Contents of Grievance. Any grievance or appeal shall contain the following:
4.1.8.1 The Article and Section alleged to have been violated.
4.1.8.2 A description of the specific grounds of the grievance including the names, date and places necessary to understand the grievance.
4.1.8.3 A listing of the specific action(s) requested in order to remedy the grievance.

All grievances must be submitted to the Chancellor or his/her designee on the grievance form contained in this section, dated and signed by the grievant, the designated Federation grievance representative, and the Federation President or his/her designee. The employer may request the Federation to reconsider and resubmit any grievance filed which is not in substantial compliance with this section. The employer shall inform the Federation of any such refusal and specify the deficiencies in the grievance. The Federation shall have an additional ten (10) days to resubmit a corrected grievance. If a corrected grievance is not submitted within ten (10) days, the right to grieve is waived.

1. It shall be specific.
2. It shall contain a synopsis of the facts giving rise to the dispute, controversy or issue.
3. It shall specify the section of the contract or rule or regulation or statute or ordinance which has been allegedly violated, misapplied or as to which the dispute arises.
4. It shall state the relief requested.
5. It shall contain the date of the alleged dispute, controversy or issue.
6. It shall be signed by the grievant.

b) specify the article(s) which are alleged to have been violated;
c) contain a precise statement of the facts giving rise to the grievance;
d) indicate the redress sought; and
e) be signed by the grievor.

2. Section of Agreement allegedly violated.
3. Date of occurrence of the alleged violation.
4. Relief sought.
Contents of Grievance a. Specific nature of the grievance including the article(s) of the MOU alleged to be involved and the identify of any RN(s).
b. The time or place of its occurrence.
c. The consideration given or steps taken to secure informal resolution, when applicable.
d. The corrective action desired. Such corrective action will not include action taken against a supervisor or other employee (e.g., disciplinary action against another).
Contents of Grievance. The statement of grievance shall:
(a) identify action taken to address the complaint at the informal level;
(b) be in writing, naming the party(ies) involved;
(c) contain a precise statement of the facts giving rise to the grievance;
(d) indicate the redress sought; and
(e) be signed by the grievor and the grievance officer in the event of a teacher grievance.
Contents of Grievance. 1. In filing a formal grievance, the employee must set forth the following information:
(a) The specific section of Personnel & Salary Ordinance or this agreement allegedly misapplied.
(b) The specific act or omission which gave rise to the alleged misapplication.
(c) The date or dates on which the misapplication occurred.
(d) What documents, witnesses or other evidence support the employee’s assertion.
(e) The remedy requested.
(f) Signature of aggrieved employee and date.
2. An employee’s failure to provide the required information shall result in the prompt return of the written formal grievance to the employee for completion. Any delay on the part of the employee to resubmit his grievance shall not extend the time limits for submittal of a grievance, except by written approval of the Director of Operations.
