Documentation of Sick Leave. The District may require documentation from a physician licensed to practice medicine, or a properly authorized physician assistant, as a basis for pay during sick leave after an absence of three (3) consecutive days for personal illness or injury, or as it may deem necessary in other cases.

Documentation of Sick Leave. A. Sick leave for three workdays or less may be approved without a medical certificate. However, when the leave approving official has reason to believe that the employee is not incapacitated for duty, the leave approving official may require medical certificates for any period of absence in accordance with 5 CFR 630, Subpart D. B. An employee requesting sick leave, for periods of illness of more than three consecutive work days must make an appropriate request and may be required to furnish evidence of the need for sick leave within a reasonable period of time after return to duty. Employees who are required to provide a medical certificate or other evidence of illness to justify a sick leave request will be carried in an Absence Without Leave (AWOL) status until acceptable medical documentation is provided. AWOL is not a disciplinary action. It is a non-pay status that may form the basis for a disciplinary action. Medical certificates or other evidence of illness which may be required will be submitted within 15 days after the employee’s return to duty. If, due to circumstances beyond the control of the employee, he or she is unable to provide the documentation within 15 calendar days, the employee must provide the evidence or medical certification no later than 30 calendar days after returning to work. An employee who does not provide the required evidence within 30 calendar days is not entitled to sick leave in accordance with 5 CFR 630.405(b). C. An employee can justify the request for sick leave by the following: 1. By medical certification from the Department’s personnel physician or the employee’s personal physician or health care professional; 2. By the employee’s own written statement in instances where the illness was not treated by a physician. The statement will indicate why a physician was not seen, for example, remoteness of area, nature of illness, or other specific reasons. The supervisor may request clarification should the employee’s written statement not be sufficient to support the request. 3. An employee with chronic medical condition that does not require medical treatment, but does result in periodic absences from work, will not be required to furnish a physician’s certificate on a continuing basis if the employee: a. Is not on leave restriction; and, b. Provides, if requested, an updated valid medical certificate every six months which clearly states the continuing need for periodic absences. 4. Documentation of Sick Leave. The University reserves the rights to require acceptable evidence of illness, injury, or disability before allowing any sick leave benefits. An employee who is (or expects to be) absent from employment shall notify the appropriate supervisor as soon as possible, and, in cases where the absence will be for more than three (3) days, the employee shall notify the supervisor of the anticipated length of absence so that arrangements can be made for the employee's duties to be assumed during said absence.

Information or documents regarding an employee’s absence for sick leave purposes are highly sensitive. Management will ensure that they are maintained in a secure and confidential manner.
Documentation of Sick Leave. When an employee utilizes sick leave for an absence of three (3) consecutive days or more, the employee may be required to furnish a certificate from the employee’s treating medical practitioner stating the nature of the illness, injury, treatment and prognosis. Additionally, if the Employer has reasonable cause to believe potential sick leave abuse is occurring, it may, in addition to any other remedy it may have, require an employee to submit a physician’s statement following any use of sick leave. Abuse of sick leave, falsification of an application for sick leave or a practitioner’s statement, or failure to submit adequate proof of illness or injury may result in the disapproval of sick leave, disciplinary action, or both.

Periods of absence on sick leave in excess of Three (3) consecutive workdays, or for a lesser period when management determines it is necessary, must ordinarily be supported by a medical certificate to be filed within fifteen (15) workdays after return to duty. In lieu of a medical certificate, the employee’s signed statement explaining the nature/purpose of her/his illness may be accepted, when it is unreasonable to require a medical certificate or because the illness does not require the services of a physician. Nature/purpose means whether or not the sick leave absence is because of illness/injury/incapacitation of the employee, care of a family member with a serious health condition or bereavement. For extenuating circumstances such as serious accident or illness, the supervisor will exercise due consideration of the circumstances in enforcing the reporting requirements.
Documentation of Sick Leave. 9.3.1 An employee absent for more than three (3) consecutive work days must supply a physician’s note to be eligible for paid sick leave, unless waived by the Employer. 9.3.2 If an employee fails to submit adequate proof of illness, injury or death of an immediate family member upon request, or in the event that upon such proof as is submitted or upon the report of medical examination, the Board, at its reasonable discretion, finds there is not satisfactory evidence of illness, injury or death sufficient to justify the employee’s absence, such, leave may, at the Board’s discretion, be considered an unauthorized leave and shall be without pay. 9.3.3 Any abuse of patterned use of sick leave shall be just and sufficient cause for disciplinary action, up to and including termination. 9.3.4 The Board may require an employee who has been absent for more than ten (10) consecutive work days, due to personal illness or injury, prior to and as a condition of his/her return to duty, to be examined by a physician designated and paid by the Board, to establish that he/she is not disabled from the performance of his/her duties and that his/her return to duty will not jeopardize the health and safety of other employees or children. 9.3.5 Medical appointments should be scheduled during non-working time if possible. Sick leave may be used for a doctor’s appointment if notice is given to management at least one (1) day prior to the appointment. In an emergency situation, a doctor appointment can be made the same day and verification of the appointment will be required.

Documentation of Sick Leave. Where medical verification is required, it is the employee’s responsibility to provide Human Resources with signed written statement, fully and accurately completed by an attending physician/medical practitioner. If the illness concerns the employee’s health, the signed written statement should document the employee’s related health condition and inability to work. If the employee verification concerns the health of an immediate family member who requires care from the employee, then the verification should be related to the family member’s health needs and the necessity for the employee’s presence. Employees are normally expected to document the use of sick leave within three (3) working days of the inception of their illness. The Employer recognizes that under certain rare circumstances, this may not be possible.

