Voluntary Checkoff. The Company agrees to deduct from the pay of each associate who authorizes such deduction, initiation fees, and monthly dues, and to remit such deductions to the Local Union once a month, together with a list of the associates showing the amount deducted from the pay of each. The Financial Secretary of the Union shall inform the Company in writing of these charges to be deducted and any change which may be authorized from time to time. Checkoff Authorization: I hereby authorize the Company to deduct from my pay the amount of the initiation fees and monthly dues, as specified by the Financial Secretary of the Local Union, to be remitted to the Local Union each month. Voluntary Checkoff. The Association shall have those rights to have its membership dues and service fees deducted as specified in Government Code Section 3543.1(d). Said deductions shall be made only upon voluntary submission of the Association's Enrollment form to the designated representative of the District. Said form shall be completed and executed by the employee and an authorized Association representative. Deductions for employees who sign such authorization at the beginning or after the commencement of the school year shall be appropriately prorated to complete the payment by the end of the school year. Upon written authorization by the employee, the District shall deduct and make appropriate remittance for annuities, insurance plans, credit union, savings bonds, charitable organizations and other plans or programs jointly approved by the Association and the District. The District shall provide tenthly a list of Association members, dues and service fee deducted and the remittance dues.

For the period of this Agreement, upon receipt of a written personally signed authorization on a form approved by Employer from any employee subject to this Agreement, the Employer will deduct from such employee’s pay, the weekly membership dues, provided, however, that the Employer shall not be obligated to deduct any delinquent dues which became delinquent prior to the effective date of the authorization. The Employer will transmit to the Secretary- Treasurer of the Union on or before the 15th day after the last pay day of each month, the total deductions made by the Employer, together with a list of those employees for whom such deductions have been made.
