Requesting Leave of Absence. Upon application to the Chief of the Department, leave of absence may be granted, without pay, to employees for thirty (30) calendar days. Requests for more than thirty (30) calendar days may be recommended by the Chief of the Department, but must be approved by the Mayor.

Requesting Leave of Absence. Except in emergencies, permission to take any of the leave of absence outlined in this policy must be requested as far in advance as possible using the appropriate form and submitting any evidence for the request, as necessary. A form for this purpose is shown in Appendix 1. The form should not be used to request annual leave, adoption, maternity, parental, paternity , paternity/maternity support leave or parental bereavement leave, as each associated policy contains the relevant form. Where a request for leave of absence is refused, the employee should be advised of the reasons for this decision.
Requesting Leave of Absence. Upon request, the City Manager or his/her designee may grant leaves of absence, without pay, to an employee for up to two (2) weeks duration. A leave of absence may extend up to six (6) months in duration and may be renewed upon proper application. An employee shall request a leave of absence in writing thirty (30) days in advance of the date desired, except in emergency situations. A leave may be requested for any legitimate purpose. Where necessary, an employee may be granted up to twelve (12) weeks off for sickness as outlined in the Family Medical Leave Act (FMLA). The City requires that when the FMLA is granted, accumulated sick days, personal days, and vacation days must first be exhausted concurrent with the leave. Once the above days are exhausted, the balance of the twelve (12) weeks shall be unpaid. No leave may be granted before an employee has completed his probationary period, except for emergencies and with the specific recommendation of the city manager or his/her designee. Seniority shall not accrue during any unpaid leave of absence longer than two (2) weeks.

No leave of absence shall be granted without the prior approval of the employee’s Department Director or their designee and the Human Resources Manager.
