Choice of Benefits. If an employee is eligible for a pension benefit pursuant to the provisions of this Appendix L and another pension benefit under a Union Pension Plan or this Appendix L that is greater, the employee may, at the time of retirement, choose to receive either benefit.
Choice of Benefits a. The District will offer bargaining unit members hired on or before June 30, 2002 a choice of three medical benefits plans during their continuous employment with the District: (1) Blue Cross/Blue Shield/Anthem Plan JW with Managed Care, (2) Blue Cross/Blue Shield/Anthem point of service (POS) plan, and (3) Blue Cross/Blue Shield/Anthem health maintenance organization (HMO) plan.
b. The District will offer bargaining unit members hired on or after July 1, 2002 a choice of two medical benefits plans: (1) Blue Cross/Blue Shield/Anthem POS plan and (2) Blue Cross/Blue Shield/Anthem HMO plan.
Choice of Benefits. 1. The District will offer eligible bargaining unit members who successfully complete the probationary period enrollment in a Consumer-Driven Health Plan (CDHP – Yellow Open Access).
2. The District and bargaining unit members will contribute toward the cost of bargaining unit members' elected benefit plan at the rates set out in sections (1) and (2) below. Said annual contribution from bargaining unit members shall be paid through payroll deduction. The annual cost of the medical benefits plans shall be provided on or before June 1 of each year.
3. The District will provide a single membership in the CDHP (Yellow Open Access) with the negotiated contribution set out below to Assistants who are scheduled to work at least 30 hours per week. Employees who are scheduled to work at least 25 hours per week and were enrolled in a District health insurance plan as of June 30, 2009, will be allowed to remain in the District health insurance plan under the same terms and negotiated contributions as Assistants who are scheduled to work at least 30 hours per week.
4. The District and the bargaining unit member will contribute the following amounts toward the annual cost of a single membership in medical benefits for bargaining unit members who elect to enroll in the CDHP (Yellow Open Access) plan:
a. For the duration of this Agreement and for those who were enrolled in a District health insurance plan as of June 30, 2015, the District will contribute 95% and the bargaining unit member will contribute 5% of the annual cost of a single membership in the CDHP (Yellow Open Access) plan.
b. For the duration of this Agreement and for those who were either eligible to enroll but were not enrolled in a District health insurance plan as of June 30, 2015, or for those who were hired on or after July 1, 2015, the District will contribute 85% and the bargaining unit member will contribute 15% of the annual cost of a single membership in the CDHP (Yellow Open Access) plan.
5. The annual payment will be provided through payroll deduction. The District shall file with the Internal Revenue Service to qualify these medical benefit contributions as pre-tax deductions pursuant to Section 125 of the Internal Revenue Code. As early as possible, the District will file to qualify all other allowable benefit contributions as pre-tax deductions pursuant to Section 125 of the Internal Revenue Code. Said contributions shall be equally divided among 20 pay periods.
6. Health and dental benefits...
Choice of Benefits. 1. The District will offer Employees who were hired on or before June 30, 2002 and are scheduled to work thirty (30) hours or more per week a choice of three medical benefits plans: (1)Blue Cross/Blue Shield/Anthem Plan JW with Managed Care or its equivalent, (2) a point of service (POS) plan, and (3) a health maintenance organization (HMO) plan. (See Exhibit D.)
2. The District will offer Employees who were hired on or after July 1, 2002 and are scheduled to work thirty (30) hours or more per week a choice of two medical benefits plans: (1) a POS plan and (2) an HMO plan.
