SERVICES TO BE PROVIDED BY CONTRACTOR. 1. Required Activities Prior to the COVID-19 Vaccination Event
2. The Contractor must complete the following required event site planning activities prior to the COVID-19 Vaccination Event:
A. Event Site Plan Development and Prior Approval
1. Coordinate with Event facility manager to develop an Event site plan which includes conducting a site survey to determine proper station layout, client flow, and address any potential security issues.
2. Submit the final Event plan to the designated DSHS Public Health Region for the Event location. This Event plan must include:
a) Flow diagram for the layout of the specific facility;
b) Site security plan;
c) Site parking plan (or road flow for drive-through events);
d) Staffing plan/roster of Event core staff, including full name, license numbers (where applicable), email addresses, and phone numbers; and
e) Plans for the disposal of biohazard materials and used sharps container;
f) Methods to encourage physical distancing and provide one-way flow through the clinic or as part of a Vaccination Event.
3. Event plans must be approved by the designated DSHS Public Health Region prior to the Event date.
B. Secure Event Staff The Contractor must assemble the following team for the COVID-19 Vaccination Event: • One on-site project manager per COVID-19 vaccination Event; • One supervising physician (licensed in the State of Texas) per COVID-19 vaccination event; • One vaccinator per 25 clients; • One vaccinator clinical assistant per 25 clients; • One administrative team member responsible for greeting/patient flow per 25 clients; • One administrative team member responsible for documentation per 25 clients; • One crowd control team member per 100 clients; and • One security guard per 100 clients. The supervising physician may also serve in the capacity of a vaccinator, but all other roles must be occupied by separate individuals for the Event. All other roles must be occupied by separate individuals uniquely serving in a single role on the event date. In some communities, it may be necessary to ensure that at least one team member speaks a language other than English. Contractor must be prepared to address language barriers through the inclusion of team members who can speak the identified foreign language (as indicated by the designated DSHS Public Health Region) or by using the HHS Language Line (as coordinated by the DSHS Public Health Region).
C. Secure Supplies and Materials Contractor must secure sufficient supplies ...
SERVICES TO BE PROVIDED BY CONTRACTOR. Contractor shall perform the services for the District as set forth in this Agreement and in the attached Exhibit A, which is fully incorporated into this Agreement by this reference (collectively “Services”), and in accordance with the terms and conditions in this Agreement. Contractor agrees to perform the Services consistent with the professional skill and care of Contractor’s profession and in compliance with all applicable laws and regulations. All of Contractor’s activities will be at its own risk and Contractor is hereby given notice of responsibility for arrangements to guard against physical, financial, and other risks as appropriate. Contractor assumes full responsibility for the acts or omissions of Contractor’s employees, agents, consultants, and subcontractors as they relate to this Agreement or the Services. The Parties agree that the Contractor will immediately remove any of Contractor’s employees, agents, consultants, or subcontractors from the District’s facilities upon the District’s instruction, as determined by the District in its sole discretion, for any or no reason. Contractor agrees to promptly provide a replacement employee, agent, consultant, or subcontractor acceptable to the District to perform the Services
SERVICES TO BE PROVIDED BY CONTRACTOR. In consideration of the payments hereinafter set forth, Contractor agrees to perform the services described in Exhibit A, attached hereto (the “Services”). All Services are to be performed and completed by June 30, 2020.

SERVICES TO BE PROVIDED BY CONTRACTOR. A. The Contractor shall furnish Internet auction services as more particularly provided for in this Agreement (the “Services”) for Tax Certificates for the Municipality.
B. The Contractor shall provide a host server (the “Server”) for the Web Site. As used herein, the term "Web Site" shall mean an Internet web site that Contractor will make available to Municipality under this Agreement. The Web Site will utilize Contractor's proprietary software, which is capable of accepting and processing competitive bids for Tax Certificates to be issued by Municipality. The Municipality acknowledges that the Contractor's Server may not be dedicated exclusively to the Web Site. The Contractor shall use its commercially reasonable efforts to make the Web Site available during all regular business hours and shall not schedule planned maintenance downtime to occur during regular business hours.
SERVICES TO BE PROVIDED BY CONTRACTOR. Pursuant to the terms of this Agreement Contractor agrees to provide the following services as requested from time to time by the Company.
