Collaborative Management Program Elements. Pursuant to section 24-1.9-102(2)(i) the Parties hereby determine that they will attempt to meet performance measures specified by the Colorado Department of Human Services, in conjunction with the Collaborative Management Program State Steering Committee, and elements of collaborative management in order to:
A. Reduce duplication and eliminate fragmentation of services provided to children, youth and families who would benefit from integrated multi-agency services by bringing together the agencies that are currently involved with referred families (or that could be) and then addresses the family and child(ren)’s strengths, areas of concern, where the family is currently receiving services and then what gaps in service exist. During the ISST Meeting, as the team addresses gaps in service as well as additional needs presented by the family, agencies work together to create a plan to take the steps necessary toward reaching their goals.
B. Increase the quality, appropriateness, and effectiveness of services delivered to children and families who would benefit from multi-agency services, to achieve better outcomes; the Parties will increase the quality, appropriateness, and effectiveness of services delivered through: Routt County ISST will increase the quality, appropriateness, and effectiveness of services delivered to children and families who would benefit from multi-agency services, to achieve better outcomes. We do this by incorporating a Wrap-Around Model within the meetings and following our 10 Guiding Principles below:

A. Reduce duplication and eliminate fragmentation of services provided to children, youth and families who would benefit from integrated multi-agency services. The Parties will reduce duplication and fragmentation of services by:
a. The partner agencies will refer youth early on to the Youth Engaging in Supportive Services (YESS) and Children and Families Engaging in Supportive Services (FACESS) for purposes of collaborative assessment and service planning. The Team will work to avoid duplication of services, establish a realistic individualized support plan and assure provider follow through. Communication between agencies, providers, and family members and collaborative integration will be priorities of the Team.
B. Increase the quality, appropriateness, and effectiveness of services delivered to children and families who would benefit from multi-agency services, to achieve better outcomes; the Parties will increase the quality, appropriateness, and effectiveness of services delivered through:
a. Each ISST and family will implement services designed to promote family wellbeing in the least restrictive, least intrusive, and most culturally competent manner possible. Service planning and provision shall be strengths based, family focused and well-coordinated.
C. Encourage cost sharing among service providers. The Parties will encourage cost sharing through:
a. Cost sharing is addressed at both the family plan level and ISST level. Partners to the MOU regularly discuss and approve cost sharing measures to ensure a strong continuum of service. Mandated and non-mandated partners agree to, at a minimum, provide their time at bi-monthly IOG meetings to discuss policy, procedure, best practice, protocol, and community needs. Additional in-kind sources, such as administrative costs and staff time at collaborative meetings, are documented in the yearly MOU.

The Parties will reduce duplication and fragmentation of services by: Building a local System of Collaborative Management to effectively examine practices and processes, engaging in a collective quality improvement process to identify and address duplication, fragmentation, and gaps in service
B. Increase the quality, appropriateness, and effectiveness of services delivered to children and families who would benefit from multi-agency services, to achieve better outcomes; the Parties will increase the quality, appropriateness, and effectiveness of services delivered through: Examining structures and practices across systems and the impacts of service delivery models on CYF and systems. RCMP engages in a monthly collective quality improvement process to identify and address improvements and opportunities to improve impacts and quality in practice strategies (at the systemic and service delivery level). This work includes the input of Family and Youth Representatives on the IOG and in the Community and includes application of a meaningful evaluation and quality improvement plan for continuous improvement in alignment with the values and principles of the Statute and to ensure an appropriate individualized approach for the very best outcomes and effective resource utilization/integration. This approach includes obtaining feedback from CYF engaged in services, service providers and systems and leads to a systematic examination of practice, identification of issues, and problem solving for improved impacts to CYF and community partners. RCMP is dedicated to developing a successful strategy for the collection of data useful in achieving these goals and to aligning systems for success.
C. Encourage cost sharing among service providers. The Parties will encourage cost sharing through: Defining the focus of the collaborative and developing a strategic action plan to achieve those goals, including examination and agreement on the funding of those goals. A. Reduce duplication and eliminate fragmentation of services provided to children, youth and families who would benefit from integrated multi-agency services. The Parties will reduce duplication and fragmentation of services by: holding ISSTs which include all systems involved with the family and sharing each system’s goal with the family to determine how to reduce duplication and fragmentation and create an integrated plan for the family.
B. Increase the quality, appropriateness, and effectiveness of services delivered to children and families who would benefit from multi-agency services, to achieve better outcomes; the Parties will increase the quality, appropriateness, and effectiveness of services delivered through: continuous quality improvement through review of general trends, resource utilization, and other data at the KCC IOG and ISSTs.
C. Encourage cost sharing among service providers. The Parties will encourage cost sharing through: addressing funding needs and issues at ISSTs and IOG meetings as well as collaboration on grants to serve the needs of Kit ▇▇▇▇▇▇ County.