3. Eligible employees (as defined in Section 16.02,A,1 above) who elect to enroll in the Blue Cross/Blue Shield/Anthem Plan JW with Managed Care or its equivalent will contribute the following toward the cost for the Employee’s health plan (i.e., single, two-person or family) as determined by the District on or before September 1 of each year:
a. For the 2006-2007 contract year, the payment will be 16.5% of the audited annual cost of the plan subscribed to by the Employee.
b. For the 2007-2008 and 2008-2009 contract years, the payment will be 19% of the audited annual cost of the plan subscribed to by the Employee.
c. The District will no longer offer Employees the JW Plan on or after July 1, 2009.
d. Eligible employees (as defined in Section 16.02,A,1 above) who elected to change their enrollment prior to July 1, 2006 from the JW plan to the HMO plan during the 2006-2007 contract year may continue to participate in the HMO plan for the 2006-2007 plan year with no contribution toward the cost of the Employee's HMO coverage. After the completion of the 2006-2007 plan year, an Employee who elects to remain on the HMO plan will contribute toward the cost of the HMO plan as provided in the then current Agreement between the parties. An Eligible Employee who changed to HMO enrollment during the 2006-2007 contract year prior to July 1, 2006 may also elect to return to either the JW or POS plan during an open enrollment period. Contributions to the Employee's selected health plan will then be as provided in the then current Agreement between the parties.
4. Employees who elect to enroll in the POS plan will contribute the following toward the cost for the individual Employee’s health plan (i.e., single, two-person or family) as determined by the District on or before September 1 of each year:
a. For the 2006-2007 contract year, the payment will be 11.5% of the audited, annual cost of the plan s...
Choice of Benefits. Each full−time unit employee may elect insurance coverage according to one of the options, MESSA Plan A or Plan B, as defined herein below with an annual Employer contribution capped as follows:
a. PLAN A: MESSA Choices II with XVA2 Rider, Saver Rx
b. PLAN B: Elect no insurance plan. Cash in lieu amount to be set at 50% of the Plan A single benefit package.
c. Employee premium contributions to be paid through a Section 125 pre−tax account. Employees will be responsible for 15% of the insurance premium or cost beyond the caps in Section C1, whichever is less costly to the district.
d. The district shall offer flex accounts for beginning unit members for medical and dependent care expenses. Flex accounts will align with calendar year (January 1 − December 31). The Flex medical plan is not available to members electing MESSA abc.

(See Paragraph 3 below for exceptions to this provision).
3. Unit members hired on or after July 1, 2013 and all new enrollees shall contribute 15% toward the cost for the individual’s health plan (i.e., single, two-person or family) as determined by the District on or before September 1 of each year.

Choice of Benefits. Each full‐time unit employee may elect insurance coverage according to one of the options, as defined herein below with an annual Employer contribution capped as follows: Hard Caps as identified above include health insurance coverage only (premiums, taxes, fees and deductibles all apply to the cap). The District will pay 20% of the costs of Dental, Vision, Life, AD&D and LTD for those electing health insurance.
Choice of Benefits. 1. The District will offer employees scheduled to work thirty (30) hours or more per week one medical benefit plans: a Consumer-Driven Health Plan (CDHP) (Exhibit D).
2. Employees who elect to enroll in the (CDHP) plan will contribute the following toward the cost for the individual employee’s health plan (i.e., single, two-person or family) as determined by the District on or before September 1 of each year: For the 2013-2014 and 2014-2015, contract years, the payment will be 0% of the audited, annual cost of the plan subscribed to by the employee. (See Paragraphs 3 and 4 below for exceptions to this provision). For the 2015-2016 contract year, the payment will be 5% of the audited, annual cost of the plan subscribed to by the employee. (See Paragraphs 3 and 4 below for exceptions to this provision).
3. Beginning in the 2013-2014 contract year, all unit members hired on or after July 1, 2013 shall contribute 15% toward the cost for the individual’s health plan (i.e., single, two- person or family) as determined by the District on or before September 1 of each year.
4. Beginning in the 2013-2014 contract year, those unit members who are eligible to enroll in the District’s health plan but are not enrolled as of July 1, 2013, and choose to enroll in the CDHP plan will contribute 5% of the audited, annual cost of the plan subscribed to by the employee.
5. Effective in the 2015-2016 contract year, health insurance-eligible employees will have an option to return to an HMO plan (Exhibit D1) only if the union and the District agree to employer/employee contribution rates. The union must notify the District of its intent to return to an HMO plan by no later than March 1, 2015.
