HMIS Participation. All agencies providing homeless services in receipt of funding from the CITY’s CDBG Program are required to fully participate in the Homeless Management Information System (“HMIS”) and work closely with the County of Santa Xxxxx Office of Supportive Housing (“CSC OSH”), to ensure the agency has the mechanisms and staffing in place to use the system appropriately and in a timely manner. Funded agencies are required to collect demographic information on all clients served by the funded projects, the services provided, and consent to release the information to CSC OSH and the CITY’s Housing Department. Funded projects must utilize all appropriate aspects of HMIS in order to generate the statistical information required for reporting to the CITY on all universal and program level elements of the HUD Data Standards. These statistical reports must be generated directly out of HMIS. No adjustments to the HMIS reports will be accepted and it is therefore incumbent on the agency to ensure that the information they put into HMIS is accurate and up to date. CITY will measure performance and outcomes relating to these funded projects through the use of the HMIS statistical data, based on the HUD data elements, or other reporting requirements as determined by the CITY. The CITY will request from the HMIS Administrator, acknowledgement of the recipient agencies’ certificate of compliance with HUD privacy and security standards, acknowledgement of use of the Shelter Point program, and statistics on the percentage of Universal and Top Level Program data captured.

The Agency participating in HMIS shall commit to abide by the governing principles of HMIS and adhere to the terms and conditions of this partnership in the Agency Participation Agreement. The Agency shall confirm their participation in HMIS by submitting a signed HMIS Agency Participation Agreement to the HMIS System Administrator. The Agency shall inform the HMIS System Administrator in writing of their intention to terminate the HMIS Agency Participation Agreement. The System Administrator will revoke access of the Agency staff to HMIS and will keep all termination records on file with the associated HMIS Agency Participation Agreement. If the System Administrator determines that the Agency is in violation of the terms of the HMIS Agency Participation Agreement, the Agency, HAWNY and possibly the HMIS Advisory Committee will work to resolve the conflict. If unable to resolve conflict(s) and it results in termination the Agency will be notified in writing of the intention to revoke their participation in HMIS. User Authorization & Passwords HMIS Users including the Agency Administrator must have a specific function and purpose for using and entering data into HMIS. The Agency staff participating in HMIS shall commit to abide by the governing principles of HMIS and adhere to the terms and conditions of the Agency Partnership Agreement. The Agency Administrator must only request user access to HMIS for those staff members that require access for business purposes only. All users must have their own user ID and password and should never be allowed to use a user ID that is not assigned to them. Licenses will only be assigned once staff member has completed all required HMIS training. The Agency will not share assigned User IDs and Passwords to access HMIS with any other organization, government entity, business, or individual. The Agency will prohibit anyone with an Agency-assigned User ID and Password from entering offensive language, profanity, or discriminatory comments based on race, color, religion, national origin, ancestry, handicap, age, sex, and/or sexual orientation. The System Administrator will confirm that the Agency provides HMIS workstation(s) that: Has and uses a hardware or software firewall. HMIS Participation. 1. An agency is considered to be an HMIS Participating Agency (“Agency”) when it collects client-level data on homeless clients for an HMIS.
2. By participating in HMIS, the Agency, including its employees, volunteers, affiliates, contractors, and associates, is subject to all data collection, privacy, and security standards set forth in HUD’s Data and Technical Standards as well as any subsequent revisions.
3. The Agency shall provide accurate and timely client data to OHS through direct data entry into HMIS and/or the submission of data extract files for upload into HMIS (data integration). The manner of participation shall be determined by OHS on the program level and shall be based on the program’s status in relation to the following factors:
a. Contractual relationship between OHS and the program; and
b. Program’s existing client-level data collection system.
4. The Agency’s minimum data collection requirements shall be determined on the program level and shall be based on the program’s status in relation to the following factors:
