Sick Leave Assistance Plan. Each school district employee beyond their second successive full term of employment and eligible for sick leave benefits may elect to participate in the District sick leave assistance plan established by this policy. The purpose of the plan is to aide employees who have exhausted all of their accumulated sick leave through extended absence due to prolonged illness or catastrophic accident.

Sick Leave Assistance Plan a. Each District employee eligible for personal sick leave benefits (excluding worker’s compensation) may elect to participate in the Sick Leave Assistance Plan established by this policy. b. Each participant who has used all of the employee's personal sick leave (current and accumulated) may petition the Sick Leave Assistance Committee for sick leave assistance. However, such leave may not be granted until the employee has utilized consecutive full days of either paid or unpaid sick leave or combination thereof in an amount equal to 200 percent of their annual sick leave allocation. Extraordinary medical circumstances may be appealed to the Superintendent. c. The maximum number of sick leave days which the Sick Leave Assistance Committee may provide to a participant is 30 days annually. d. Such additional sick leave days shall not be deducted from the recipient’s future accumulated sick leave. However, each participant who has received assistance from the Sick leave Assistance Plan must contribute not less than one (1) day of the employee’s unused sick leave entitlement to the plan. e. The Sick Leave Assistance Committee shall be composed of one (1) representative from each of the employee groups participating in the Plan, to be selected by the respective employee groups. f. The Committee shall establish guidelines to govern its decision-making process regarding the granting or denial of request for sick leave assistance. These guidelines are specified on the application provided by the Human Resources Department and must be complied with completely. These guidelines provide a process of insuring that sick leave assistance is made available only to those applicants who demonstrate a need for such assistance. g. The application for additional sick leave shall be submitted to the Human Resources Department on a form provided to the buildings by the Human Resources Department. The Human Resources Department will forward copies to the Sick Leave Assistance Committee. A copy of the Committee’s decision will be forwarded to the employee, the building principal, the departmental director and the appropriate employee organization. h. The Human Resources Department shall annually make a written report to the Superintendent and the committee regarding the operation of the plan during the preceding year.
Sick Leave Assistance Plan. The sick leave assistance plan provides a specific number of accumulated sick leave days to a pool which allows the use of contributed sick leave days by a teacher who has exhausted his/her said allowance.
Sick Leave Assistance Plan. A sick leave assistance bank will be established for members of the bargaining unit under the following conditions and provisions: 1.6.1 Each participating teacher will contribute 8 hours (one (1) day) of his/her sick leave per year to the bank unless he/she gives written notice of intent not to participate on or before the tenth (10th) day of the current school year. Participation election is open only to teachers who are beginning their first year of service to the district. Eligible teachers declining participation in the bank will not be eligible to participate in the assistance bank at any future time. Persons who elect to cease contributing hours to the bank shall relinquish all rights to drawing hours from the bank. 1.6.2 This sick leave assistance bank is for the protection of individual teachers during a long-term extended illness, disability of the teacher, death or extended illness in the family. 1.6.3 Use of this bank may begin when the teacher has exhausted his/her own accumulated sick leave and personal leave. The teacher shall request hours from the bank by submitting a written statement of rationale and the number of hours requested. 1.6.4 A Leave Assistance Committee, consisting of three teachers appointed by the Association, the District Business Manager and one member of the school board appointed by the board, shall act to approve or deny requests for sick leave assistance within five working days of receipt of the written request. 1.6.5 Each individual request may not exceed 40 hours, however, individual teachers may submit multiple requests in any year. During any school year the committee may not grant any individual more than a total of 240 hours or 50% of the total number of days in the leave bank, whichever is smaller. If an individual’s requests exceed five (5) days, the committee may request a doctor’s statement identifying the reason for the extended leave request. Each 5 day request to draw upon the Bank must be submitted to the Leave Assistance committee within five school days of the requested usage. In extreme and unusual circumstances, exceptions may be made by the Leave Assistance committee. 1.6.6 Single day requests will be handled on a case by case basis with final consideration determined by the sick leave bank committee. 1.6.7 In the case of pregnancy, leave shall be granted if the mother is diagnosed with medical complications that meet the above definition. 1.6.8 Post-delivery leave will be granted if there are cont...

