Sick Leave Supplement. When an employee suffering from a serious health condition as defined by FMLA exhausts his/her sick leave due to illness or injury and the employee is not eligible for salary replacement benefits (either on a full or partial basis) through another source, such as Worker’s Compensation, disability insurance, PERS benefits, or Social Security, the Association may request for a personal and/or sick leave donation from members of the bargaining unit with the following conditions:

Request the Association leadership group to ask for sick leave donation from members of the bargaining unit. It is not the obligation of the district to coordinate supplemental sick leave donations for association members. b. It is understood the donation is voluntary and minimum of four hours and a maximum of eight hours may be donated for any occurrence. 1) When an 8-hour Classified employee requestsa day” the donation across classification by a Certified employee will result in a deduction of 8 sick leave hours. 2) When a 7-hour Classified employee requests “a day” the donation across classification by a Certified employee will result in a deduction of 7 sick leave hours. 3) When a 6-hour Classified employee requests “a day” the donation across classification by a Certified employee will result in a deduction of 6 sick leave hours. 4) When a 5-hour Classified employee requests “a day” the donation across classification by a Certified employee will result in a deduction of 5 sick leave hours. 5) When a 4-hour Classified employee requests “a day” the donation across classification by a Certified employee will result in a deduction of 4 sick leave hours. 6) When a 3-hour Classified employee requests “a day” the donation across classification by a Certified employee will result in a deduction of 3 sick leave hours. c. An additional donation may be made if all donated hours are exhausted before the employee qualifies for other benefits. Sick leave supplemental donations must be received by the district office prior to the processing of payroll in any given month for employees to receive credit for donated leave. d. The donated hours will be used in the order in which they were contributed and any hours not used will be credited back to the donor. When a professional educator returns to work, any unused donated sick leave will be credited back to the donor. Donated hours are not cumulative and do not contribute to the employee’s leave bank for future use. An email notice will inform donors if their contribution was used or credited back. e. The district will give each individual donor e-mail notification as to whether or not their donated time was used in this occurrence, immediately following the occurrence. e. No sick leave bank is intended or established by the terms of this Agreement. f. It is understood that once an employee agrees to make a donation, that decision is final for this occurrence. g. Any Association grievance arising out of the implementation or the provisions of Section 11 shall render the provisions of this Section null and void. h. Maternity/paternity leave and well infant/child care appointments do not constitute an illness or injury regarding the sick leave supplement and do not qualify for this supplement.

  • Vacation; Sick Leave During the Employment Term, the Executive shall be entitled to not less than four (4) weeks of vacation during each calendar year and sick leave in accordance with the Company’s policies and practices with respect to its executives.

  • Sick Leave Plan The benefits of the Company’s Sick Leave Plan shall be considered as part of this Agreement. However, it is recognized that its provisions are not an automatic right of an employee and the administration of this plan and all decisions regarding the appropriateness or degree of its application shall be vested solely in the Company. The Company’s Sick Leave Plan will provide that probationary and regular employees will commence with a credit of eight days at 100 percent (100%) and 15 days at 75 percent (75%) pay, payable from the first day of sickness. This credit will continue to be available until the employee attains his/her first annual accumulation date as a regular employee. At the time of this accumulation date and each subsequent accumulation date he/she will acquire additional credits of eight days at 100 percent (100%) pay and 15 days at 75 percent (75%) pay. The accumulation of credits will be subject to the provisions of the Company’s Sick Leave Plan. Regular part-time employees shall receive a pro-rated number of sick days. When a regular part-time employee is absent due to illness on a scheduled day of work, they shall be paid for the hours of work scheduled for that day provided sick leave credits are available. Normally employees will be expected to arrange routine medical or dental appointments during non-working hours. Where such appointments cannot be arranged during non-working hours and the employee can be released from his/her duties, then the time shall be charged against an employee's sick leave time except in the case of medical appointments of less than half a day where normal earnings will be maintained. Employees who are on sick leave for 30 days or more may be eligible to participate in a vocational rehabilitation program in accordance with the Company’s policy. All major medical absence forms will be completed for any absence of four (4) continuous days/shifts or more or when requested by management. The Company will compensate the employee for the cost associated with completing these forms up to a maximum of $30.00. Additionally, the company will compensate the employee for the full cost of all medical notes, medical forms or medical information required to support LTD or other Wellness programs. This provision applies to Doctor’s notes requested by Line Management as part of the administration of the sick leave plan. Employees will be required to submit all forms required by management through their personal physician. Sick Leave benefits are conditional upon receipt of these forms and it is the responsibility of the employee to ensure that the employer receives these forms within a reasonable period of time. Any discipline related to sick leave that is imposed and grieved by the union will be referred directly to Xxxxxx Xxxxxxxxx for resolution.

  • Sick Leave Policy It is the policy of the State of Ohio to not unreasonably deny sick leave to employees when requested. It is also the policy of the State to take corrective action for unauthorized use of sick leave and/or abuse of sick leave. It is further the policy of the State that when corrective and/or disciplinary action is taken, it will be applied progressively and consistently. It is the desire of the State of Ohio that when discipline is applied it will serve the purpose of correcting the performance of the employee.

  • Sick Leave Benefits Sick leave is an indemnity benefit and not an acquired right. A Nurse who is absent from a scheduled shift on approved sick leave shall only be entitled to sick leave pay if the Nurse is not otherwise receiving pay for that day, and providing the Nurse has sufficient sick leave credits.

  • SICK LEAVE WITH PAY Sick leave with pay for employees shall be determined in the following manner:

  • Sick Leave Benefit Plan The Sick Leave Benefit Plan will provide sick leave days and short term disability days for reasons of personal illness, personal injury, including personal medical appointments and personal dental appointments.

  • Sick Leave Use An employee shall be granted sick leave with pay to the extent of the employee's accumulation for absences necessitated by the following conditions:

  • Vacation and Sick Leave During the Term, Executive shall be entitled to (a) sick leave in accordance with the Company’s policies applicable to similarly situated executive officers of the Company from time to time and (b) 4 weeks paid vacation each calendar year (up to 40 hours of which may be carried forward to a succeeding year).

  • Sick Leave Accrual All eligible employees shall accrue sick leave at the rate of four (4) hours per pay period of continuous employment beginning with their date of eligibility. Eligible employees being paid for less than a full eighty (80) hour pay period shall have sick leave accruals pro-rated in accord with the schedule set forth in Appendix D.

  • Sick Leave Allowance Faculty with a full-time assignment shall accrue sick leave at the rate of eight
