STUDENT ELIGIBILITY CRITERIA. In order to participate in the reverse transfer process, a student must meet the following requirements: X. Xx currently enrolled in a state university; B. Earned more than 30 hours of college credit at a single FCS institution; C. Transferred to a state university before earning an AA degree; D. Not previously earned an AA degree from the state university; E. Completed a minimum of 60 hours of college credit; F. Completed general education requirements of either the home FCS institution or the SUS institution; G. Not earned a baccalaureate degree; X. Xx in good academic standing and be current with financial obligations at the SUS institution; I. Be in good academic standing and be current with financial obligations at the awarding FCS institution; J. Have met the civic literacy requirement per section 1007.25(4), Florida Statutes; K. Have met college-level communication and computation competency per SBOE Rule 6A-10.030, Florida Administrative Code; L. Demonstrate competency in a foreign language per s. 1007.25(8); M. Consent to allowing the SUS institution to transmit the necessary information to the FCS institution; and N. Meet any other associate in arts degree graduation institutional requirement of the degree-granting FCS institution. If a reverse transfer candidate attended more than one FCS institution prior to transferring to an SUS institution, the degree-granting institution will be the FCS institution where the student earned the most credits, provided the student earned more than 30 credits at that institution. In the event the student has earned the same number of credits and meets the degree requirements at two or more FCS institutions, the FCS institution that the student attended most recently will be considered as the degree-granting institution. To adhere to the FERPA requirements, the student must actively affirm and agree (“opt in”) to the exchange of course histories and/or official transcripts among all the institutions attended. The SUS institution must obtain written consent (e.g., hard copy, electronic consent) from those students who appear to have the credits for associate degree completion prior to sending information to the FCS institution. Non-responding students will be considered to be non-consenting and will not be eligible. STUDENT ELIGIBILITY CRITERIA. To be eligible for the New Hampshire Transfer Guarantee, a student must have graduated with an associate degree from a New Hampshire community college and have a minimum GPA of 2.0, 2.5 or 3.0. Participating four-year receiving institutions will self-select a qualifying GPA for guaranteed admission in Appendix D. Admission is guaranteed given the student has met all eligibility criteria for the selected institution and the institution has available capacity within the intended major. Participating four-year institutions recognize that students who have completed an associate degree within the Community College System of New Hampshire have received a rigorous, high-quality education, and, thus, are prepared to succeed at a four- year institution. The New Hampshire Transfer Guarantee is, as described above, a guaranteed admissions model to community college transfer admissions. It should be distinguished from a 2+2 model to the extent that it does not involve guarantees of completion at the receiving institution within two years or 60 credits.
STUDENT ELIGIBILITY CRITERIA. Specialized transportation charges may be claimed, only for qualifying students. It is imperative to comply with the student selection criteria defined below when determining billable days of specialized transportation services. MO HealthNet payments will not be made to school districts for specialized transportation services delivered to ineligible students as those services would not be considered reimbursable. Students for whom specialized transportation costs are billed to the MO HealthNet program must meet ALL of the following criteria: 1. At the time of service, the student must be eligible for the MO HealthNet program. MO HealthNet participants who are eligible for State-only-funded programs are not eligible for specialized transportation services. Participants identified by ME codes 02, 08, 52, 55, 57, 59, 64, 65, 73, 74, 75, 80, 82, 89, 91, 92, 93 and 97 are not eligible for specialized transportation services. 2. The student must be receiving a MO HealthNet covered direct service identified through the students Individualized Education Program (IEP). A school district may provide only the specialized transportation service necessary to support the delivery of the MO HealthNet approved direct service(s) it gives a student. (i.e., occupational therapy, speech therapy, etc.). Specialized transportation services associated with a MO HealthNet participant receiving assessment procedures conducted by or on behalf of the school district are billable only when the student is found to have a disability and an IEP is developed. 3. The student's IEP must specify the need for a MO HealthNet covered direct service as well as specialized transportation, and at least one of the three criteria from a-c below must be met. Specialized transportation must be specified in the IEP as a related service. For the provision of specialized transportation services, the IEP serves as the plan of care and authorization for the need for service. School districts are reminded of their obligation to provide, at no cost to the parent, any service agreed to be educationally necessary on an IEP. a. STUDENT ELIGIBILITY CRITERIA. To meet eligibility requirements for participation in the Dual Enrollment Program at HCC, home education students shall meet the following criteria: a. Provide proof of enrollment in a home education program, pursuant to s. 1002.41, F.S.; b. Be responsible for his or her own transportation; c. Demonstrate acceptable placement test scores on the PERT, SAT or ACT (not more than two (2) years old) for entrance into college level courses. STUDENT ELIGIBILITY CRITERIA. I. The Board shall identify the students qualified for participation in the Dual Enrollment Program.   • Service Eligibility Criteria 5.3.4.1 High capacity EELs must comply with the following service eligibility requirements. <<customer_short_name>> must certify for each high-capacity EEL that all of the following service eligibility criteria are met: 5.3.4.1.1 <<customer_short_name>> has received state certification to provide local voice service in the area being served; 5.3.4.2 For each combined circuit, including each DS1 circuit, each DS1 EEL, and each DS1-equivalent circuit on a DS3 EEL: 5.3.4.2.1 1) Each circuit to be provided to each End User will be assigned a local number prior to the provision of service over that circuit; 5.3.4.2.2 2) Each DS1-equivalent circuit on a DS3 EEL must have its own local number assignment so that each DS3 must have at least twenty-eight (28) local voice numbers assigned to it; 5.3.4.2.3 3) Each circuit to be provided to each End User will have 911 or E911 capability prior to provision of service over that circuit; 5.3.4.2.4 4) Each circuit to be provided to each End User will terminate in a collocation arrangement that meets the requirements of 47 C.F.R. § 51.318(c); 5.3.4.2.4 5) Each circuit to be provided to each End User will be served by an interconnection trunk over which <<customer_short_name>> will transmit the calling party’s number in connection with calls exchanged over the trunk; 5.3.4.2.5 6) For each twenty-four (24) DS1 EELs or other facilities having equivalent capacity, <<customer_short_name>> will have at least one (1) active DS1 local service interconnection trunk over which <<customer_short_name>> will transmit the calling party’s number in connection with calls exchanged over the trunk; and 5.3.4.2.6 7) Each circuit to be provided to each End User will be served by a switch capable of switching local voice traffic. 5.3.4.3 BellSouth may, on an annual basis, audit <<customer_short_name>>’s records in order to verify compliance with the qualifying service eligibility criteria. The audit shall be conducted by a third party independent auditor, and the audit must be performed in accordance with the standards established by the American Institute for Certified Public Accountants (AICPA). To the extent the independent auditor’s report concludes that <<customer_short_name>> failed to comply with the service eligibility criteria, <<customer_short_name>> must true-up any difference in payments, convert all noncompliant circuits to the appropriate service, and make the correct payments on a going-forward basis. In the event the auditor’s report concludes that <<customer_short_name>> did not comply overall in any material respect with the service eligibility criteria, <<customer_short_name>> shall reimburse BellSouth for the cost of the independent auditor. To the extent the auditor’s report concludes that <<customer_short_name>> did comply in all material respects with the service eligibility criteria, BellSouth will reimburse <<customer_short_name>> for its reasonable and demonstrable costs associated with the audit. <<customer_short_name>> will maintain appropriate documentation to support its certifications. 5.3.4.4 In the event <<customer_short_name>> converts special access services to UNEs, <<customer_short_name>> shall be subject to the termination liability provisions in the applicable special access tariffs, if any.

