Employee and Family Assistance Program (EFAP. The Employer agrees to provide an EFAP as described in the Joint Stakeholder Agreement dated 29 March 2007 and as amended from time to time by the EAP Board. The Stakeholder Agreement may be modified from time to time with the approval of the EAP Board following consultation with all parties to the Agreement. The Association will provide a representative to the EAP Board as per the Stakeholder Agreement.
Employee and Family Assistance Program (EFAP a. The present Employee and Family Assistance Program will remain in effect unless changed by the mutual agreement of the Employer and the Union. The Employer shall pay 50 percent of the premium cost.
b. This plan shall be administered by a Joint Committee consisting of one representative of the Employer and one representative of the Union.
Employee and Family Assistance Program (EFAP. The Corporation will pay 100% of the cost of an Employee and Family Assistance Program (EFAP) as provided by the Corporation. In the event of a strike or lock- out, the Corporation will not xxxx the CUPE National or the local for this benefit.
Employee and Family Assistance Program (EFAP. A mutually acceptable employee and family assistance program. Coverage will be effective on the first day of the month following receipt by a continuing or probationary employee of their first pay. Enrolment in the plan for such employees is compulsory. Provides short-term counselling, advisory and information service to employees and their eligible family members for personal problems that affect work life, family life, and general well-being.
Employee and Family Assistance Program (EFAP. The Employer agrees to provide a professional and confidential EFAP service in full consultation with the Union.
Employee and Family Assistance Program (EFAP. The Employer agrees to provide a mutually agreeable Employee and Family Assistance Program (EFAP). The Union agrees to assign any Employment Insurance premium reduction rebate to the Employer to assist in funding the EFAP.
Employee and Family Assistance Program (EFAP. The Board shall be responsible for administering and paying for the EFAP, and the employer portion of all the following statutory benefits: Canada Pension Plan, Employment Insurance, Employer Health Tax.
L8.02 Benefits While on Leave See also L – Article 11: Leaves
L8.02.01 All teachers on unpaid leaves of absence and long term disability shall be eligible to be a member of the benefit plan provided the teacher pays the full cost of the premiums.
L8.02.02 Teachers on unpaid leaves of absence, with the exception of those on statutory Pregnancy/Parental Leave or during the LTD qualifying period, shall be eligible to be a member of the benefit plan (subject to the insurer eligibility provisions), provided the teacher makes the arrangements directly with the insurer and pays the full premium cost to the insurer.
L8.02.03 Teachers on leaves of absence or on LTD will be responsible for paying directly to the insurer all administration fees, charged by the insurer, for the continuance of their benefits while on leave or LTD.
L8.02.04 All teachers on long term disability and during the LTD qualifying period retain full entitlement to all benefits provided under L – Article 8 and shall make arrangements for benefits directly with the insurer.
L8.02.05 Teachers who qualify for LTD, or who are absent from work while they are in the appeals process shall be eligible for benefits subject to the rules of the OSSTF ELHT.
L8.02.06 The rate paid by the teachers in L8.02.04 shall be the group rate charged by the insurer.
Employee and Family Assistance Program (EFAP. 1.1 Deduction and payment of premiums Each month, the school board shall deduct the full one hundred percent (100%) of the premiums for each teacher in order to establish and maintain an EFAP. These premiums shall become part of the provincial LTDIP stipulated in the long-term disability insurance plan clause. The monthly premiums thereby required shall be paid to the AEFO's LTDIP administrator, which administers the services related to this coverage.
1.2 Notices of change of premium or change of administration procedures The AEFO or the LTDIP administrator shall notify the school board of changes of premiums in writing, at least 45 days before the new premiums come into effect.
Employee and Family Assistance Program (EFAP. There shall be no mandatory referral to any Employee and Family Assistance Program (EFAP) and no employee shall be required to provide private information as a result of voluntarily attending EFAP or counseling. The Employer shall keep confidential any private information of which it becomes aware.
Employee and Family Assistance Program (EFAP. The Employer shall provide Members with copies of the group benefits booklets and shall place copies of current booklets on the Human Resources web site.
