SICKNESS AND ACCIDENT PLAN. ‌ Non-Occupational Disability Pay (Short-Term Disability Pay) Section 1. Eligibility‌ Provided the “Conditions of Payment” outlined in Section 2 below are met, an hourly paid employee shall receive weekly, as due, non-occupational disability payments if he or she: (a) has one (1) month or more of continuous service as determined in accordance with the rules set forth in Article 8, Section 2. (b) provides the Company, if it so requests, with a doctor’s certificate as proof that absence was due to a legitimate non-occupational disability. (c) is absent in excess of twenty-four (24) consecutive scheduled work hours. An employee out sick for four (4) consecutive workdays may at his/her option utilize his/her vacation to offset any portion of the twenty-four (24) hour waiting period under this paragraph. An employee who is disabled for twenty-four (24) or more consecutively scheduled workdays, and receives disability pay for the twenty-four (24) hour waiting period under this paragraph may, at his/her option, arrange to repay the vacation pay and have the vacation time reinstated. An employee who opts to reinstate the vacation time should contact his/her supervisor. (d) reports the absence and the cause of absence to immediate supervision and Human Resources at a time of absence. Failure to timely report absence, other than in circumstances outside of employee’s control, may result in disciplinary action due to unreported absence.

SICKNESS AND ACCIDENT PLAN. Those employees with less than ten (10) years service will be treated according to the 8-8 rule. For the first day of sickness, all shall receive the equivalent of four (4) hours paid. If an employee works more than six (6) hours, he/she shall be paid for those hours worked and that day will not be treated as a day of absence. If an employee works less than six (6) hours and must leave due to sickness and/or accident, he/she will be paid for those hours worked and that day will be treated as a day of absence. If an employee works less than four (4) hours and must leave due to accident and/or sickness, he/she shall be paid the equivalent of four (4) hours worked.
SICKNESS AND ACCIDENT PLAN. Sickness and Accident coverage is sixty-six and two thirds percent (66-2/3%)of regular straight time earnings (maximum 39 weeks). Benefits will be paid from the first (1st) day of disability for outpatient or day surgery at a Hospital, the fourth (4th) day of disability for illness. Mandated 5/12ths will continue to be applied to cover ongoing costs associated with benefit improvements.
SICKNESS AND ACCIDENT PLAN. Xxxxxxxx Spring-Kingsville Plant certifies that it has named the Human Resource Clerk as the administrator of the Sickness and Accident claims policy and the Accountant as an alternate administrator should the Human Resource clerk be unavailable. Xxxxxxxx Spring-Kingsville Plant further certifies that it will recognize a new committee formed to consult on disputed issues regarding Sickness and Accident claims. Such committee will consist of the Union Plant Chairperson, a Union Committee person, Human Resource Manager and the Accountant.

In the event the payment from the insurance company is delayed longer than two weeks from claims submission date the credit union will advance the employee an amount equivalent to the benefit. Any overpayment of monies advanced to the employee will be the responsibility of the employee to reimburse the overpayment to the credit union.

SICKNESS AND ACCIDENT PLAN. 34:01 In the event of personal sickness or accident, a seniority employee shall first apply for EI sick benefits for the allowable period of illness or accident. The amount received from EI shall be topped up by the employer to a maximum of 66-2/3% of pre-injury earnings for the eligible period of the claim. 34:02 Individual paid sick days shall cover any incapacity from first day of hospitalization or from the fourth day of illness. 34:03 All health and welfare benefits shall be continued while an employee is off on sick leave for a period of at least one (1) year.

Effective February 1, 2006, employees become eligible for benefits on their first day at work following the date of completion of three (3) months of continuous service. B. For Short-Term Disability claims, where benefits are approved and disability absence commences prior to January 1, 2013, benefits shall commence on the first day of disability and shall continue for the earlier of the length of the disability or until the end of the twenty-sixth (26th) week.