  • Eligibility Criteria Students seeking college credit through this bilateral articulation agreement must meet the following criteria/requirements: • The student must have completed the secondary Tech Prep competencies (academic and technical) relevant to the Central Ohio Technical College courses, with a grade of A, B, or C. • An application and subsequent matriculation to Engineering Technology at Central Ohio Technical College must occur within 3 years of the Adult Education completion date. • The student must meet all college entrance, general admission and program specific requirements and complete the Accuplacer or ACT assessment. • The student is responsible to inform college admissions personnel of his/her eligibility for articulated college credit and verify that appropriate documentation has been provided to the college by the secondary school. • Secondary instructors are required to have the appropriate teaching credential as defined by the Ohio Department of Education. • For the purpose of compliance with state, program or regional accreditation standards, Central Ohio Technical College reserves the right to review, validate and copy the credentials (e.g., college transcripts and resumes) of the instructors of articulated courses for external auditing purposes. • The secondary instructor(s) will complete an Instructor Recommendation Form for each student upon completion which will be maintained in the student’s secondary permanent file. An Instructor Recommendation Form must be submitted to the Records and Registration Office prior to receiving credit. • The college and secondary school will develop methods of publicizing the agreement to encourage students to take advantage of seamless transitions and advanced placement opportunities. • There will be no charge for college credit awarded through this agreement. • Student eligibility for technical or related credit is primarily determined by high school teachers evaluation based upon predetermined criteria set by the college. In certain cases, record of relevant student external certification, college proficiency testing, review of portfolios, or other forms of assessment may also be required. • Placement in Central Ohio Technical College’s courses is determined by the student’s scores on the college’s required placement test(s). Credit is available only for the technical courses specified on the attached list of articulated courses. • Articulated courses are recorded on the student’s permanent records after a student enrolls in and completes a non-articulated college credit course at Central Ohio Technical College. The administrators and faculty of the program at both levels pledge their commitment and support to continuing this relationship and to promoting these articulation opportunities to the students.

  • Student Eligibility The LEA and POSTSECONDARY INSTITUTION shall qualify and advise candidates for dual credit from the pool of eligible high school students. A candidate for dual credit is eligible for consideration for fall, spring, and summer semesters if he or she: a. is enrolled during the fall and spring in a LEA in one-half or more of the minimum course requirements approved by PED for public school students under its jurisdiction or by being in physical attendance at a bureau of Indian education-funded high school at least three documented contact hours per day pursuant to 25 CFR 39.211(c); b. obtains permission from the LEA representative (in consultation with the student’s individualized education program team, as needed), the student’s parent or guardian if the student is under 18 years old, and POSTSECONDARY INSTITUTION representative prior to enrolling in a dual credit course; and c. meets POSTSECONDARY INSTITUTION requirements to enroll as a dual credit student.

  • General Criteria (a) If general criteria are required as part of the Application, only one (1) set shall be completed. General criteria measure the quality of the Land Manager’s overall OHV program. The Applicants shall answer these questions with respect to the entirety of the OHV operation, including all trails and areas available for OHV Recreation, not just specific Projects or Project Areas.   • Dependent Eligibility For all programs covered in this article, eligible dependents are an employee’s lawful spouse or domestic partner (as defined by Section 297 of the California Family Code), and unmarried children (natural, step, adopted, legal guardianship, and/or xxxxxx) of the employee or domestic partner, who are qualified IRS dependents of the employee or domestic partner, up to twenty-three (23) years of age. Disabled dependents may be able to continue coverage beyond the limiting age if the disability occurred while the dependent was covered under a County-sponsored medical plan or prior to the dependent’s 19th birthday, and is certified by a licensed physician.

  • Eligibility Requirements The Trustee hereunder shall at all times (i) be a corporation or association having its principal office in a state and city acceptable to the Seller, organized and doing business under the laws of such state or the United States of America, authorized under such laws to exercise corporate trust powers, having a combined capital and surplus of at least $50,000,000, or shall be a member of a bank holding system, the aggregate combined capital and surplus of which is at least $50,000,000, provided that its separate capital and surplus shall at all times be at least the amount specified in Section 310(a)(2) of the Trust Indenture Act of 1939, (ii) be subject to supervision or examination by federal or state authority and (iii) have a credit rating or be otherwise acceptable to the Rating Agencies such that neither of the Rating Agencies would reduce their respective then current ratings of the Certificates (or have provided such security from time to time as is sufficient to avoid such reduction) as evidenced in writing by each Rating Agency. If such corporation or association publishes reports of condition at least annually, pursuant to law or to the requirements of the aforesaid supervising or examining authority, then for the purposes of this Section the combined capital and surplus of such corporation or association shall be deemed to be its combined capital and surplus as set forth in its most recent report of condition so published. In case at any time the Trustee shall cease to be eligible in accordance with the provisions of this Section, the Trustee shall resign immediately in the manner and with the effect specified in Section 8.08.

  • Criteria (1) Annual Evaluation Criteria. All performance evaluations shall be based upon assigned duties, and shall carefully consider the nature of the assignment in terms, where applicable, of: a. Teaching effectiveness, including effectiveness in presenting knowledge, information, and ideas by means or methods such as lecture, discussion, assignment and recitation, demonstration, laboratory exercise, practical experience, supervision of interns, theses, professional projects and/or dissertations, and direct consultation with students. The evaluation shall include consideration of effectiveness in imparting knowledge and skills, and effectiveness in stimulating students' critical thinking and/or creative abilities, the development or revision of curriculum and course structure, and adherence to accepted standards of professional behavior in meeting responsibilities to students. The evaluator may take into account class notes, syllabi, student exams and assignments, and any other materials relevant to the employee's teaching assignment. The teaching evaluation must take into account any relevant materials submitted by the employee, including the results of peer evaluations of teaching, and may not be based solely on student evaluations when this additional information has been made available to the evaluator. b. Contribution to the discovery of new knowledge, development of new educational techniques, and other forms of creative activity. Evidence of research and other creative activity shall include, but not be limited to, published books; articles and papers in professional journals; musical compositions, paintings, sculpture; works of performing art; papers presented at meetings of professional societies; funded grant activities; and research and creative accomplishments that have not yet resulted in publication, display, or performance. The evaluation shall include consideration of the employee's productivity, including the quality and quantity of the employee's research and other creative programs and contributions during the year, as well as recognition by the academic or professional community of what has been done. c. Public service that extends professional or discipline-related contributions to the community, the State, public schools, and/or the national and international community. This public service includes contributions to scholarly and professional organizations, governmental boards, agencies, and commissions that are beneficial to such groups and individuals. d. Participation in the governance processes of the University through significant service on committees, councils, and senates, beyond that associated with the expected responsibility to participate in the governance of the University through participation in regular departmental or college meetings. e. Other assigned University duties, such as attending University events, advising, counseling, and academic administration, or as described in a Position Description, if any, of the position held by the employee. Other assigned duties may include entrepreneurial activities that contribute to the further development of the University with an end result of creating a new venture. Evidence of entrepreneurial contributions shall include, but not be limited to, creation of self- supporting centers or institutes, development of multi- disciplinary research partnerships, and applications of research to implementations in society.
