SHORT TERM/LONG TERM DISABILITY INSURANCE. A short-term disability program is provided to coordinate with the long-term disability insurance program for Police Unit employees. The short-term disability benefit will begin on the 8th day of absence (or first day if the employee is hospitalized) and continue for 180 days (6 months). The long-term disability policy will begin on the 181st day. Police Unit employees shall be included in the short-term/long-term disability insurance program in accordance with the City’s group policy. The City shall contribute an amount equal to the monthly premium for such coverage. The monthly benefit for short-term and long-term disability shall be 66-2/3 percent of salary to a maximum monthly benefit of $9,500.
SHORT TERM/LONG TERM DISABILITY INSURANCE. A. The City shall maintain a short-term / long-term disability (STD/LTD) insurance program for non- industrial illnesses or injuries. Eligibility for benefits is subject to the requirements and approval of the STD/LTD insurance carrier.

SHORT TERM/LONG TERM DISABILITY INSURANCE. Section 1. Full-time employees and .5 FTE and above part-time employees may purchase short term disability coverage under the terms of the plan selected by the Employer. All benefits eligible Employees will be covered under the group LTD Plan selected by the Employer which provides sixty percent (60%) of base salary if disabled (maximum monthly benefit of $5,000.00).
SHORT TERM/LONG TERM DISABILITY INSURANCE. Benefits eligible employees may purchase short term disability coverage which provides sixty percent (60%) of base salary. Benefits for certified disabilities begin after a fourteen (14) day waiting period and may pay up to eleven (11) weeks of a disability. All benefits eligible employees will be covered under the employers group LTD Plan which provides sixty percent (60%) of base salary if disabled (maximum monthly benefit of $5,000.00). The benefit(s) identified in this Article shall be subject to Article 39 - Benefit Modification.

  • Long Term Disability Insurance 250. The City, at its own cost, shall provide to employees a Long Term Disability (LTD) benefit that provides, after a one hundred and eighty (180) day elimination period, sixty percent salary (60%) (subject to integration) up to age sixty-five (65). Employees who are receiving or who are eligible to receive LTD shall be eligible to participate in the City's Catastrophic Illness Program as set forth in the ordinance governing such program.

  • Long Term Disability Insurance Plan The Employer shall provide a mutually acceptable long-term disability insurance plan, a copy of which shall appear in Appendix “A” – Long-Term Disability Insurance Plan.   • Short Term Disability The Employer agrees to provide Short Term Disability benefits to all active full-time employees from the first (1st) day of an accident or the first (1st) full-time day of hospitalized or the fourth (4th) day of sickness. The Plan will pay sixty-six and two thirds percent (66 2/3%) of basic earnings for the first two (2) weeks, then Unemployment Insurance will pay fifteen (15) weeks, then the Plan will resume payments for thirty-five (35 weeks).

  • DISABILITY INSURANCE PLAN Management shall expend for active employees of this unit who are members of LACERS the sum necessary to cover the cost of a basic disability insurance plan. Management shall also maintain a Supplemental Disability Insurance Plan, enrollment in which is at the discretion of each employee. The full cost of the Supplemental Disability Insurance Plan premiums shall be paid by the individual employees who enroll in the plan.
