Request for Leave of Absence. Employees are encouraged to submit requests for leave of absence as early as possible. Except in extenuating circumstances:
(a) All requests for leave of absence of seven (7) calendar days or less shall be submitted at least seven (7) days in advance.
(b) All requests for leave of absence of more than seven (7) calendar days shall be submitted at least twenty-eight (28) days in advance.
(i) Requests to extend the length of leave shall be submitted at least twenty-eight (28) days in advance of the previously agreed upon date of return.
(ii) Requests to reduce the length of leave shall be submitted at least twenty-eight (28) days in advance of the new date of return. Requests shall include dates of commencement and return. The Employer shall provide the Employee with a written response within 3 calendar days of the request if LOA is for less than seven (7) days and within fourteen (14) calendar days of the request if the LOA is greater than seven (7) days. Whenever a request is denied, the written response must include reasons for the leave of absence being denied.
Request for Leave of Absence. All leaves of absence shall be requested by notifying the manager and contacting the third-party administrator at least thirty (30) days in advance, if possible, stating the reason for the leave and the amount of time requested. A written reply to grant or deny the request shall be given by the Employer within two (2) weeks, unless the leave is of an emergent nature, in which case the Employer will respond within twenty-four (24) hours or as soon as possible. A leave of absence begins on the first day of absence from work. Whenever an employee is eligible for more than one type of leave, all applicable leaves will run concurrently unless stated otherwise.
Request for Leave of Absence. Maternity leave covers employee absence after the birth of the baby and the employee is eligible for sick leave until released by physician. If, on the advice of the physician, the employee requires leave prior to the birth of the baby, such leave will be considered medical leave. If additional time is needed / desired, the employee may request a leave of absence, without pay, for family reasons.
Request for Leave of Absence. Any request for a leave of absence other than for vacation and sick leave shall be submitted in writing by the employee to his/her immediate supervisor. The request shall state the reason the leave of absence is being requested and the length of time off the employee desires. Request for Leave of Absence. Upon request, an employee shall be granted a leave of absence from her position prior to the birth of a child. The beginning date of the leave shall be determined by the employee’s physician.

All leaves of absence must be requested in writing to the person designated by the Employer, unless the employee is physically disabled to the extent that such advance request is not possible, and shall state: (1) the reasons, (2) date leave is to begin, and (3) expected date of return to work. Leaves of absence shall be granted in writing in advance, and a copy shall be given to the employee.
Request for Leave of Absence. An employee, upon request, shall be granted a leave of absence from her position prior to the birth of a child, the exact date to be determined by the employee's physician. The procedure governing leaves of absence for personal illness or injury relating to pregnancy will apply as follows:
A. An employee may use accumulated sick leave for any period of disability caused by pregnancy, miscarriage, abortion, childbirth and/or recovery therefrom.
B. The District may request a certification of disability or fitness for return to work under the same rules applicable to other uses of sick leave.
Request for Leave of Absence. Except in cases of sick leave and bereavement leave, requests for leaves of absence will be completed in the following manner: the employee shall submit his/her request in writing to his/her immediate supervisor. Such request shall include the reason(s) for the leave, the requested beginning date of the leave and the date of return from the leave.
