GRIEVANCE APPEALS COMMITTEE. The Committee shall be composed of three (3) people selected as follows: - one (1) appointee made by the Union; - one (1) appointee made by the Agency; - one (1) member chosen by mutual agreement of the other two members. If the Union and Agency representatives cannot agree on a third party, they will contact State Mediation and Conciliation for a list of five (5) arbitrators. The parties shall alternately strike names from the list until only one remains. The remaining arbitrator shall serve as the third person on the Committee. Which party strikes the first name shall be determined by chance, such as a tossing a coin.

GRIEVANCE APPEALS COMMITTEE. An employee who is aggrieved by any report, recommendation or action by SA Lotteries may seek to resolve such grievance in accordance with the following procedure, provided however the decision to terminate employment shall not be subject to the provisions of this clause.
GRIEVANCE APPEALS COMMITTEE. A grievance which has not been resolved at the level of the Chancellor may be submitted by any interested party to a grievance appeal committee. There shall be three members of the grievance appeals committee. One shall be appointed by the Chancellor; one by the President of Local 891; and a permanent impartial arbitrator will be selected as Chairman of the Grievance Appeals Committee and will serve at the pleasure of the parties. The proceeding may be initiated by filing with the Department and the Chairman or the American Arbitration Association, as the case may be, a notice of arbitration. The notice shall be filed within ten (10) working days of the receipt of the decision of the Chancellor. The notice shall include a brief statement setting forth precisely the issues to be decided by the grievance appeals committee and the specific provision of the agreement involved.
GRIEVANCE APPEALS COMMITTEE a. The grievant may appeal the City Manager’s decision by completing Section 5 of the grievance form and filing it with the Personnel Office within ten (10) working days.

  • Appeals Committee ‌ An Appeals Committee is hereby established composed of one member appointed by the Union, one member appointed by the Employer or by the Association, as the case may be, and a Public Member appointed by both these members.

The determination letter will provide you with the rationale for our response as well as information on any possible next steps available to you. When filing a complaint, please provide the following information: • your name, address, member ID number; • the date of the incident or service; • summary of the issue; • any previous contact with BCBSRI concerning the issue; • a brief description of the relief or solution you are seeking; and • additional information such as referral forms, claims, or any other documentation that you would like us to review.   • Benefits Committee As per LOA#10, a benefits committee comprised of the employee representatives and the employer representatives, including the Crown, shall convene upon request to address all matters that may arise in the operation of the OSSTF ELHT.

  • Review Committee A Student may ask that the decision of the Housing Director or designee to deny the cancellation be reviewed. The review will be conducted by a committee consisting of University officials.

  • Claims Review The IRO shall perform the Claims Review annually to cover each of the five Reporting Periods. The IRO shall perform all components of each Claims Review.

  • Disciplinary Appeals All forms of disciplinary action which are not appealable to the Civil Service Commission or the courts, except written or oral reprimands and Forms 475, shall be subject to review through Steps 3, 4, 5 and 6 of the grievance procedure.

  • Grievance Commissioner The Employer and Union may mutually agree in writing to invoke the Commissioner Process rather than proceed to arbitration as set out in this Collective Agreement. All cases referred to arbitration where an employee has a grievance concerning discipline will only be referred to a Grievance Commissioner if the Employer and the Union agree in writing on all the facts. The parties may also agree to group grievances before a single Grievance Commissioner. A Grievance Commissioner (where more than one, acting in rotation) will set aside such time as may be requested by the Employer and the Union to consider and determine grievances referred to them. A Grievance Commissioner shall have the same powers and be subject to the same limitations as a Board of Arbitration hereunder, save and except as expressly provided in 9.18 to 9.22 hereof.
