the increased demand during an Outbreak or Increased Outbreak Preparation Need is in excess of the quantities that Awardee is able to supply to CEPI based on Awardee`s commercially reasonable best efforts, Awardee shall not be obliged to supply Product to CEPI under this Clause 16.1 to the extent that the supply of such quantities of Product to CEPI would result in Awardee being in breach of any binding contracts in existence on the date of service of the Outbreak Notice (which for the avoidance of doubt may include the supply of Products to customers for Awardee’s Traveler’s Market or in connection with any clinical trials). Outbreak. The definition of an “outbreak” for the purposes of this protocol is three or more cases of an infectious disease in youth program within a 14-day period. If the program director suspects an outbreak, they must contact the UW Oshkosh Police Department immediately to assist with next steps. Program closure and/or additional illness prevention measures, such as those listed above in Section 4.1, may be enacted for the remainder of the program to prevent further spread of disease.

Outbreak. 27.01 Upon recommendation of the Medical Officer of Health, all employees may take such treatments as the Officer may direct. If the cost of such treatments is not covered by some other source the cost will be borne by the Employer. If an employee does not take the recommended course of treatment, or fails to complete it, she shall be placed on an unpaid leave of absence until such time as the situation is resolved. If an employee does not complete a course of treatment initiated by the Employer any subsequent course of treatment required as a result of the same situation shall be undertaken at the employee's expense. An employee who does not take the recommended course of treatment for verified medical or bona fide religious reasons is entitled to such accommodation as the Employer may direct or, failing that sick leave if the credits are available. Where it is permitted by the Medical Officer of Health, or designate, and where it is otherwise possible, an employee who cannot work due to not taking the influenza vaccine may be reassigned to work in another area of the home until the outbreak is declared over. In the case of employees who work at more than one health care facility, and an outbreak occurs in one of the facilities with the result being the medical officer of health or designate limits the employee to working at one facility only, the employer will attempt to offer the employee call-in hours, being respectful of the agreed to call-in procedures.

Outbreak. Despite my efforts and the efforts of my therapist, I understand that therapy may sometimes lead to unanticipated emotional stress as well as relief and growth, and that the FTC does not guarantee any particular results or outcome from the therapy process, including telehealth services. In order to receive telehealth services from the FTC, I understand that I am responsible for 1) providing the electronic device(s), such as a telephone or a computer with a webcam and/or audio capability, and the internet access necessary for my telehealth sessions, 2) maintaining the security of my electronic device(s), 3) arranging a location with sufficient privacy that is free from distractions or other intrusions and 4) maintaining a secure internet connection (instead of public/free Wi-­‐Fi). I understand that my therapist will guide me in the use of the technology associated with our telehealth sessions. I further understand that there are potential risks associated with participating in telehealth services. I understand my therapist uses secure audio-­‐visual transmission software to deliver telehealth services. However, it is still possible that my personal information could be obtained by an unauthorized third party. I agree that my therapist or I can discontinue any telehealth session if we believe that the audio/video connection or setting is not adequate to insure my confidentiality and privacy. I also understand that my therapist will stop any telehealth session immediately if an unauthorized party attempts to participate in any way. Despite reasonable efforts on the part of the therapist, I understand that my telehealth sessions may be disrupted or distorted by technical failures or difficulties. If this occurs, the therapist will make a good faith effort to troubleshoot and/or restore contact using the telephone or the chat feature of the audio-­‐ visual platform. If contact cannot be restored relatively quickly (within 15 minutes or so), the telehealth session will be rescheduled. I understand that my therapist will wait 15 minutes for me to join any scheduled telehealth session. After that time, I understand that my therapist will not necessarily be available, and I will need to contact my therapist through our agreed upon contact phone number (i.e., FTC therapist voicemail or Google Voice) to reschedule.
