Notification to District a. At least 3 calendar months prior to the estimated delivery date of the child, the employee shall be required to notify the District in writing whether or not the employee intends to take child care leave. This election may be changed at any time before the teacher is no longer disabled from working due to childbirth or pregnancy related disability.
Notification to District. A unit member shall notify the District Human Resources Department of his/her intent to accept or refuse reemployment within ten (10) working days following receipt of a reemployment offer. If the unit member accepts reemployment, the unit member must report to work within ten (10) working days following acceptance of the reemployment offer.
Notification to District. 1. In September of each year during the term of the contract, the Association shall furnish the District in writing with the names of its current officers and business agents or representatives.
Notification to District. The Contracting Officer shall provide the District with the most up-to-date list of presiding and alternate judges at least three weeks before the statutory deadline to order the Election.

The Union shall furnish the District and appropriate department heads and District negotiators with a list of officers and building representatives and alternates and shall, as soon as possible, notify said appropriate District officials in writing of any changes thereto. Only those who are officers and building representatives shall be recognized by the District for the purpose of meetings with the site or building administrator.
Notification to District. It shall be the retiree’s responsibility to keep the District informed regarding the correct subscriber coverage; failure to do so, which results in overpayment to the insurance company, may result in an equivalent credit reduction to the retiree.
Notification to District. Except in case of a military emergency, unit members shall notify the District, at least twenty (20) work days prior to the start of the leave, of the unit member’s intent to take such leave. Further, except in the case of a military emergency, unit members shall submit to the District, at least ten (10) work days prior to the start of the leave, a copy of military orders authorizing such leave.
Notification to District. An employee who is unable to work due to illness shall notify the Human Resources Office prior to his/her scheduled starting time. Any failure to comply may result in a salary deduction for that period.
Notification to District. An employee shall report her/his absence through the District-approved Attendance System at least one hour prior to the scheduled start time. If unable to access the Attendance System, an employee shall notify the Human Resources Office telephonically via the District attendance line prior to his/her scheduled starting time. In the event of an emergency that occurs less than one (1) hour prior to the scheduled start time, the employee shall notify the Human Resources Office through the District- approved Attendance System or telephonically via the District attendance line as well as to his/her supervisor via phone, email, or text message. Failure to report an absence will be considered an unexcused absence as required and shall be subject to investigation by the District.
Notification to District. Contractor shall notify District of all unusual or actual incidents (suspected abuse, injuries, deaths, etc.) affecting District clients within twenty-four hours (24) of occurrence and provide District with a copy of all investigation reports concerning incidents and the disposition of, or corrective action taken to resolve the complaint.
