Quarterly Operational Reports. The state must submit progress reports no later than 60 days following the end of each quarter for every demonstration year (DY) within the format outlined in Appendix A. The intent of these reports is to present the state’s data along with an analysis of the status of the various operational areas under the demonstration. These quarterly reports must include, but are not limited to: a) Quarterly expenditures for the demonstration population, with administrative costs reported separately; b) Quarterly enrollment reports for demonstration enrollees (enrollees include all individuals enrolled in the demonstration) that include the member months for each DY, as required to evaluate compliance with the budget neutrality agreement and as specified in STC 33; c) Total number of participants served monthly during the quarter for each DY (participants include all individuals who obtain one or more covered family planning services through the demonstration); d) Events occurring during the quarter, or anticipated to occur in the near future that affect health care delivery, benefits, enrollment, systems, grievances, quality of care, access, payment rates, pertinent legislative activity, eligibility verification activities, eligibility redetermination processes (including the option to utilize administrative redetermination), and other operational issues; e) Notification of any changes in enrollment and/or participation that fluctuate 10 percent or more in relation to the previous quarter within the same DY and the same quarter in the previous DY; f) Action plans for addressing any policy, administrative or budget issues identified; g) An updated budget neutrality monitoring worksheet; and h) Evaluation activities and interim findings.

Events occurring during the quarter or anticipated to occur in the near future that affect health care delivery including approval and contracting with new plans; benefits; cost- sharing, enrollment; grievances; quality of care; access; health plan financial performance that is relevant to the demonstration; pertinent legislative activity; and other operational issues; c. Action plans for addressing any policy and administrative issues identified; d. Any changes the state made or plans to make to the demonstration as a Category I or II change; e. The number of individuals enrolled in each major program of the Comprehensive demonstration; including, but not limited to TANF and related programs, the extended family planning program; each of the limited benefit programs; and ABD with breakouts for the LTC reform community and institutional programs; f. The number of individuals served by and costs of the Marketplace Subsidy program; and g. Evaluation and Quality Assurance and Monitoring activities and interim findings.

The intent of these reports is to present the Commonwealth’s analysis and the status of the various operational areas under the demonstration. These quarterly reports must include, but are not limited to: Updated budget neutrality monitoring spreadsheets; Events occurring during the quarter or anticipated to occur in the near future that effect health care delivery including approval and contracting with new plans, benefits, enrollment, grievances, quality of care, access, proposed changes to payment rates, health plan financial performance that is relevant to the demonstration, payment reform initiatives or delivery system reforms impacting demonstration population and/or undertaken in relation to the SNCP including ICB grant programs, updates on activities related to the pediatric bundled payment pilot program, pertinent legislative activity, and other operational issues; Action plans for addressing any policy and administrative issues identified; Quarterly enrollment reports that include the member months for each demonstration population; Updates on any state health care reform activities to coordinate the transition of coverage through the Affordable Care Act; Activities and planning related to payments made under the Safety Net Care Pool pursuant to reporting requirements outlined in section VIII of the STCs; Updates on data related to the provisional eligibility authority. The State may combine the quarterly report due for the quarter ending September with the annual report in STC 49. Notwithstanding this requirement, the fourth-quarter Quarterly Report may be included as an addendum to the annual report required in paragraph 54.

(Participants include all individuals who obtain one or more covered family planning services through the demonstration.) c) Events occurring during the quarter, or anticipated to occur in the near future that affect health care delivery, benefits, enrollment, grievances, quality of care, access, pertinent legislative activity, eligibility verification activities, and other operational issues; d) Action plans for addressing any policy and administrative issues identified; and e) Evaluation activities and interim findings.

The intent of these reports is to present the state’s data along with an analysis of the status of the various operational areas under the demonstration. These quarterly reports must include, but are not limited to: a. Updated budget neutrality and allotment neutrality monitoring spreadsheets. b. Events occurring during the quarter, or anticipated to occur in the near future that affect health care delivery, including approval and contracting with new plans; benefits changes; enrollment counts with description of variances upward or downward; grievances by type and count; complaints by type and count; quality initiatives and plans; access; health plan financial performance that is relevant to the demonstration; pertinent legislative activity; and other operational issues. c. Action plans for addressing any policy and administrative issues identified. d. Evaluation activities and interim findings. e. Reporting Requirements Related to the ICS Program. f. The state will include data on the Program and its enrollees in the Community Options waiver annual report. It will be completing each March, as specified in Appendix A of the state’s approved Community Options waiver.

Reporting Requirements Related to the Family Planning Program: 1) Quarterly enrollment reports for Demonstration eligible (eligibles include all individuals enrolled in the Demonstration); 2) Total number participants served during the quarter (participants include all individuals who obtain one or more covered family planning services through the Demonstration); 3) Notification of any changes in enrollment and/or participation that fluctuate 10 percent or more in relation to the previous quarter within the same DY and the same quarter in the previous DY;