  • Sick Leave Plan The benefits of the Company’s Sick Leave Plan shall be considered as part of this Agreement. However, it is recognized that its provisions are not an automatic right of an employee and the administration of this plan and all decisions regarding the appropriateness or degree of its application shall be vested solely in the Company. The Company’s Sick Leave Plan will provide that probationary and regular employees will commence with a credit of eight days at 100 percent (100%) and 15 days at 75 percent (75%) pay, payable from the first day of sickness. This credit will continue to be available until the employee attains his/her first annual accumulation date as a regular employee. At the time of this accumulation date and each subsequent accumulation date he/she will acquire additional credits of eight days at 100 percent (100%) pay and 15 days at 75 percent (75%) pay. The accumulation of credits will be subject to the provisions of the Company’s Sick Leave Plan. Regular part-time employees shall receive a pro-rated number of sick days. When a regular part-time employee is absent due to illness on a scheduled day of work, they shall be paid for the hours of work scheduled for that day provided sick leave credits are available. Normally employees will be expected to arrange routine medical or dental appointments during non-working hours. Where such appointments cannot be arranged during non-working hours and the employee can be released from his/her duties, then the time shall be charged against an employee's sick leave time except in the case of medical appointments of less than half a day where normal earnings will be maintained. Employees who are on sick leave for 30 days or more may be eligible to participate in a vocational rehabilitation program in accordance with the Company’s policy. All major medical absence forms will be completed for any absence of four (4) continuous days/shifts or more or when requested by management. The Company will compensate the employee for the cost associated with completing these forms up to a maximum of $30.00. Additionally, the company will compensate the employee for the full cost of all medical notes, medical forms or medical information required to support LTD or other Wellness programs. This provision applies to Doctor’s notes requested by Line Management as part of the administration of the sick leave plan. Employees will be required to submit all forms required by management through their personal physician. Sick Leave benefits are conditional upon receipt of these forms and it is the responsibility of the employee to ensure that the employer receives these forms within a reasonable period of time. Any discipline related to sick leave that is imposed and grieved by the union will be referred directly to Xxxxxx Xxxxxxxxx for resolution.

  • Employee Assistance Program (EAP) Section 1. The Employer agrees to provide to the Union the statistical and program evaluation information provided to management concerning Employee Assistance Program(s). Section 2. No information gathered by an Employee Assistance Program may be used to discipline an employee. Section 3. Employees shall be entitled to use accrued sick leave for participation in an Employee Assistance Program. Section 4. Each university will offer training to local Union Stewards on the Employee Assistance Program available in their university, on university time, where an Employee Assistance Program is available.

  • Sick Leave Donation Program A Labor Management Committee will be established for the purpose of proposing rules and procedures for a new, program. The LMC will be to develop consistent, transparent and equitable proposals for processes across all departments within the City. The LMC shall also explore proposals to lower the minimum leave bank required to donate sick leave and permit donation of sick leave upon separation from the City. The LMC must consult with the Office of Civil Rights to ensure compliance with the City’s Race and Social Justice Initiative. Once the LMC has developed its list of proposals, the City and Coalition of City Unions agrees to reopen each contract on this subject.

  • Dependent Care Assistance Program The County offers the option of enrolling in a Dependent Care Assistance Program (DCAP) designed to qualify for tax savings under Section 129 of the Internal Revenue Code, but such savings are not guaranteed. The program allows employees to set aside up to five thousand dollars ($5,000) of annual salary (before taxes) per calendar year to pay for eligible dependent care (child and elder care) expenses. Any unused balance is forfeited and cannot be recovered by the employee.

  • Employee Assistance Drug and alcohol counseling, rehabilitation, and employee assistance are available from or through the Employer’s employee assistance program provider(s) (E.A.P.).

  • Sick Leave Policy It is the policy of the State of Ohio to not unreasonably deny sick leave to employees when requested.
