Responsibilities of the Organization. The Organization shall provide the student (and the University, as required) with an orientation to its facilities, administrative structure and policies, including any requirements of HI Act or any other applicable legislation, where required, so that the student can effectively and safely participate in the Practicum. The Organization shall ensure that its applicable staff is made aware of the policies, rules and regulations of the University as they apply to the Practicum and the Organization shall take reasonable steps to ensure compliance. During the term of the Practicum, the Organization shall make available facilities to the University and the student(s) such as suitable conference room space on a scheduled basis; and such other materials and equipment as is reasonably required. The Organization agrees to ensure that the student receives adequate, consistent and appropriate supervision, direction and learning opportunities during the Practicum. The Organization recognizes the student status of the student and shall not require the student to undertake activities, responsibilities or duties that exceed the requisite hours of the practicum and/or capabilities of the student. The Organization, acting reasonably and providing timely notification to the University, reserves the right to refuse access to its facilities to any student. The Organization shall notify the University as soon as reasonably possible in the event that student illness, injury, incapacitation or absence that may adversely impact the successful completion of the Practicum. The University requires immediate notification of any situation that may give rise to a claim in affiliation with the Practicum for which the student and/or the University may be legally liable. After notification, and to the extent practical, the Organization and the University shall collectively determine an appropriate response to the matter or issue arising from the student’s Practicum. The parties acknowledge that this Agreement is subject to FOIP, HI Act and all other relevant privacy legislation and all of the terms and conditions of this Agreement shall be carried out in compliance with such legislation and any statutes or laws regarding confidentiality of personal or health care records as may apply in the jurisdiction of the Organization and the location of the Practicum. The Organization hereby agrees to keep confidential any student records or information it may obtain in accordance with the Family Educational Rights and Privacy Act of 1974, as amended, unless it has otherwise obtained prior written consent of the student. 4.5. All medical or health care (emergency or otherwise) that a student requires will be at the expense of the individual involved.

All medical or health care (emergency or otherwise) that a student requires will be at the expense of the individual involved.
Responsibilities of the Organization. At the Facility, the Organization shall: a) Provide a dedicated area reasonably acceptable to Contractor (the “Athletic Training Room”) to perform the Services. b) Provide all reasonably necessary, appropriate and required supplies and equipment required in connection with the performance of the Services upon written request from Contractor. Any supplies and equipment furnished by the Organization and not consumed in the performance of the Services shall be returned to the Organization upon the expiration or earlier termination of this Agreement. For purposes of this Agreement the following items, without any additional written request by Contractor, shall be deemed reasonably necessary for the performance of services: defibrillator, basic training room supplies (e.g., tape, scissors, splints), DME (e.g., crutches). At no time will Contractor or any Athletic Trainer be required to reimburse the Organization for supplies used in connection with the provision of the Services. c) Take such action and adopt such policies as are reasonable and desirable to facilitate communication between the Organization, its coaches and other representatives of its Athletic Department, Athletic Trainer, and Contractor. d) Designate the Athletic Director to serve as Contractor’s primary contact for communication with the Organization. e) Provide Contractor with reasonable access to a telephone in, or in close proximity to the Facility. f) Cooperate with Contractor in all respects to achieve the objectives of this Agreement.

The Organization is fully responsible for the use of IRIS by the Organization, its Primary Account Holder and Organizational User(s) any other Organization representatives, in- cluding but not limited to any use of IRIS that is in breach of this Agreement. 3.2. At any given time, there can only be one active Primary Account Holder within an Organi- zation. Should the Organization's Primary Account Holder change, the Organization will comply with section 3.5 of this Agreement. 3.3. Organizational Users are authorized to be enrolled and use Infrastructure Recipient In- formation System in accordance with the Agreement. 3.4. The Organization must notify Canada if it knows or suspects that 3.4.1. Any representatives of the Organization using IRIS, including but not limited to Primary Account Holder and Organizational User(s), username or password has been or may have been disclosed to anyone; 3.4.2. Any representatives of the Organization using IRIS, including but not limited to Primary Account Holder and Organizational User(s) username, password or account has been or may have been accessed or used by anyone other than that Primary Ac- count Holder or Account Holder; 3.4.3. Any provision of this Agreement has been or may have been contravened whether by act or omission; 3.4.4. Any breach of security that impacts or may impact on the security or integrity of IRIS has occurred. 3.5. In accordance with the process established by Canada and found on the IRIS Portal web- site, the Organization must: 3.5.1. Notify Canada of any change of Primary Account Holder and submit an updated Portal Access Authorization Form to Canada to reflect the change prior to the new Primary Account Holder's use of IRIS; 3.5.2. Have the incoming Primary Account Holder identified and authenticated prior to their use of IRIS; and, 3.5.3. Ensure any new Primary Account Holder and Organizational User(s) appointed in the context of this Agreement agrees with the terms and conditions of this Agree- ment prior to their use of IRIS.
