RESPONSIBILITIES & REQUIREMENTS. A. Requirements for services provided include, but are not limited to, the following:
1) Water Truck Services (includes potable water, gray water and black water pumping and removal services) to for entire facility for the following interior and exterior assets:
RESPONSIBILITIES & REQUIREMENTS. The board resolution, order, motion, ordinance or similar document shall be approved by the State before the parties can enter into a valid contract. The Contractor shall not perform any tasks prior to contract execution. A list of all tasks and deliverables are set forth below. Each county is required to have a Board legally binding resolution, order, motions or ordinance or similar document from the local governing body authorizing execution of the agreement.
RESPONSIBILITIES & REQUIREMENTS. A. The Contractor shall supply all required expertise, labor, tools, equipment, materials, and Services required to provide security presence and access control to the premises.
B. Security Guards shall ensure that the facility is a safe environment for FMS clients and personnel, maintaining high visibility around both the interior and exterior of the premises, as a deterrent to unsafe, unauthorized or illegal activities.
1) Inside Guards - Contractor will supply security guards and security guard to patrol interior of FMS. (Number of guards is subject to change depending on needs of the FMS.)
a) Inside Security Guards shall and make continuous rounds throughout the interior of the FMS location, as directed by the on- site Los Angeles FMS Coordinator.
2) Outside Guards - Contractor will supply security guards to patrol exterior of FMS. (Number of guards is subject to change depending on needs of the FMS.)
a) Outside Security Guards shall monitor and authorize entrance and departure of employees, visitors, and other persons to guard against theft, threat, illegal activities and maintain security of premises.
b) Outside Security Guards shall complete of the designated location as directed by the on-site Los Angeles FMS Coordinator.
C. Security Guards shall write reports of daily activities and irregularities, such as equipment or property damage, theft, presence of unauthorized persons, or unusual occurrences.
D. Personal Protective Equipment (PPE) will be supplied to guards by the on-site Los Angeles FMS Coordinator as needed prior to beginning of shift. All guards must use PPE as directed by the on-site Los Angeles FMS Coordinator.
E. If a person, who is not authorized to conduct business in this facility, or a client uses abusive language, or behaves in a threatening manner, the security guard should attempt to defuse the situation in a professional and polite manner. If the person continues with an abusive attitude or threatening manner, the security guard will immediately contact 911 or the Los Angeles City Police Department and notify the on-site Los Angeles FMS Coordinator.
F. Security Guards are expected to fully cooperate with all law enforcement, fire and other emergency personnel.
RESPONSIBILITIES & REQUIREMENTS. A. Requirements for services provided include, but are not limited to, the following:
1) Contractor will ensure all updated CDC or County guidance are implemented as appropriate.
2) Contractor will service all areas of the facility that are being utilized including restrooms, patient rooms, storage areas, waste areas, laundry areas, portable showers, portable handwash stations and portable toilets and other specified locations twice per day or as directed by facility staff.
3) Contractor will replenish paper products and soap in the portable showers, portable handwash stations and portable toilets and other specified locations, as needed, using supplies provided.
4) Contractor will supply all cleaning and disinfection supplies and equipment.
5) Contractor will collect waste twice per day or as directed by staff and place in designated receptacle.
6) Contractor will collect locked containers housing medical sharps as directed by facility staff and place in designated receptable.
7) Contractor will comply with all Cal/OSHA and Federal OSHA requirements specific to worker safety.
8) Proper required PPE to be provided by vendor will be worn at all times including, Gloves, Gown, Mask, Face Shield, and Booties.
9) Contractor will maintain all required paperwork and logs before and after entering the site.
B. Contractor will provide services (with at least 24 hours’ notice) on an “as needed” basis, as requested by the FMS Point of Contact.
C. Services shall be provided at the COVID-19 positive Federal Medical Station located at: Riverside FMS #2 0000 Xxxxxxxxx Xxx. Riverside, CA 92504
RESPONSIBILITIES & REQUIREMENTS. A. Requirements for services provided include, but are not limited to, the following: • Contractor will provide regular cleaning and disinfection in accordance with all current CDC Guidance for Healthcare Facilities (xxxxx://xxx.xxx.xxx/coronavirus/2019-ncov/healthcare- facilities/index.html) as amended. This includes all referenced guidance including the Infection Prevention and Control guidelines. • Services include the use of EPA registered hospital grade disinfectants, and frequent cleaning of high touch surfaces and shared patient equipment. • Contractor to service approximately 190,000 sq.ft facility and service all areas that are being utilized including, but not limited to, restrooms, patient rooms, storage areas, waste areas, and laundry areas. • Services include the following: o Empty waste bins and replace trash liner; o Clean and sanitize all surfaces; o Mop and vacuum floors; and o Refill paper products • Contractor will provide all equipment and materials needed to provide services for up to 300 patients, including paper products (toilet paper, toilet seat liners, etc.) • Proper PPE to be provided by the Contactor and shall be worn at all times. • Contractor shall ensure services are available up to twenty-four (24) hours per day, seven (7) days per week.
B. Services shall be provided at the following location: Fairview Developmental Center 0000 Xxxxxx Xxxx. Costa Mesa, CA 92626
RESPONSIBILITIES & REQUIREMENTS. A. Requirements for services provided include, but are not limited to, the following: • Transportation and delivery of diesel fuel for six (6) 4000W Narrow Vertical Mast Light Towers,1KW Generator, and 1 60KVA Generator • Offloading fuel onto light towers and generators • Diesel fuel delivery on an on-call basis
B. Services shall be provided at the COVID-19 positive Federal Medical Station located at: Riverside FMS (Old Sears Building) 0000 Xxxxxxxxx Xxx., Xxxxxxxxx, Xx. 92504.
