Reporting of Illness. For each occasion of illness the nurse shall be required to report such illness as soon as reasonably possible but no later than two (2) hours prior to the commencement of the scheduled shift in accordance with Hospital procedures. Any nurse who has been absent due to illness shall further be required to report her/his intention to return to work, naming a specific date before she/he actually returns. A nurse who fails to report an illness shall be considered absent without leave.
Reporting of Illness. In any case of absence due to sickness or accident, the matter must be reported as soon as possible to the designated Employer representative in the worksite.

An employee who fails to report an illness shall be considered absent without leave. The Hospital may require the employee to present documented proof of illness.
Reporting of Illness. Each employee is required to report an unplanned absence due to illness or ill dependant at least one (1) hour, unless not reasonably possible, prior to his/her start time. Employees will only be required to make a single phone call in order to report their absence. Each employee will be provided with the phone number he/she must call to report such absence. Each employee is required to indicate whether the absence is due to sickness or ill dependant. He/she is also required to notify of his/her anticipated date of return and will be expected to return to work as reported. He/she will not be required to report daily during the period identified. If the date of return is not specified or known, he/she must report on a daily basis as above.
Reporting of Illness. Where an employee is ill, she must report by telephone or otherwise to an employee of South Hill Child Care Centre, the night before or the morning of the absence. Unreported time loss will be considered unpaid leave, except for emergency situations.

Each employee absent from duty on account of illness or injury must at once notify their immediate supervisor and the employee shall not be entitled to benefits for the time previous to such notice unless delay shall be shown to be unavoidable and satisfactory evidence of disability furnished.

Reporting of Illness.
A. In order to be eligible for sick leave with pay, an employee shall:
1. Call on duty crew as to the reason for absence a minimum of one-hour prior to the time set for beginning the shift of the first day's absence from duty.
2. Keep his/her Supervisor or Fire Chief informed of his/her condition, if the illness/absence will extend beyond one day.
3. Upon return to work, submit a doctor's excuse, if required under section 9.4
4. Where a question exists as to the returning fitness or sick leave abuse, the employee shall be required to be examined by the City's doctor to substantiate the illness or injury. The required doctor visit shall be arranged and paid for by the City.
B. Leave taken shall be reported on the Department's designated Absence Report Form and sent via email to the Fire Chief, Assistant Fire Chief and Administrative Assistant.
Reporting of Illness. For each occasion of illness, the employee shall be required to report as soon as reasonably possible such illness but no later than one and one-half (1.5) hours before the scheduled day shift and two (2) hours prior to the scheduled evening or night shift; absolute incapacity excepted. If the employee does not abide by this time notification, no pay will be allowed. Any employee who has been absent due to illness shall further be required to report his intention to return to work, naming a specific date, before he actually returns and he may be required to show proof of illness. Employees will be required to report their illness by telephone personally, absolute incapacity excepted. An employee who has reported that she will be unable to attend work due to illness will not be required or expected to report until she is well again, subject to a daily reporting until a medical certificate is obtained. This certificate will be obtained after two (2) days of illness or the employer has the right to request a medical certificate for an absence before or after the employee’s vacation.
