WAIVER OF LIABILITY AND INDEMNITY AGREEMENT. I understand and acknowledge that the Program presents risks to me and my property, and that Pepperdine cannot guarantee my safety or the safety of my property. I understand and agree that the Program activities may be dangerous and may involve risks to personal safety and physical risks which can range from but are not limited to: minor injuries such as scratches, bruises, and sprains to catastrophic injuries, including paralysis and death. I understand and agree that the Program involves international travel and may involve unfamiliar or different terrain, customs, climate, food and drink, laws, social norms, sports practices, rules and regulations, communications, criminal and law enforcement activities, disability access, driving practices and road conditions, premises conditions and/or maintenance. Risks may also include: exposure to disease and virus outbreaks, inadequate health care, natural disasters, acts of God, war, civil unrest, terrorism, kidnapping and assault, physical violence, verbal abuse, sexual abuse, short and/or long term disability, loss of income and/or career and earning opportunities. Nonetheless, I acknowledge that my participation in the Program is voluntary and that I assume all risks, whether known or

Prior to publicizing or holding an event, CPAR must approve this proposal and use of name or logo. By publicly naming CPAR as the beneficiary of my event, I agree to donate the full amount of the net proceeds within 14 days following the event.
WAIVER OF LIABILITY AND INDEMNITY AGREEMENT. In consideration for being permitted to participate in Quincy Community Theatre (hereinafter “QCT”) classes and/or for my children listed below to participate in QCT classes, the undersigned, on behalf of himself or herself and such participating children and any personal representatives, heirs or next of kin (hereinafter “the undersigned”) hereby acknowledges, agrees and represents that he or she has read the Safety Notice and Rules and will abide by them.

WAIVER OF LIABILITY AND INDEMNITY AGREEMENT. The undersigned (“Volunteer”), for himself/herself, his/her personal representatives, heirs and next of kin, in consideration for being permitted to work as a Volunteer for Give Kids The World, Inc. (GKTW), a Florida not for profit corporation, whether at the GKTW Village, or off site as an airport greeter, runner or otherwise, including in any areas where any activity related to events for GKTW’s guests occur, and other good and valuable consideration, the receipt of which is hereby acknowledged, voluntarily and knowingly executes this Release, Waiver of Liability and Indemnity Agreement (“Agreement”), with the express intention of giving a release and indemnification in favor of GKTW (including their officers, directors, members, agents, servants, employees, and assigns) and giving other covenants and warranties as follows:
WAIVER OF LIABILITY AND INDEMNITY AGREEMENT. The nature of the above referenced activities and the minor’s experience and capabilities (as well as my own), I believe the minor (and/or myself) to be qualified to participate in such activity. I hereby release, discharge, covenant not to sue and AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releasee from all liability, claims, demands, losses or damages on the minor’s account caused or alleged to have been caused in whole or in part by the negligence of the releasee or otherwise, including negligent rescue operations, and further agree that if, despite this release, I, the minor’s parent/legal guardian/xxxx, or anyone on the minor’s behalf makes a claim against any of the above releasees I WILL INDEMNIFY, SAVE AND HOLD HARMLESS the releasee from any litigation expenses, attorney fees, loss liability, damage, or cost any releasee may incur as the result of any such claim.

In consideration for being permitted to utilize the facilities, services and programs of the YMCA for any purpose, including but not limited to observation or use of facilities or equipment, participation in any program affiliated with the YMCA, without respect to location, the undersigned, for himself and herself and any personal representatives, heirs, and next of kin, hereby acknowledges, agrees and represents that he or she has, or immediately upon entering or participating will inspect and carefully consider such premises and facilities or the affiliated program. It is further warranted that such entry into the YMCA for observation or use of any facilities or equipment or participation in such affiliated program constitutes an acknowledgement that such premises and all facilities and equipment thereon and such affiliated programs have been inspected and carefully considered and that the undersigned finds and accepts same as being safe reasonably suited for the purpose of such observation, use or participation. Individuals who are listed on a sex offenders registry are not allowed to participate in any YMCA memberships or program activities. IN FURTHER CONSIDERATION OF BEING PERMITTED TO ENTER THE YMCA FOR ANY PURPOSE, INCLUDING BUT NOT LIMITED TO OBSERVATION OR USE OF FACILITIES OR EQUIPMENT, OR PARTICIPATION IN ANY PROGRAM AFFILIATED WITH THE YMCA, WITHOUT RESPECT TO LOCATION, THE UNDERSIGNED HEREBY AGREES TO THE FOLLOWING:
WAIVER OF LIABILITY AND INDEMNITY AGREEMENT. I understand that participating in the recreational program and/or event selected involves a potential for risk of injury. These risks could include inclement weather, accidents while traveling, equipment problems or failures, contacts with and actions of other participants, slips/trips/falls, and musculoskeletal injuries, among others. I hereby waive, release, and discharge any and all claims for damages for death, personal injury, or property damage which I may have, or which hereafter accrue to me, against the Town of Xxxxxx as a result of my participation in the program and/or event. I acknowledge all risks of injury, illness, and death and affirm that I have assumed all responsibility of injury, illness, or death in any way connected with participation in the program. This release is intended to discharge the Town of Xxxxxx, its agents and elected officials, employees, and any other involved municipalities or public entities from and against any and all liability arising out of or connected in any way with my participation in the activity, even though that liability may arise out of the negligence or carelessness on the part of persons or entities mentioned above. I also agree for myself, and for any child participant, to follow all rules and procedures of the program and to follow the reasonable instructions of the teachers and supervisors of the program. If I am registering a child for a program, I agree that I am a parent, legal guardian, or am otherwise responsible for the child whom I am registering and that I release, waive, and discharge any legal rights that I may assert on behalf of the child participating in the program. In return for the opportunity to participate in this program, I agree for myself and for my heirs, assigns, executors, and administrators to release, waive, and discharge any legal rights I may have to seek payment or relief of any kind from the Town of Xxxxxx, its employees/volunteers, or its agents and agree to indemnify the Town of Xxxxxx for all claims, damages, losses, or expenses, including reasonable attorneys’ fees, if a suit is filed concerning an injury, illness, or death to me or to my child resulting from participation in the program.
