Sick Leave and Leave of Absence for Health Reasons. (1) Members who are unable to carry out their duties due to accident or physical or psychiatric illness, including drug and alcohol addiction shall be entitled to receive one hundred percent (100%) salary and benefits for the duration of illness or injury up to a maximum of one hundred and eighty (180) calendar days following which time the provisions of the Long Term Disability (LTD) plan shall apply. Members shall notify the Chair /Director who shall notify the Xxxx/Administrator of his/her absence and its probable duration as soon as is reasonably possible.

Membersshall notify their (Students) who shall, as appropriate, notify their Area Xxxx of their absence and its probable duration as soon as is reasonably possible. Where an absence due to illness or injury exceeds five (5) working days, the C (Students)shall notify the Executive Director of Human Resources as to the absence. Members may be required to submit a medicalcertificate to cover illnessesor injurieswhich cause more than five (5) working days absence. The University may require the Member to obtain a second medical opinion from a physician mutually agreed upon by the Executive Director of Human Resourcesand the Member either during a period of sick leave or prior to a return to regular duties. In the event that the Executive Director of Human Resources and the Member are unable to agree upon a physician, the Member shall select from a list, mutually agreed upon by the President of and the Executive Director of Human Resources, containing a maximum of five physicians who are specialists in the areas related to the Such agreement shall not be unreasonably withheld. The Memberwill authorize physician to release medical informationto the physician so selected. The University shall pay the medical costs associated with a second medical opinion. After one hundred and eighty (180) calendar days sick leave, a Member is entitled to a maximum of two (2) years leave of absence for health reasons, without pay if is not eligible for benefits under the Long Term Disability plan. If is replaced while on such leave, replacement's term of appointment may not exceed the length of the sick leave and the Member may not return from sick leave to regular duties before the replacement's appointment has expired. If a member recovers before the termination of the replacement's appointment and is not entitled to benefits under the the Member shall be assigned appropriate responsibilities consistent with Article and after consultation with the Member and the Librarian or Vice-president (Students).

  • Paid Leave of Absence Upon request an employee shall be granted a leave of absence with pay for:

  • SICK LEAVE OF ABSENCE 25.01 Employees, who are permitted to go home due to non- occupational illness or injury, will not be paid for the remainder of their shift.

  • Leave of Absences A leave of absence, subject to the College’s discretion may be granted for a minimum of one term’s absence. All requests for a planned leave of absence from the College, for one term or more, must be submitted in writing to the relevant Head of School for approval at least one full term in advance of the commencement of the leave. The request must include the proposed dates of absence and the reason for the absence. The request can be granted only where all Tuition Fees and Charges and any other amounts owing to the College at the time of the request have been paid in full. For an approved leave of absence, 30% of the applicable Tuition Fees will be charged in advance as a holding fee to guarantee re-entry. This is non-refundable and not offset against future Tuition Fees. In addition to the holding fee, the notebook payout fee will be charged to your Family Account. During a leave of absence, the Student will be provided with a broad outline of the curriculum. A leave of absence will not be considered, nor will Tuition Fees or Charges be refunded where schoolwork is still provided, assessed and supervised and/or reports submitted. On a Student’s final day prior to commencing a leave of absence, any College issued device must be returned to the Centre for Computing and Communications (CCC). Should the parent(s) elect to retain the device throughout the leave of absence period, approval must be granted by the Principal. Upon approval, the full payout fee for the device will be charged.

  • LEAVE OF ABSENCE With respect to the Award, the Company may, in its sole discretion, determine that if you are on leave of absence for any reason you will not be considered as having terminated employment with the Company; however, your rights to the Restricted Stock Units during a leave of absence will be limited to the extent to which those rights were earned or vested when the leave of absence began.

  • Long Term Leave of Absence The District may grant any employee an unpaid long-term leave of absence for up to one (1) year for study, child rearing, travel, medical, or other mutually agreed to reason(s). Upon Board approval leave may be extended an additional year. Employees granted such a leave shall be permitted to stay on the District insurance programs at their own expense (carriers permitting), shall not gain or lose seniority or other benefits, but shall not be granted advancement credit on the salary schedule for the period of the leave. Upon return from such leave, the employee shall be entitled to the same position or a position substantially equivalent to the position held prior to the leave.

  • Return from Leave of Absence (a) Before a Nurse may return to work from a leave granted under Article 9.00, she or he must provide a minimum of four (4) weeks written notice of the specific date of his or her return to work, or such shorter time as mutually agreed.
