Self-Certification. For the Consultant and each subconsultant or subcontractor, the following must be completed. Socially disadvantaged individuals are those who have been subjected to racial or ethnic prejudice or cultural bias because of their identity as members of a group without regard to their individual qualities. Economically disadvantaged individuals are those socially disadvantaged individuals whose ability to compete in the free private enterprise system has been impaired due to diminished capital and credit opportunities as compared to others in the same business area who are not socially disadvantaged. Business owners who certify that they are members of named groups (Black Americans, Hispanic Americans, Native Americans, Asian-Pacific Americans, Asian-Indian Americans) are to be considered socially and economically disadvantaged.
Self-Certification. For each Proposed Product, Licensee must self-certify, at Licensee's expense, that such Proposed Product: (i) is compatible with all Compatible Products, as applicable, (ii) meets all requirements of the applicable Licensed Specifications and other Documentation, and (iii) complies with all other requirements set forth in this Use License. Licensee must document its self-certification of each Proposed Product with appropriate reports and supporting materials and will promptly make such documentation available to Apple upon request.
Self-Certification. 3.1 Licensee agrees that all of its products bearing or marketed using the "DIGITALEUROPE UHD Display" Logo shall conform to the applicable "DIGITALEUROPE UHD Display" Minimum Requirements and that all uses of the "DIGITALEUROPE UHD Display" Logo shall fully comply with the "DIGITALEUROPE UHD Display" Logo Usage Guidelines (Annex D).
3.2 Licensee agrees that it will not sell, market, promote or distribute a product bearing the "DIGITALEUROPE UHD Display" Logo that is not in full compliance with the applicable "DIGITALEUROPE UHD Display" Minimum Requirements.
3.3 Licensee agrees that it shall, prior to the initial sale of a product, test the product at its own quality assurance facility according to the "DIGITALEUROPE UHD Display" Testing and Verification Procedure. If, in Licensee’s reasonable judgement, the result of the testing and verification is that such product fails to comply with the applicable "DIGITALEUROPE UHD Display" Minimum Requirements, Licensee shall not sell, market, promote or distribute such non- compliant product with the "DIGITALEUROPE UHD Display" Logo. Licensee shall keep records of the tests conducted in the format as defined in the "DIGITALEUROPE UHD Display" Testing and Verification Procedure. Upon request in writing and on reasonable notice of Licensor, Licensee shall provide these test records to Licensor.
3.4 If at any time during the term of this Agreement, Licensor determines, in its sole reasonable judgement that Licensee is selling, marketing, promoting or distributing a product with the "DIGITALEUROPE UHD Display" Logo that is not in full compliance with the applicable "DIGITALEUROPE UHD Display" Minimum Requirements, then at the request of Licensor, Licensee shall forthwith stop marketing, promoting or distributing the non-compliant product with the "DIGITALEUROPE UHD Display" Logo and withdraw the "DIGITALEUROPE UHD Display" Logo from the non-compliant products.
3.5 If Licensee does not comply with the request of the Licensor as described in Article 3.4 of this Agreement, Licensor shall have the right to terminate the rights relating to the "DIGITALEUROPE UHD Display" Logo granted to Licensee under this Agreement as set forth in Article 9.2 of this Agreement.
Self-Certification. Documentation of Sick Leave Absences of Three (3) Consecutive Work Days (e.g., not including regular days off (RDOs), shift trades off) or Less: A bargaining unit employee’s completed leave request will be used as self-certification for sick leave absences of three (3) consecutive work days (e.g., not including RDOs, shift trades off) or less, unless the bargaining unit employee is on sick leave restriction or management has specific knowledge of bargaining unit employee misuse or abuse of leave.
Self-Certification. The participant file must document participants’ eligibility. Because of the circumstances surrounding the disaster, documentation of eligibility may be difficult to obtain during the initial stages. The Department is prepared to accept an individual's signed certification that they meet the eligibility criteria. The Grantee should have a system in place to
Self-Certification. No later than thirty (30) days prior to the Alternative Format Roll-out Date, each CRA will develop a method for Consumers to certify that they are Persons with a Visual Impairment. The form may include the following language: “I certify that I am a person who is blind or visually impaired within the meaning of the Americans with Disabilities Act.” The self-certification method shall be in the same format in which the Request for Disclosures has been made, or in another format that provides effective communication to the requesting Consumer and will not unreasonably delay processing the Request for Disclosure. This self- certification method will not require proof of visual impairment, as set forth in section 12.6 below, except during periods of Extraordinary Request Volume as set forth in section 12.3 herein. Each CRA will provide the draft self-certification form to Claimants for their review and comment. Claimants will provide their feedback on the draft self- certification within ten (10) days. Each CRA will consider Claimants’ comments in good faith.

For periods of up to three (3) consecutive days, the Agency shall normally consider an employee's self-certification as to the reason for their absence as administratively acceptable evidence. A supervisor may consider an employee's self- certification as to the reason for their absence as administratively acceptable evidence, regardless of the duration of the absence.
Self-Certification. On returning to work after an absence of 7 or less calendar days (including weekends and public holidays), you will be required to complete a self- certification form. Doctor’s Certificate: Where the absence extends beyond 7 continuous calendar days (including weekends and public holidays) you must obtain a certificate from your Doctor/GP, which you must give to your Manager. If you do not return to work following the expiration of your doctor’s certificate you must provide a further doctor’s certificate to your Manager, which should run concurrently. If you are a hospital in-patient, it will be sufficient for medical certificates to be provided to cover the dates of admission and discharge from hospital. Medical certificates will also be required to cover periods of convalescence.
Self-Certification. Tenant shall submit all properly executed Alteration Type II forms or Alteration Type I forms, Department of Buildings Form PW1 and Department of Buildings Technical Report Forms for legal application for permits and sign-offs as required by the scope of the work to be performed