RESPONSIBILITIES & REQUIREMENTS. A. Requirements for services provided include, but are not limited to, the following: • Contractor will ensure all updated CDC or County guidance are implemented as appropriate • Contractor will comply with all Cal/OSHA and Federal OSHA requirements specific to worker safety • Contractor will communicate directly with Santa Xxxxx Convention Center on daily pick up and delivery requirements for all laundry services • Contractor to provide bags for soiled laundry placement • Contractor to provide carts for separation of clean and soiled laundry storage • Laundry Services include pillow slip, single sheet, washcloths, patient gowns, bath towels, blankets, scrubs
B. Services shall be provided at the following location: Santa Xxxxx Convention Center, 0000 Xxxxx Xxxxxxx Xxxx, Xxxxx Xxxxx, XX 00000.
RESPONSIBILITIES & REQUIREMENTS. A. Requirements for services provided include, but are not limited to, the following:
1) Rental of the following:
a. One (1) Shower Trailer - 16 Stalls, with the option to request additional shower units with not less than ten (10) stalls, to be delivered not more than four (4) days after requesting such
2) Delivery, set up, tear down, and pick up at conclusion of rental.
3) Contractor will provide the following:
a. Water heaters, electrical power, and all fuels and maintenance of heaters and generators
b. Shower soaps, bath towels, and face cloths
c. Not less than one (1) attendant per shift of not greater than three (3) shifts to sanitize each shower cell, after each use. The number of shifts and employees may be increased or decreased based on usage, with not less than 24 hour notice.
d. Technical service and maintenance technicians that can be on scene within 30 minutes after notification
e. Contractor will ensure all permits and operating licenses are complied with
f. Contractor will provide adequately trained, licensed, or qualified employees to conduct the operation, and shall provide for its employees all equipment, detergents, safety gear, and personal protection to ensure their employees may execute a healthy, safe, and efficient operation.
g. Provide employee or representative names and company affiliation not less than two (2) hours prior to arrival to gain entry to the site. All visitors, contractors, their agents, employees, or workers must be health screened prior to interacting with anyone on site. No exceptions until further notice.
B. Services shall be provided at the following location: Porterville Developmental Center 00000 Xxx 000 Porterville, CA 93257
RESPONSIBILITIES & REQUIREMENTS. A. Requirements for services at both Xxxxxxxxxxx Auditorium and Taj Mahal buildings include, but are not limited to, the following:
1) Coordination and installation of stand-alone temporary heating and cooling equipment with associated air distribution including monthly rental for equipment, delivery, pick-up and maintenance of equipment.
a. Utilize packaged DX air conditioning units with electric strip heaters for supply and air handlers for exhaust achieving 100% outside air. Temporary duct shall be a combination of flex duct and duct sock. Support systems with basic materials such as rope.
b. Target space temperature of 65°F-75°F (no humidity target).
c. Provide MERV10 filters or greater on the supply.
d. Target space pressurization of -0.10” WC to +0.10” WC.
e. Utilize ASHRAE: Desert Resorts Regional, CA USA (WMO: 747187), Cooling DB – 2% = 107.1°F. Heating 58°F – Design for typical conditions of April – September.
f. Provide controls to interlock HVAC equipment and maintain space temperature.
g. Generators for the temporary HVAC equipment as required.
h. Fueling for the generators.
i. Equipment inspections and monitoring of fluid levels as required.
2) Coordination and installation of a 3,000-amp emergency generator, three phase automatic transfer switch and associated cabling to 100% back up the building.
a. Includes monthly rental for equipment, delivery, pick-up and maintenance of equipment.
b. Daily equipment inspections and monitoring of fluid levels will be provided by on-site facilities.
c. Fuel and fluids for temporary emergency generator as required.
3) Verify existing emergency lighting and add temporary emergency lighting as required to meet minimum foot-candles in patient bed area.
4) Verify Wi-Fi and fire alarm are working and functional. Add smoke detectors for both supply and return for all temporary units.
5) Duct detectors are wired and tested by the County of Riverside fire alarm service contractor. Xxxxxx Construction only provides supervision of this portion of the work.
6) Coordination and demobilization of stand-alone temporary heating and cooling equipment with associated air distribution and generators.
7) Coordination of demobilization of emergency generator, automatic transfer switch and associated cabling.

RESPONSIBILITIES & REQUIREMENTS. This section shall indicate the name and coordinates of: · The Supplier Technical Responsible in charge of the Contract · The Supplier Quality Representative for the Contract It shall identify the different organizations involved to perform the Contract and detail the breakdown of responsibilities (an organizational flowchart should facilitate the understanding). The Supplier Technical Responsible shall coordinate planning, performance and control of the work, including work assigned to subcontractors. The Supplier Technical Responsible shall keep time schedule and issue progress report. The Supplier Quality Representative shall control that the Quality Plan, quality procedures and detailed work instructions are followed during the course of the Contract in order to ensure that the contractual quality requirements are met. The Supplier Quality Representative shall assess and control the management quality at the subcontractor’s premises. F4E shall be informed of any change of Supplier Technical Responsible or Quality Representative.
