Supportive Housing. The CONTRACTOR shall have a full-time Supportive Housing Specialist dedicated to this Agreement to work with Members to assess housing needs and identify appropriate resources in order to help them attain and maintain housing. 4.4.1.7.1 The Supportive Housing Specialist shall serve as the internal resource to provide training and technical assistance to the CONTRACTOR’s care coordinators. 4.4.1.7.2 Supportive Housing specifically targets the following populations: • Individuals who are chronically homeless, as defined by the U.S. Department of Housing and Urban Development (HUD) or precariously housed; • Individuals with frequent or lengthy institutional care; • Individuals with serious mental illness or chronic substance use disorders, crisis stabilization, high emergency department or inpatient utilization; • Individuals with frequent or lengthy adult residential care or treatment stays; • Individuals with LTSS and frequent turnover of in-home caregivers or Providers; and • Individuals at highest levels of risk for expensive care and negative outcomes, defined by a Predictive Risk Intelligence System (PRISM) risk score of 1.5 or higher or similar risk measures.

Supportive Housing. Funds allow grantees and supportive housing developers/operators offering services to their residents, to utilize the grant for leasing activities, utilities, maintenance, repairs, supportive services, and overall operations of supportive housing sites. The planning aspect includes capacity building and development costs associated with supportive housing sites but EXCLUDES the ability to use the funds for acquiring, rehabilitating, or converting existing sites. • Services and Case management o Wrap around case management services; o Referrals to resources and education (employment training, financial counseling, youth, health, and tenant) programs. • Planning o Funds can be used to support the planning and development of future supportive housing sites through surveys, partnerships, consultants, and technical assistance services.
Supportive Housing. 1. The SE shall participate in Collaborative supportive housing policy and program development, capacity building and other planning activities to assure effective implementation of supportive housing as requested by the Collaborative and articulated in the Collaborative’s Long Range Housing Plan and/or other Collaborative policy documents. The SE shall identify an individual to serve as the primary, statewide contact with authority to act on behalf of the SE in these activities. 2. The SE shall participate in local capacity building and implementation activities associated with the implementation of this Long Range Housing Plan. The SE shall designate regional and/or local lead contacts who shall be active participants in any Regional or Local Collaborative or local community capacity building initiative related to supportive housing. The SE shall identify the regional and/or local contact to the Collaborative’s Supportive Housing Coordinator. The SE shall assure this role is filled at all times. The regional and/or local contact shall be available on a regularly scheduled basis for implementation activities. 3. The SE shall ensure that the regional and/or local staff are available for briefings and training related to supportive housing to assure the SE has knowledge of local supportive housing development challenges and best practices, fund sources, provider requirements and general supportive housing principles. It is anticipated the SE must make staff available for these briefings and trainings a minimum of four (4) days annually. 4. The SE shall participate in the ongoing education and promotion of the benefits of supportive housing particularly the benefits of exemplary and best practice models, providing information to providers and stakeholders on the Collaborative’s Long Term goals and principles for supportive housing. This includes promotion of the benefits of a cooperative working relationships between providers, Collaborative member agency staff and local housing related organizations (e.g., local and regional Public Housing Authorities, non profit and for profit housing development organizations, Continuums of Care, etc.). 5. The SE shall work closely with the Collaborative’s Supportive Housing Leadership Team and Collaborative member agency staff on development and implementation of practice standards, strategies and requirements for the Collaborative’s continuum of supportive housing including but not limited to outreach and engagement, re...

The Contractor’s identified Supportive Housing Specialist shall be located within one (1) of four (4) regions within the State, and shall be knowledgeable about housing resources in urban, rural, and frontier areas within their specified region. The CONTRACTOR shall provide regular and ongoing comprehensive training, including focus on supporting a Trauma responsive lens throughout the system of care for CONTRACTOR staff to ensure that they understand the goals of Turquoise Care, including the integration of Physical Health, LTC, and Behavioral Health, the provisions and limitations of the ABP, and the requirements of this Agreement. As issues are identified by the CONTRACTOR and/or HSD, the CONTRACTOR shall provide timely and targeted training to staff. The CONTRACTOR shall provide an initial orientation and training as well as ongoing training, including training targeted to different types of staff, to ensure compliance with this Agreement. Targeted training must include: Care Coordination; NF LOC Determinations; Setting of Care (SOC) Submissions; Community Benefit Services and Supplemental Questionnaire; Behavioral Health Services; Trauma-responsive training as approved by HSD; Cultural Competency; The CONTRACTOR shall provide training to all staff within the organization o...

Supportive Housing. Participants will agree to live with other participants and a live-in Arms of Love Coach. Participants will be required to participate in regular house community nights. Each participant will have weekly household responsibilities. Participants are expected to communicate with roommates, treat all housemates with respect as well as comply with requests of the AOL Coach.