A. Engineering and related technical services in the mechanical, electrical, structural and marine engineering disciplines, pertaining to the construction, installation, testing, commissioning and operation of the marine systems applicable to the Drilling Unit.
B. Assistance to the Company for the procurement and delivery of necessary equipment, spare parts and supplies applicable to the marine system of the Drilling Unit in a timely manner.
C. Policies, procedures and systems for project management, management information, safety, preventive maintenance and inventory control applicable to the marine systems of the Drilling Unit.
D. Assistance in the recruitment of qualified and experienced marine personnel for the Drilling Unit by the Company and for the training of marine personnel to be assigned to the Drilling Unit.
E. Project management, inspection and related technical assistance services applicable to the marine systems of the Drilling Unit prior to delivery of the Drilling Unit from the shipyard. If Contractor is unable to provide the services requested for whatever reason, (i) Contractor shall promptly advise Company of same in writing, (ii) Contractor will owe no further obligation to Company with respect to Contractor providing the requested services, and (iii) Contractor agrees that it will at the Company's request provide assistance to the Company in identifying and procuring direct for the Company's account other contractors who may be able to provide such services. All services provided by Contractor under this Agreement shall be in conformance with good and accepted marine practice and standard operating procedures and practices of the industry.
SERVICES TO BE PROVIDED BY CONTRACTOR. 4 In compliance with all of the terms and conditions of this Agreement, 5 CONTRACTOR shall provide to ANAHEIM all labor, materials, tools, equipment, services, and 6 incidental and customary work necessary to fully and adequately supply the professional 7 consulting services necessary for the Project (“Services”). The Services are more particularly 8 described in the Scope of Services attached hereto as Exhibit A and incorporated herein by this 10 Manager") may elect to delete certain tasks from the Scope of Services in his sole discretion.
SERVICES TO BE PROVIDED BY CONTRACTOR. A. The Contractor shall furnish Internet auction services as more particularly provided for in this Agreement (the “Services”) for Tax Certificates for the Municipality. If the Services under this Agreement have been awarded to the Contractor pursuant to a Request for Proposal (“RFP”) or Request for Quotation (“RFQ”) issued by the Municipality, the parties agree that the terms and conditions of the RFP or RFQ (as applicable) and Contractor’s RFP or RFQ response (as applicable) accepted by Municipality (collectively, the “Final RFP/RFQ”), are hereby incorporated as if fully set forth herein and are expressly included in the defined term “Services.” The Electronic Sales Regulations are also hereby incorporated by reference as if fully set forth herein, and all Services shall be provided in accordance with the Electronic Sales Regulations. In the event of a conflict among the New Regulations, LFN, the Final RFP/RFQ or this Agreement, the terms and conditions of the New Regulations shall prevail, followed by the LFN, the Final RFP/RFQ and then this Agreement. For avoidance of doubt, Contractor shall not be required to comply with any terms and conditions in the Final RFP/RFQ that is expressly prohibited by the Electronic Sales Regulations.

The Municipality acknowledges that the Contractor's Server may not be dedicated exclusively to the Web Site. The Contractor shall use commercially reasonable efforts to make the Web Site available during all regular business hours (8:30 a.m. to 5:30 p.m. EST) and shall not schedule planned maintenance downtime to occur during these regular business hours.
SERVICES TO BE PROVIDED BY CONTRACTOR. CONTRACTOR shall provide medical examiner investigation (MEI) services and office support to the Saginaw County Medical Examiner’s Office on an as needed basis. Work times shall be as established in consultation with the Saginaw County Controller/CAO or his designee. Such services shall include, but not be limited to, obtaining medical records from hospitals and doctor offices, responding to inquiries or requests from funeral homes or family members/next of kin, and preparing the documents necessary to issue death certificates and creation permits. The services to be provided by CONTRACTOR are more fully detailed in the Medical Examiner Investigator Job Description, which is attached hereto as Exhibit A to this Agreement and is fully incorporated into this Agreement by reference. In providing all services pursuant to this Agreement, CONTRACTOR shall abide by all statutes, ordinances, rules and regulations pertaining to, or regulating the provision of such services, including those now in effect and hereafter adopted. Failure to do so shall constitute a material breach of this Agreement.
