Provisions of Eligibility a. All Bargaining unit members shall be eligible to be members of the SLB. b. After the start of each school year, each bargaining unit member who is not a member of the SLB will receive an intent form from the Treasurer's Office for the purpose of enrolling in the SLB Initial Membership will consist of one (1) day sick leave, to be designated by the bargaining unit member, to the SLB, on such Form between September 1st and September 15th. c. New members may join by September 15th of each year. Membership shall be continuous unless canceled, in writing, to the Treasurer’s Office during the period of September 1st through September 15th. d. If 30 participants are not enrolled by the initial enrollment deadline of September 15th, the bank will not be established for that school year.

The Association shall provide the names of members participating in the SLB to the Treasurer’s Office no later than ten (10) work days after September 5 each year.

After the start of each school year, all new employees and employees not currently in the bank will receive a written notice of the open enrollment period which will provide an opportunity to enroll in the bank before October 1st. Initial membership will consist of two (2) non-refundable days of sick leave, to be deducted from the employee’s sick leave accumulation and transferred to the S.L.B. prior to October 1. Each employee will receive a notice of receipt indicating his/her participation in the program. c. Employees may join by October 1 of each year. Membership shall be continuous unless canceled, in writing, to the Treasurer's Office during the period of September 1 through October 1. No annual contribution of sick days will be required to maintain membership in the S.L.

After the start of each school year, all new classified employees will receive an intent form from the administration for the purpose of enrolling in the Sick Leave Bank. In addition, an open enrollment period will be made available to classified employees who are not members of the Sick Leave Bank. Initial membership will consist of one (1) non-refundable sick leave day to be designated by the classified employee to the Sick Leave Bank on such form between November 1 and December 1. At such time that the administration has received the classified employee’s intent, it will be recorded and submitted to the Treasurer’s Office. Each classified employee will receive a notice of receipt indicating his/her participation in the program. The donated sick leave day will be deducted the last payday of January. 3. Enrollment in the Sick Leave Bank shall be continuous from year to year until a member withdraws. Withdrawals are accepted only during an enrollment period and only upon written notice by the member to the Sick Leave Bank Committee (SBC) of his/her intent to withdraw. 4. If fifty (50) participants are not enrolled by the initial enrollment deadline of November 25th the bank will not be established for that school year (or) the District administration and association will consider a merger program with the Mad River Local Education Association to allow interested association members to participate.
Provisions of Eligibility. A. All full time certified & classified employees shall be eligible to be members of the E.L.F. 1. Initial voluntary membership will consist of a non-refundable day of sick leave to be applied by the employee to the E.L.F. program upon time of hire. Between November 1, and December 1, future enrollment and withdrawal must be done in writing. 2. Membership shall be continuous unless canceled during the period of November 1 through December 1, or an assessment period. All employees will receive an intent form from the Human Resources Office for the purpose of enrolling in or withdrawing from the E.L.F. At such time that the Human Resources Office has received the employee’s intent form, it will be recorded and submitted to the Treasurer’s Office. Each employee will receive a notice of receipt indicating their participation in the program. Employees not wishing to participate in the E.L.F. will also receive a notice of receipt indicating that they do not wish to participate. 3. New members may join by December 1 of each year or during each assessment period. B. Days of E.L.F. eligibility are governed by years of service in the Middletown School District as follows:

Members will be required to submit the S.L.B. membership form (Attachment A) to the Treasurer no later than October 1 to join the S.L.B. (3) Membership shall be continuous unless canceled by notifying, in writing, the Treasurer's Office during the period of September 1 through September 30. It is understood that the District will automatically deduct three (3) accumulated and unused sick leave days from the bargaining unit member’s sick leave balance each year based upon the member’s continued enrollment. The automatic deduction shall occur on the first pay period following October 1st annually. (4) Should a member have an insufficient sick leave balance to permit the automatic deduction of three (3) accumulated and unused sick leave days to allow continuous enrollment, the member shall NOT be eligible to participate in the S.L.B. and shall forfeit his/her right to any/all days previously donated, but shall have the right to participate in future years provided he/she has a sufficient sick leave balance to participate. (5) A bargaining unit member is required to re-enroll to participate in the S.L.B. should he/she break continuous enrollment. (6) All unused days in the S.L.B. will roll over into the next year's S.L.B. If membership is canceled, donated days remaining in the S.L.B. will not be returned.