  • Donation of Sick Leave 8.10.1 On a case-by case basis and with mutual agreement between the Association and the District, any bargaining unit member may donate five (5) days (40 hours) of accumulated sick leave to another bargaining unit member who has suffered a long- term, non-industrial related illness or injury and who will exhaust all fully paid leaves. The employee suffering from such illness or designee must request, in writing, donations of sick leave from bargaining unit members only, through the Personnel Office. Requests shall be made prior to the exhaustion of all fully paid leave. Donation of sick leave will not be retroactive. 8.10.2 The Personnel Office will send out the notification of the request for donations of sick leave. The request will be sent for posting on all CUEA bulletin boards and the CUEA President notified of the request. All donations of sick leave will be voluntary with no personal solicitation of donors allowed. The names of any bargaining unit members donating sick leave under this provision will not be made public. 8.10.3 Only bargaining unit members who have a minimum of fifteen (15) days (120 hours) of accumulated sick leave remaining after donating five (5) days (40 hours) of sick leave under this provision will be permitted to participate in this program. Employees eligible to donate sick leave will do so on a District approved form and must submit that form to the Personnel Office. 8.10.4 The maximum amount of sick leave that may be donated to any one person requesting donations under this provision will be equivalent to sixty (60) days. 8.10.5 Donated sick leave will be utilized for the specified employee in the following manner: (A) donated sick leave will be assigned a usage number. The first donated sick leave received by the Personnel Office will be the first sick leave used by the beneficiary. As sick leave is used by the unit member requesting it, the leave time will be charge against the unit member donating the sick leave. (B) if the employee returns to work prior to using all donated days, unused sick leave will be returned to the bargaining unit member donating the sick leave. 8.10.6 Up to an additional twenty (20) days (160 hours) of donated time may be requested in writing to the Assistant Superintendent, Personnel Services by the affected member or his/her immediate family. In this case, Section 8.10.5 procedures will be put into effect. 8.10.7 When all paid leaves of absence have been exhausted, and the unit member is unable to return to work, in lieu of resigning the unit member may elect to do one of the following: take a personal leave without pay pursuant to Section 8.5 or Article VIII for not to exceed the remainder of the school year in which the leave occurs, or retire if eligible pursuant to the provisions of the State retirement system.

  • Verification of Sick Leave The appointing officer or designee to whom application for sick leave is made may make such independent investigation as to the necessity for sick leave as is deemed proper and may require certification for any period of sick leave, provided that the employee has been previously notified in writing that such certification for absence of less than five working days shall be required. The Human Resources Director may at any time make such independent investigation as may be deemed proper regarding the illness of any person on sick leave.

  • Abuse of Sick Leave Misuse of leave, violation of orders, directives, or contractual requirements concerning the use of sick leave and other forms of leave used in lieu of sick leave are cause for disciplinary action.

  • Extension of Sick Leave Unless an employee otherwise requests in writing, an employee who has used up her sick leave credits shall be granted advanced sick leave with pay for a period up to fifteen (15) days subject to the deduction of such advanced leave from any sick leave credits subsequently earned.

  • Use of Sick Leave Sick leave is available the first day of the pay period following the pay period in which it was earned, and is taken in units of one-tenth (1/10) of one hour. Use of sick leave is subject to the approval of the appointing authority. Upon request of an employee, the appointing authority may allow the substitution of three (3) or more days of sick leave for paid vacation, provided the employee furnishes a doctor's statement or other satisfactory evidence that the employee was ill or injured for three (3) or more consecutive days.

  • Accumulation of Sick Leave The unused portion of an Employee's sick leave shall accrue for her future benefit, up to a maximum of one hundred and thirty (130) days.

  • Portability of Sick Leave 1. The employer will accept up to sixty (60) accumulated sick leave days from other school districts in British Columbia, for employees hired to or on exchange in the district. 2. An employee hired to or on exchange in the district shall accumulate and utilize sick leave credit according to the provisions of the Collective Agreement as it applies in that district.

  • Accrual of Sick Leave a. A full-time employee shall accrue four (4) hours of sick leave for each biweekly pay period, or the number of hours that are directly proportionate to the number of days worked during less than a full-pay period, without limitation as to the total number of hours that may be accrued. b. A part-time employee shall accrue sick leave at a rate directly proportionate to the percent of time employed. c. An employee appointed under Other Personal Services (OPS) shall not accrue sick leave.

  • Family Sick Leave An employee may use sick leave credits for family illness or injury only if the employee must provide direct care to an immediate family member. For purposes of family sick leave, “immediate family member” will mean the employee’s parent, spouse, or child, including step-child and xxxxxx child.

  • Uses of Sick Leave a. Sick leave shall be accrued before being taken, provided that an employee who participates in a sick leave pool shall not be prohibited from using sick leave otherwise available to the employee through the sick leave pool. b. Sick leave shall be authorized for the following: i. The employee's personal illness or exposure to a contagious disease, which would endanger others. ii. The employee's personal appointments with a health care provider. iii. The illness or injury of a member of the employee's immediate family, at the discretion of the supervisor. Approval of requests for use of reasonable amounts of sick leave for caring for a member of the employee's immediate family shall not be unreasonably withheld. "Immediate family" means the spouse and the grandparents, parents, brothers, sisters, children, and grandchildren of both the employee and the spouse, and dependents living in the household.
